Diabetic foot ulcer risk assessment and prevention in patients with diabetes: a best practice implementation project.

INTRODUCTION AND AIM: Diabetic foot ulcer (DFU) is one of the most serious complications in patients with diabetes. Early identification of risk factors can prevent its occurrence and delay its progression. The aim of this project is to conduct an audit of DFU risk assessment protocols at a large tertiary hospital and evaluate the impact of any changes in compliance with the developed evidence-based best practice criteria. METHODS: Preimplementation and postimplementation audits based on JBI's Practical Application of Clinical Evidence System were conducted at the Department of Endocrinology

UTI in patients with urethral catheters: an audit tool.

This article presents an audit tool for the evaluation of practice relating to urinary tract infection in hospital patients with indwelling urethral catheters. It has been formulated primarily because urinary tract infection is a known complication of catheterization, and because studies have shown practitioners' knowledge in this area to be poor. Although health professionals have an obligation to ensure their practice is evidence based, this requires substantial time and skills in critical appraisal. The standard presented here is based on evidence from an extensive literature review on how

Partnering an implementation strategy with a multi-modal education to improve delirium practices.

AIMS: To improve assessment of and communication for delirium to 85% of eligible patients and to improve nurses' knowledge of and self-efficacy for delirium. BACKGROUND: Detection and prevention of delirium have become a high priority safety and quality issue globally. However, inadequate screening and objective assessment by nurses remains ongoing problems, often attributed to an inadequate knowledge base. METHODS: A repeated cross-sectional audit study, incorporating the 4Es implementation model and a multimodal education program for nurses, was undertaken across four wards of a rural hosp

New method to evaluate the practice of positive pressure ventilation in intensive care units.

BACKGROUND: There is good evidence to support the use of a "protective" ventilation strategy, using small tidal volumes and inspiratory pressures, in patients with acute respiratory distress syndrome. Many general intensive care units in the UK are being slow to adopt this approach and we therefore set out to devise a method to audit ventilation and possibly influence practice in these units. METHODS: Using variables that are routinely documented by intensive care nurses, we assessed the relationship between ventilator settings and arterial blood gas values on 30 consecutive ventilated patien

Urethral catheter audit.

An audit was undertaken in one children's hospital to evaluate practice relating to indwelling urethral catheterisation of children. The records of 47 patients admitted over an eight-month period were reviewed to obtain information regarding catheter use. A questionnaire was sent to 384 qualified nursing staff and 174 medical staff to identify what training they had received and their level of knowledge about catheterisation practice. In the absence of national guidelines for urethral catheter care, locally developed standards were used for comparison in the audit. Timing and location of cathe

Nasogastric tube care and flushing in a Portuguese general medicine ward: a best practice implementation project.

OBJECTIVES: To implement evidence-based practice related to care and flushing of nasogastric tubes inserted for feeding, in a general medicine ward. INTRODUCTION: Nasogastric tube feeding is a widely used nutritional support form. Although performed by qualified professionals, it is not a harm-free intervention. Progression to the trachea during initial insertion, aspiration of gastric contents, improper position for patient feeding, and/or a blocked tube are examples of problems that can occur. METHODS: The project used JBI's methodological approach of the Clinical Evidence System and Getti

Improving postoperative pain management in children by providing regular training and an updated pain therapy concept.

BACKGROUND: In recent years, children's hospitals have increasingly implemented postoperative pain management protocols to reduce postoperative pain and improve patient satisfaction. The effectiveness and long-term sustainability of such protocols have rarely been studied. Therefore, we conducted a prospective intervention study to assess the impact of regular training and improvement of clinical processes on the quality of postoperative pain management. METHODS: We conducted an initial assessment of the status quo of postoperative pain management (Audit 1) followed by repeated training and i

Hand hygiene among nurses in pediatric blood and marrow transplantation setting: a best practice implementation project.

OBJECTIVES: The aim of this project was to promote evidence-based best practice regarding hand hygiene among nurses in a pediatric blood and marrow transplantation (BMT) setting. INTRODUCTION: Healthcare-associated infections (HAI) represent important complications of healthcare with potentially major impact on patient outcomes. Most HAI are transmitted through the hands of healthcare professionals (HCP) and therefore hand hygiene is the simplest, most effective and proven method for preventing HAI. However, the overall compliance rate of HCP with hand hygiene remains at a very low level. In

Alcohol-history taking by nurses and doctors--how accurate are they really?

The aim of this study was to look at how nurses and doctors record alcohol histories on the patients under their care and the frequency and appropriateness of the way they use an alcohol withdrawal scale in the acute hospital setting. To achieve these aims patient records were audited and compared at two points in time, in 1992 and 1994. Results of the study showed that nurses and doctors took alcohol histories from patients on approximately three-quarters of occasions and that this level of recording has not changed over time. The adequacy of alcohol-history taking has, however, increased for

Urinary catheterization in care homes for older people: self-reported questionnaire audit of catheter management by care home staff.

A self-administered questionnaire was used to determine care home staff's reported knowledge of the urinary catheter care standards published by the National Institute for Clinical Excellence (NICE) and the Association of Continence Care, and to see whether this differed in homes with higher catheterization rates. Seven hundred and fifty out of 1438 (52%) nursing and care staff from 37 randomly selected care homes with high, medium and low catheterization rates responded. There was no difference in reported practice in care homes in the three health districts sampled or those with differing ca

Effectiveness of a Nurse-Led Brief Psychosocial Intervention on Harmful Alcohol Use Among Indian Undergraduate Students.

BACKGROUND: Harmful alcohol use among undergraduate students is a rising concern, with implications for physical, psychological, and academic outcomes. Evidence for nurse-led brief psychosocial interventions based on the FRAMES (Feedback, Responsibility, Advice, Menu of options, Empathy, and Self-efficacy) model remains limited in Indian settings. METHODS: A quasi-experimental study was conducted among 330 undergraduate students identified as harmful alcohol users using the Alcohol Use Disorders Identification Test (AUDIT), allocated into experimental (n = 162) and control (n = 168) groups.
