In this article, we aim to summarize the impacts of COVID-19 on the practice of otologic surgery. Cadaveric studies have indicated COVID-19 viral particles are present in the middle ear mucosa of infected hosts. Otologic procedures can generate significant amounts of droplets due to reliance on high-speed drills. Multiple guidelines have been developed to improve patient and provider safety peri-operatively. Particle dispersion can be mitigated during microscopic mastoidectomy by utilizing barrier drape techniques. The barrier drape may similarly be applied to the surgical exoscope. Endoscopic

OBJECTIVE: To describe a new clinical observation of "tactile dysgeusia," a phenomenon associated with otologic and skull base surgery likely caused by injury to the chorda tympani nerve (CTN) or the nervus intermedius (NI) with subsequent aberrant cross-innervation with somatosensory fibers in the surgical field.
STUDY DESIGN: Descriptive case series.
SETTING: Tertiary university referral center.
METHOD: Eight patients described a sensation of touch-evoked dysgeusia after surgery. Seven patients underwent a variety of middle ear procedures including stapedotomies and mastoidectomies. One p

The translabyrinthine approach is the most direct route to the cerebellopontine angle. It is the preferred approach for removal of all tumors in patients with poor hearing and for large tumors when the likelihood of hearing preservation is slight. This approach offers the advantages of minimum cerebellar retraction, identification of the facial nerve proximally and medially, and the ability to repair immediately the facial nerve if it is severed during acoustic tumor removal. This approach has the lowest morbidity with regard to spinal fluid leaks and also postoperative headaches.

Vestibular schwannomas are benign tumors that usually originate from the balance portion of cranial nerve VIII. The treatment options currently available for vestibular schwannomas include observation with serial imaging, stereotactic radiation, and microsurgical removal. Although the ultimate goal in treatment of vestibular schwannomas is preservation of life, the best option for each patient depends on symptoms, tumor size, tumor location, and the patient's general health and age. Surgical exposure of the cerebellopontine angle for removal of vestibular schwannomas can be performed safely vi

Endoscopic ear surgery is a recently developed technique for minimally invasive middle ear reconstruction. It allows for a panoramic and detailed visualization of middle ear structures, enabling precise procedures to be performed with reduced morbidity. Applicable to a wide range of pathologies (cholesteatoma, tympanoplasty, ossiculoplasty, otosclerosis), it offers anatomical and functional results equivalent to those of traditional microscopic surgery. Despite certain technical challenges (two-dimensional vision, one-handed surgery) and a challenging learning curve, the approach through the e

The use of lasers in microsurgery is now an accepted procedure. As we learn more about the absorption properties of biologic tissue, we can use different electromagnetic wave lengths to accomplish surgical alteration of these tissues. The electromagnetic wave length of the Argon laser is compatible for surgical alteration of the soft tissue and bone of the middle and inner ear. Thirty patients requiring external ear canal or middle ear surgery were treated using the Argon laser during the course of the surgical procedure. The treatment of soft tissue pathology included tympanoplasty with fasci

OBJECTIVE: To describe the practice of otology in America during the first quarter century of the American Otological Society (AOS).
METHODS: Two sources were used to determine the most prevalent disease conditions cared for and surgical procedures undertaken during this era. All articles published in the AOS transactions between 1868 and 1893 were studied as were the otology textbooks published by 6 of the first 10 Presidents of the Society.
RESULTS: The primary emphasis of late 19th century American otological scholarship was on chronic ear infection with numerous articles focusing on comp

Ear surgeries such as mastoidectomy require a clear surgical field with minimal bleeding for optimal visualization and outcomes. However, bleeding during these microscopic and endoscopic procedures can obscure the surgeon's view and potentially impact surgical precision and efficiency. The study aimed to identify the effect of intravenous tranexamic acid during ear surgery. PubMed/MEDLINE, Cochrane Library, and Embase were systematically searched from inception through August 31, 2024. Two authors also manually and independently searched all relevant studies. We included RCTs with (1) patients

OBJECTIVE: Noise-induced hearing loss in the non-surgical ear during otologic/neurotologic surgery has not been well studied. The purpose of this study was to evaluate changes in hearing that may occur in the contralateral (i.e., non-surgical) ear after various otologic/neurotologic surgeries due to noise generated by drills. We hypothesized that otologic/neurotologic surgeries, longer in duration, would suggest longer drilling times and result in decreased hearing in the contralateral ear as evidenced by a change post-operative pure tone air conduction thresholds when compared to pre-operativ

AIM: Day surgery (DS) in otology in France is insufficiently implemented compared to other countries of comparable socio-economic level. The aim of the present study was to evaluate changes in surgical practice in "major otology" cases in a hospital center after launching a dedicated ENT DS unit.
MATERIAL AND METHODS: This new unit, designed in collaboration with the surgeons, was inaugurated in 2014. Number of procedures, patient demographics, surgery durations, and rates of crossover from DS to conventional management were recorded prospectively for the year before and the year after the la

HYPOTHESIS: Higher light intensity settings do not yield improved image quality in endoscopic ear surgery.
BACKGROUND: Light intensity is a parameter with major impact on the quality of digital images. For ear surgery, light produces heat associated with a thermal risk to ear structures and the light source setting should be accordingly optimized.
METHODS: Several series of still images were acquired during live middle ear surgery, using cadaveric and plastic temporal bone models and with three-dimensional printed models. Images obtained under varying light intensities were compared with the

The microscopic transcanal (aka transmeatal) surgical approach was first described in the 60s, offering a minimally invasive means of reaching the external auditory canal, the middle ear, and epitympanon. Such an approach avoids a retroauricular or endaural skin incision; however, working through a narrow space needs angled microsurgical instruments and specific training in otologic surgery. The transcanal approach restricts the working space; however, it offers a binocular microscopic vision into the middle ear without extended skin incisions and thus, reducing post-operative pain and bleedin

Acquired atresia of the external acoustic meatus is a rare pathological condition characterized by obliteration of the medial part of the external acoustic canal by a soft fibrous plug. The present article presents an overview of a series of cases of acquired atresia and stenosis of the external acoustic meatus with the description of etiology, pathogenesis, and methods for the treatment of this condition. In the majority of the cases, atresia develops at the final stage of granulation external otitis with or without an accompanying dermatological pathology. Another common cause of the acquire

CONCLUSION: In the presence of simulated congenital aural atresia with fixation of the ossicular chain to the surrounding bone, access to the chain for placement of a middle ear prosthesis using the rotating burr is not associated with significant stapes movement that would cause vibrational trauma to the cochlea.
OBJECTIVES: To determine the energy transmitted to the cochlea while drilling the mastoid in an ear with simulated congenital aural atresia and fixation of the ossicular chain to the surrounding bone.
METHODS: Eight human cadaveric temporal bones were used. Cement was placed in the

INTRODUCTION: Basic surgical techniques in the treatment of middle ear cholesteatoma include the intact-canal-wall and canal-wall-down tympanoplasty and combined method, i.e. the "mobile-bridge" tympanoplasty. The techniques including reconstruction of the posterior bone wall of the external auditory canal are combination of formerly mentioned methods. These tympanoplasty procedures involve a partial or complete removal of the posterior bone wall of the meatus. Subsequently, after the elimination of pathological process, the reconstruction of the middle ear is performed.
