Functional pain disorders - more than nociplastic pain.

BACKGROUND: Nociplastic pain has been recently introduced as a third mechanistic descriptor of pain arising primarily from alterations of neural processing, in contrast to pain due to tissue damage leading to nociceptor activation (nociceptive) or due to lesion or disease of the somatosensory nervous system (neuropathic). It is characterized by hyperalgesia and allodynia, inconsistency and reversibility, as well as dynamic cross-system interactions with biological and psychobehavioral factors. Along with this renewed understanding, functional pain disorders, also classified as chronic primary …

Pediatric Pain Syndromes and Noninflammatory Musculoskeletal Pain.

Chronic musculoskeletal pain (CMP) is one of the main reasons for referral to a pediatric rheumatologist and is the third most common cause of chronic pain in children and adolescents. Causes of CMP include amplified musculoskeletal pain, benign limb pain of childhood, hypermobility, overuse syndromes, and back pain. CMP can negatively affect physical, social, academic, and psychological function so it is essential that clinicians know how to diagnose and treat these conditions. This article provides an overview of the epidemiology and impact of CMP, the steps in a comprehensive pain assessmen…

Pain Part 5b: Non-Odontogenic Dysfunctional Pain.

UNLABELLED: Orofacial chronic pain provides a significant challenge to all clinicians and the patients seeking treatment for it. Due to the anatomical and regional complexities, diagnosis can be extremely difficult, and due to the lack of cross specialty training, patients will undergo a variety of treatment under different disciplines. Dysfunctional pain provides a unique challenge for patient management and requires a multidisciplinary team. CLINICAL RELEVANCE: Lack of recognition of dysfunctional chronic pain can result in inappropriate dental treatment and further damage. to the patient. …

Pain Part 5a: Chronic (Neuropathic) Orofacial Pain.

Neuropathic pain is a significant social and economic burden. Back pain, joint pain and headaches affect over 30% of the population. Chronic orofacial pain is a common condition and is difficult to diagnose and manage. This two-part paper aims to provide an overview of novel understanding of neuropathic pain, and furnish clinical teams with an update on the less common and less well-recognized chronic orofacial conditions. Headaches and temporomandibular disorders are the most common conditions and are covered in separate papers (6 and 10). Trigeminal neuralgia, burning mouth, and trigeminal a…

[Low back pain vs. leg dominant pain].

There are two patterns of back pain: 1) back-dominant pain and 2) leg pain dominant, greater than back pain. The causes of back pain are very different and numerous, but mostly are due to vertebral, mechanical etiology, and rarely because of non vertebral, visceral etiology. Leg pain greater than back pain is mostly disease of spinal nerve root, generally presented by radicular pain in a dermatomal distribution. Mechanical compression of spinal roots, caused by disc herniation or by spinal stenosis, results in radicular symptoms. Rarely, in about 1% of patients, there are some other reasons ex…

Pain and the brain: lower back pain.

Chronic lower back pain is a common and frequently disabling problem, but an organic cause cannot always be identified. Pain often co-occurs with mood and anxiety disorders, but when patients present with somatic symptoms in the primary care setting, clinicians often overlook mood and anxiety symptoms. Because appropriate treatment includes both pharmacologic and nonpharmacologic strategies, a team approach that includes both mental health and primary care professionals is warranted to alleviate the patient's pain, manage psychiatric comorbidities, and improve the patient's functioning and qua…

Pain and musculoskeletal pain syndromes in adolescents.

The presence of musculoskeletal pain was evaluated in adolescents. Pain was reported by 40% of respondents, benign joint hypermobility syndrome by 10%, myofascial syndrome by 5%, tendonitis by 2%, and fibromialgia by 1%. Logistical regression analysis indicated that sex and age were predictive of pain.

Chronic regional pain and chronic pain syndromes.

This paper seeks to consider the validity and utility of two related terms in spinal and other injuries: complex regional pain syndrome (CRPS) and chronic pain syndrome (CPS). It is argued that the words chronic regional pain syndrome convey neither understanding of the condition nor of its mechanism. They simply redefine the clinical problem, but fail to establish specific diagnostic features or consistent primary pathogenesis. CRPS is best construed as a reaction to injury, or to excessive, often iatrogenic, immobilization after injury; but it is not an independent disease. The diagnosis of …

Pain drawing and myelography in sciatic pain.

In order to assess the predictive value of pain drawings of sciatic pain, 81 patients admitted because of symptoms and signs of lumbar disc herniation were asked to chart their pain on a schematic body drawing prior to myelography. There were no differences in sexes, ages, or pain duration between the group of 37 patients with a herniated lumbar disc compared to the 44 patients with a normal myelogram. From the features of the pain drawing alone, it was possible to predict the presence or absence of a herniated disc with a high statistical significance (p less than 0.001). Patients with a disc…

Conditioned pain modulation in patients with nonspecific chronic back pain with chronic local pain, chronic widespread pain, and fibromyalgia.

Findings considering conditioned pain modulation (CPM) in chronic back pain (CBP) are contradictory. This might be because many patients with CBP report pain in further areas of the body, and altered CPM might influence spatial extent of pain rather than CBP per se. Therefore, we compared CPM in patients with CBP with different pain extent. Patients with fibromyalgia syndrome (FMS), for whom CPM impairment is reported most consistently, were measured for comparison. Based on clinical evaluation and pain drawings, patients were categorized into chronic local back pain (CLP; n = 53), chronic wid…

Refractory Orofacial Pain: Is It the Patient or the Pain?

AIMS: To highlight and discuss the term "refractory" when used to describe pain conditions and its application to orofacial pain, as well as to highlight the factors that must be considered in a refractory patient. METHODS: A scoping review of recent publications (2010 to 2021) applying the term "refractory" to orofacial pain was conducted, and this paper presents their limitations and definitions. RESULTS: The term "refractory" is often used to describe pain instead of "persistent" or "nonresponsive." There are clear definitions in the use of refractory for migraine, cluster headaches, and …

On the Origin of Pain - the 'Pain Channel' Hypothesis.

Theories of pain have traditionally attributed the sensory experience of pain to the brain. We present here a new hypothesis on the origin of pain which is based on a novel approach to the management of persistent pain. We call it the 'pain channel' hypothesis of the origin of pain. There are key components to the development of our hypothesis: [1] Our clinical outcome of a persistently pain-free state, representing a maintained pain score of 0/10 has been achieved in a growing cohort of various presentations of persistent pain now exceeding 130 patients over the course of the last five years.…
