Mesenteric Panniculitis.

BACKGROUND: Mesenteric panniculitis (MP) is an uncommon non-neoplastic idiopathic inflammation of adipose tissue, mainly affecting the mesentery of the small intestine, with its etiology remaining largely speculative. The difference in prevalence of MP among females and males varies across multiple studies. In most cases, MP is asymptomatic; however, patients can present with nonspecific abdominal symptoms or can mimic underlying gastrointestinal and abdominal diseases. The diagnosis is suggested by computed tomography and is usually confirmed by surgical biopsies if necessary. Treatment is ge…

Mesenteric panniculitis.

A 53-year-old black woman presented with a 3-day history of abdominal pain. Ultrasound of the abdomen showed a gall bladder packed with small stones. She gave a history of abdominal surgery for a gynaecological condition. She had a cholecystectomy done, but her symptoms continued after cholecystectomy. She then had anendoscopic retrograde cholangiopancreatogram (ERCP) and sphincterotomy done, again her symptoms remained the same. A CT scan of the abdomen was done, and mesenteric panniculitis was suspected. A laparoscopic biopsy of the mesentery was performed, and it confirmed mesenteric pannic…

Pancreatic panniculitis.

We describe a rare case of a patient with pancreatic adenocarcinoma who presented initially with a rash on her lower legs. Skin biopsy showed lobular panniculitis and characteristic "ghost" adipocytes consistent with pancreatitic panniculitis. This clinical case is an interesting example where a seemingly innocuous skin condition heralds an underlying malignant disease process.

Pancreatic panniculitis.

Pancreatic panniculitis (PP) is a rare variant of panniculitis characterized by subcutaneous fat necrosis, that affects 0.3-3% of patients across a range of different pancreatic disorders. It presents with painful, tender, erythematous to violaceous nodules that may undergo spontaneous ulceration and discharge of an oily brown, viscous material, resulting from liquefactive necrosis of adipocytes. These lesions usually involve the lower extremities, although may also spread over the buttocks, trunk, arms and scalp. In addition to the skin, fat necrosis may involve periarticular, abdominal and i…

Pancreatic panniculitis.

Pancreatic panniculitis is an uncommon complication of pancreatic disease, most frequently pancreatitis and pancreatic carcinoma. The pathogenesis of the process remains unknown, but possibly the release of pancreatic enzymes may induce permeability of the microcirculation and cause fat necrosis. Clinically, pancreatic panniculitis presents with tender, ill-defined, red-brown nodules in the lower extremities that may ulcerate and drain an oily substance and usually precedes pancreatic disease. The histopathologic picture consists of a mostly lobular panniculitis without vasculitis, with the pr…

Infective panniculitis.

Infective panniculitides are infections of the subcutaneous fat induced by any kind of micro-organism. They have rarely been considered as an entity within the spectrum of the panniculitis. Because of the increase in the immunosuppressive population, cutaneous infections' incidence is growing and atypical clinical presentations can be found. In this article, we analyze the etiology, clinical picture, histopathologic findings, diagnostics tools, and treatment of the more relevant infective panniculitis. We divide them according to the causative micro-organisms in bacterial, mycobacterial, funga…

Traumatic panniculitis.

Traumatic panniculitis refers to changes in the subcutaneous fat related to physical or chemical agents. The clinical picture of traumatic panniculitis is nonspecific. Cutaneous lesions are indurated, warm, red, subcutaneous plaques or nodules not necessary related to the intensity of the injury. The histologic picture includes fat microcysts surrounded by histiocytes, collections of foam cells, and inflammatory cells. Late lesions may show fibrosis, lipomembranous changes, or dystrophic calcic deposits. Traumatic panniculitis is usually a self-limiting disorder and requires only symptomatic t…

Cold panniculitis.

Cold panniculitis has been described in children and young women following cold exposure. Histopathologically, cold panniculitis shows a mostly lobular panniculitis, which consists of an infiltrate of lymphocytes and histiocytes in the fat lobules. Usually, the dermis shows a superficial and deep perivascular infiltrate mostly composed of lymphocytes, with no vasculitis. Inflammation is most intense at the dermal-subcutaneous junction. Differential diagnosis of cold panniculitis should be established with subcutaneous fat necrosis of the newborn, sclerema neonatorum, poststeroid panniculitis, …

Factitial panniculitis.

Factitial panniculitides are subcutaneous tissue injuries produced by external agents or actions. In most cases, factitial panniculitis is caused by self-injection of different substances. Factitial panniculitis can also appear as an iatrogenic consequence of injections of drugs or immunization agents. The clinical features of factitial panniculitis are quite variable, depending on the inciting agent. The histopathology of factitial panniculitis usually shows a pattern of an acute lobular panniculitis associated with fat necrosis and an abundant inflammatory infiltrate predominantly composed o…
