All patients with a cleft lip deformity have an associated nasal deformity that varies in degree of severity. A three-dimensional understanding of the anatomy of the cleft nose aids surgeons in selecting the proper technique for repair. Lengthening short nose is one of the most difficult rhinoplasty problems. Two suggestions are made. First, substantial release of soft tissues is mandatory to lengthen the short nose. Second, the technique espoused involves: (1) releasing the septal mucoperichondrium bilaterally, (2) releasing the septal upper from the lower lateral cartilage, and (3) using a baton graft attached to the septum, which in turn holds the tip cartilages in a more caudal position. Thirteen surgeries were performed with satisfactory results and few complications.

The saddle nose is caused by trauma, inadequate septum resection, or septal abscess. The extramucous method of rhinoplasty offers an ideal and very clear approach to the reconstruction of the cartilaginous septum, to the enlargement of the nasal valve, and thus to the reconstruction of the nasal pyramid. Our aim is, first of all, to use material taken from the nose. If this material cannot be obtained, autogenic cartilage from the rib or allogenic cartilage from the cartilage bank are employed.

The deformities known as saddle nose and boxer's nose result from injuries or poorly planned nasal surgery. Boxer's nose may eventually be corrected using Straith's method. It is possible to correct both deformities by simply using a bone graft. Neither the use of cartilage grafts nor organic or inorganic inclusions are recommended, the former because they tend to deform, and the latter because they are not well tolerated.
