Osteonecrosis of the hip (ON) in the adult is still an unsolved therapeutic problem. New pathophysiological findings have increased our knowledge of the disease course. Technical progress in imaging modalities and a better understanding of pathomorphology have improved the diagnostic procedure. The most significant progress for pathophysiology, early diagnosis and therapeutic decision making has been evolved by the routine use of MR-imaging. Using the international ARCO staging system, the pathomorphological and imaging changes during the different ON stages are described in this review. A dia…

BACKGROUND: Since 2003, reports have been published on necrosis of the jaw bones possibly being associated with the administration of bispho … x patients (two women and four men) with a median age of 69 years (range 55-37) were diagnosed with osteonecrosis of the maxilla and/or mandible. These osteonecroses did not react adequately to local treatment and systemic therapy with antibiotics. Four patients suffered from plasmocytoma and two patients had a history of metastasising breast cancer. Besides cy …

We think that osteonecrosis, to a large degree, develops because of familial and acquired thrombophilia and hypofibrinolysis causing venous thrombosis in the femoral head. We postulate that venous thrombosis leads to increased intraosseus venous pressure, reduced arterial flow, and hypoxic bone death. Many studies suggest that familial and acquired thrombophilia and hypofibrinolysis may play an important etiologic role in adults with osteonecrosis of the hip and jaw and in children with Legg-Perthes disease (pediatric osteonecrosis). We have limited pilot data in adults with osteonecrosis asso…

The femoral head is the main location of avascular osteonecrosis. The lesion remains asymptomatic for several months or years before causing non specific hip pain. Risk factors have been identified, mainly femoral neck fractures, corticosteroid therapy and related conditions (lupus erythematosus, organ transplantations), alcohol abuse, dyslipidemia, sickle cell disease, HIV infection, caisson workers, Gaucher's disease, male sex. When typical radiological signs are lacking, MRI is the best investigation. Progression toward hip joint damage highly depends on the necrotic volume assessed at MRI.…

Atraumatic secondary osteonecrosis of the patella is a rare entity and has been reported in only several case reports. The purpose of this study was to define the clinical and radiographic characteristics and outcome of this disease. Osteonecrosis of the patella was found in 25 knees in 19 patients. The mean age was 42 years (range, 21-63 years). Eighteen (95%) patients had greater than 2 g of lifetime corticosteroid exposure, and six (32%) patients had systemic lupus erythematosus. Osteonecrosis was found in the superior pole of the patella in all 23 knees (17 patients) that had magnetic reso…

BACKGROUND: The purposes of this study were to define the clinical, demographic, and radiographic patterns of atraumatic osteonecrosis of the distal part of the femur and the proximal part of the tibia at presentation and to report the outcome of treatment of this condition.
METHODS: Two hundred and forty-eight knees in 136 patients who were younger than the age of fifty-five years were treated at our institution between July 1, 1974, and September 15, 1998, for atraumatic osteonecrosis of the distal part of the femur or the proximal part of the tibia, or both. Demographic and radiographic fe…

OBJECTIVE: To investigate the possible occurrence of osteonecrosis in the acetabulum in patients with non-traumatic necrosis of the femoral head.
DESIGN AND PATIENTS: One hundred and seventy-nine patients with non-traumatic femoral head necrosis were assessed by MRI and radiography for the presence of acetabular necrosis. Three criteria were established to differentiate between osteonecrosis and osteoarthritic changes: (1) heterogeneous morphology and irregular contours of the lesion; (2) typical demarcation lines of osteonecrosis; (3) deficient accumulation of intravenous gadolinium in the a…

This article discusses the case of a 77-yr-old man suffering from osteonecrosis of the femur head. Diagnostic imaging and clinical manifestations are discussed along with the appropriate patient management. If untreated, osteonecrosis of the hip will often result in severe degenerative joint disease. Successful treatment depends on early diagnosis.

Spontaneous osteonecrosis is a common cause of knee pain in older patients, but the diagnosis is often overlooked. Sudden knee pain in older women, with marked joint line tenderness and a decreased range of motion, should alert the physician to the diagnosis. Two to three weeks after the onset of symptoms, plain radiographs will usually be normal but bone scan will be markedly positive. Treatment is initially conservative. Surgical intervention (either osteotomy or arthroplasty) is reserved for patients who develop a large radiolucent lesion in the subchondral femoral condyle.

Clinical and experimental data accumulated within the past 2 decades explain the relationship between fat embolism and osteonecrosis, which now appears to be more causal than coincidental. Evidence for fatty liver, coalescence of endogenous plasma lipoproteins, and/or disruption of depot or marrow fat, all resulting in continuous or intermittent fat embolism, is related to 13 different clinical conditions associated with osteonecrosis, most recently including pregnancy, carbon tetrachloride poisoning, and possibly Legg-Calvé-Perthes disease. Intraosseous fat embolism, then, appears to trigger …

Avascular necrosis (AVN), also referred to as aseptic osteonecrosis (AO), can develop in several osseous districts of the body. Most commonly described is avascular necrosis of the femoral head (AVNFH), a debilitating and progressively disabling condition with partially understood, wide-ranging etiology and pathogenesis. Pathology is chiefly caused by a reduced vascularization of a terminal vascular bed, such as the one perfusing the femoral head, or similar vascular distributions such as femoral condyles, humeral head, the talus, the calcaneus, the navicularis, and other bony structures. Hypo…

Osteonecrosis of the knee can present as a spontaneous, primary (SPON) or a secondary clinical entity (SON). The natural history of SPON follows a course of several sequential stages which seem to be irreversible in later stages of both entities. Early diagnosis of ON is crucial and the earlier the stage of the lesion at the time of diagnosis, the better the prognosis. Clinically, early diagnosis and treatment of ON might prevent unnecessary surgery in cases with a concomitant degenerative meniscal tear. From a medicolegal viewpoint early-stage ON should be ruled out prior to surgery as arthro…
