BACKGROUND: Asthma is the most common pulmonary condition encountered during pregnancy. Based on our clinical experience as a pulmonologist and an internal medicine specialist, we frequently manage pregnant women with asthma, often without prior referral from an obstetrician. This study aimed to evaluate obstetricians' clinical approaches to asthma management during pregnancy using a structured questionnaire.
METHODS: A 12-item questionnaire was developed and distributed electronically to obstetricians nationwide. Participation was voluntary, and respondents were assured of full anonymity. Th…

The purpose of this study was to determine the management of postterm pregnancy by contemporary practicing obstetricians. A questionnaire investigating practice patterns pertaining to postterm pregnancy was mailed to 1000 randomly selected American College of Obstetricians and Gynecologists (ACOG) Fellows and Junior Fellows in March 2004. The response rate was 52.2% (522/1000). Statistical analysis included the answers from the 420 practicing obstetricians. Males comprised 55.7% (234/420) of the responding obstetricians. The majority of responding obstetricians (95.4%) rated their residency tr…

Iain Chalmers and the National Perinatal Epidemiology Unit, Oxford, England, developed a comprehensive listing of perinatal care procedures shown to reduce the frequency of adverse outcomes during pregnancy and childbirth. This list was used as a framework for a pilot study conducted in 1992 that reviewed similarities and differences in opinion and practice style between certified nurse-midwives (CNMs) and obstetrician/gynecologists. Twenty CNMs and 57 obstetrician/gynecologists who were active clinical practitioners in San Diego commented on 24 items drawn from Chalmers' work. The CNMs were m

Health-care workers are exposed to risk of HIV infection. Blood and body fluids containing blood are the most important sources of HIV and the major risk for health-care workers occurs by incidental parenteral contact with these fluids. Therefore, the risk can be minimized by taking care to prevent injuries and by using protective barriers for mucous membranes and skin. However, the number of cases reported in literature with occupational HIV infection is extremely low.

BACKGROUND: The midwife specialists (MS) are trained midwifery professionals with advanced knowledge and skills to perform interventions to respond to complicated maternal and neonatal conditions. A midwife specialist is expected to function independently and interdependently with other healthcare professionals such as obstetricians and paediatricians. However, studies have shown that the circumstances within public hospitals could be more favourable to the midwife specialist's independent and autonomous practice.
AIM: This study aims to explore and describe obstetricians' perceptions of midw…

INTRODUCTION: Advances in neonatal care have improved the prognosis in extremely preterm infants. The gestational age considered for active treatment has decreased globally. Despite implemented guidelines, several studies show variability in practice. The aim of this study was to understand theperspectives of Portuguese neonatologists and obstetricians regarding the management of extremely preterm infants.
METHODS: An online survey was sent through the Portuguese Neonatology Society and the Portuguese Society of Obstetrics and Maternal-Fetal Medicine from August to September 2023.
RESULTS: W…

Background: Preeclampsia affects over 4% of pregnancies in the United States. Management of preeclampsia is dependent on the severity of the condition and can range from expectant management to early delivery and inpatient observation. After publication of the hypertension in Pregnancy Task Force guidelines in 2013, little is known about their implementation and acceptance by practicing obstetricians and maternal-fetal medicine (MFM) specialists. Objective: To evaluate Obstetricians' knowledge and practices regarding the management of preeclampsia. Methods: A prospective survey was administered …

Pemphigoid gestationis (herpes gestationis) is an autoimmune bullous dermotosis occuring dining pregnancy or in the post-partum period. The interaction of this rare pathology with pregnancy is underestimated by obstetricians. Authors report one case of pemphigoid gestationis diagnosed in a pregnant woman at term. After a literature review, clinical aspects of this dermatosis and especially its interactions with pregnancy are assessed.

OBJECTIVE: The Royal College of Obstetricians and Gynaecologists (RCOG) defines fetal growth restriction as ultrasound-estimated fetal weight less than the 10th percentile or abdominal circumference less than the 10th percentile; the American College of Obstetricians and Gynecologists (ACOG) defines fetal growth restriction as estimated fetal weight less than the 10th percentile alone. We compared each method's ability to predict small for gestational age (SGA) at birth.

IMPORTANCE: Penicillin allergies are commonly reported in pregnancy, yet patients are rarely truly allergic. Identification of pregnant patients who would benefit from penicillin allergy testing is an important public health initiative to reduce alternative antibiotic usage and reduce associated adverse outcomes.
OBJECTIVE: This article reviews the epidemiology of penicillin allergy, the pathophysiology of allergy in general, in addition to penicillin allergy, the safety of penicillin allergy testing in pregnancy, the impact of penicillin allergy testing on maternal and neonatal outcomes, and

Due to the high risk of morbidity and mortality from unrecognized and untreated pre-eclampsia, clinicians should have a high index of suspicion to evaluate, treat and monitor patients presenting with signs concerning for pre-eclampsia. Early blood pressure management and seizure prophylaxis during labor are critical for maternal safety. Intrapartum, special anesthetic considerations should be employed to ensure the safety of the parturient and fetus. Patients who have pre-eclampsia should be aware that they are at high risk for the future development of cardiovascular disease.

The opioid epidemic in the United States is unprecedented and continues to worsen. Many opioid abusers obtain their pills through legitimate … medication risks, avoid excessive reliance on opioids, and adequately monitor and educate patients. Acute kidney injury (AKI) is a complex disorder that occurs in several clinical settings. During pregnancy, there are additional unique conditions that contribute to AKI. The clinical manifestations of AKI during pregnancy range from a minimal elevation in serum creatinine to renal failure requiring renal replacement therapy, similar to AKI in the general population. Recent epidemiologic studies in the general population show an increase in mortality associated with AKI, particularly when dialysis is required. The incidence of AKI in pregnancy remains a cause of significant morbidity and morta
