The aggressive surgical management of hypercalcemia due to ectopic parathormone production.

A patient with slowly progressive metastatic renal cell carcinoma had life-threatening hypercalcemia secondary to ectopic parathormone production. Aggressive surgical debulking resulted in prolonged control of the hypercalcemia. This case illustrates the value of this surgery in the control of paraneoplastic endocrine syndromes refractory to medical management in patients with slow-growing metastatic disease.

Modulation of autonomic balance by tumors and viruses.

Autonomic balance, a function generally under host control, is subject to modulation by other signalers. In some cases, modulation of host autonomic function through behavioral and physical stressors exerted by another individual may have negative consequences for the stress recipient by inducing sympathetic bias. Modulation of autonomic function may sometimes benefit one party at the expense of another. Tumors and HIV are examples of illegitimate signalers who may induce host sympathetic bias to promote their own growth and evade host immune surveillance. Paraneoplastic and paraviral syndrome

A Case of Ulcerative Colitis Induced by Paraneoplastic Syndrome?

BACKGROUND: Paraneoplastic syndromes often cause endocrine, neurological, cutaneous, and hematologic pathologies, and cases with digestive symptoms as prominent cases are rare. CASE DESCRIPTION: A 1-year-old child admitted to the emergency department with severe abdominal distension was later diagnosed with sacrococcygeal yolk cystoma with ulcerative colitis. After symptomatic management, surgical removal of the tumor, and JEB chemotherapy, the symptoms of ulcerative colitis disappeared completely. After 7 years of follow-up, the child grew and developed well, and there was no recurrence of t

HIF2 drives PTHrP-mediated cancer cachexia.

Cachexia and hypercalcemia frequently complicate advanced renal cancer. A recent Nature Medicine study by Abu-Remaileh et al. shows that pharmacologic hypoxia-inducible factor 2 (HIF2) inhibition rapidly suppresses parathyroid hormone-related protein (PTHrP), reverses metabolic wasting, and normalizes calcium levels, redefining these paraneoplastic syndromes as targetable endocrine disorders rather than secondary consequences of tumor burden.

[Etiology of ectopic hormone producing tumors].

Inappropriate secretion of peptide hormones and other associated proteins by tumors is the most common cause of paraneoplastic syndromes. Sensitive assay techniques have permitted detection of polypeptide hormone secretion by tumors even in the absence of clinically overt syndromes. Moreover, it has been demonstrated that most or all nonendocrine tissues produce small amounts of a variety of peptide hormones and hormone precursors. Nevertheless, the term ectopic hormone syndromes occurring with nonendocrine neoplasms is well established and widely used. We summarize etiology of 'classic' ectop

Paraneoplastic syndromes in patients with oral cancer.

The diagnosis of a paraneoplastic syndrome (PNS) may precede, follow or be concurrent with the diagnosis of a malignant tumor. There is increasing knowledge of association between PNS and head and neck cancers, but the relationship between oral cancer and paraneoplastic disease has not been previously investigated. PNS associated with head and neck cancer can be divided into six main groups: endocrine, cutaneous or dermatologic, hematologic, osteoarticular or rheumatologic, neurologic, and ocular syndromes. We have comprehensively reviewed the literature to evaluate the incidence of occurrence

Pulmocrinology: Interplay of pulmonary and endocrine diseases.

The interplay between pulmonary and endocrine systems modify and influence the pathogenesis and manifestation of several disease processes. Endocrine dysfunctions predispose to numerous pulmonary disorders, including various respiratory infections. On the other hand, pulmonary conditions like chronic obstructive pulmonary disease and obstructive sleep apnoea can produce critical metabolic and endocrine derangements. Varied manifestations such as primary adrenal insufficiency, hypophysitis and hypercalcaemia can result from chronic granulomatous conditions like tuberculosis and sarcoidosis. Var

Endocrinopathy and ectopic hormones in malignancy.

This article reports progress in the field of endocrinopathies and focuses on the molecular aspects of these diseases. Implications for genetics and metabolic study are presented. Although limitations of earlier approaches are confirmed, progress is noted, particularly with regard to the contribution of octreotide scintigraphy. Integrated with the evolving applications of molecular insights, significant clinical progress has been recorded.

Toe necrosis and acute myocardial infarction precipitated by a pheochromocytoma in a patient with systemic sclerosis.

Systemic sclerosis (SSc) patients typically experience Raynaud phenomena that is often complicated by digital ischemic lesions, gangrene, and digital loss. Other causes of peripheral ischemia, such as atherosclerosis, cryoglobulinemia, antiphospholipid syndrome, myeloproliferative disorders, paraneoplastic syndromes, and hyperadrenergic endocrine conditions, may be masked in SSc patients. We present a woman with limited SSc who developed toe necrosis and acute coronary events as a complication of a previously undiagnosed pheochromocytoma.

Paraneoplastic syndromes in patients with nasopharyngeal cancer.

Paraneoplastic syndromes (PNS) represent the clinical manifestation of the remote and indirect effects produced by tumor metabolites or other products. Paraneoplastic effects are not directly mediated by tumor invasion of normal tissue, or by the disruption of normal function of the involved organ, or by distant metastases. More than 260 cases of nasopharyngeal carcinoma (NPC) associated with PNS have been reported in the literature. These syndromes can be divided into six main groups: cutaneous or dermatologic, endocrine, hematologic, osteoarticular or rheumatologic, neurologic, and ocular. T

Paraneoplastic syndromes: A focus on pathophysiology and supportive care.

PURPOSE: This article aims to increase awareness of, outline pathophysiology for, and offer guidance on supportive care strategies for specific endocrine, neurological, and immunological syndromes associated with paraneoplastic syndromes (PNSs). SUMMARY: PNS refers to remote effects that cannot be attributed to the direct or invasive effects of a malignancy. These syndromes are considered clinically important because they may provide early recognition, diagnosis, and management of the malignancy in a timely manner. Many of their presenting symptoms such as ectopic Cushing's syndrome, hypercal

Thyrotoxicosis and Impending Thyroid Storm: A Rare Paraneoplastic Syndrome in an Infant With Hepatoblastoma.

Graves' disease is the most common cause of pediatric hyperthyroidism and thyrotoxicosis. Thyroid storm is a rare initial manifestation of Graves' disease and represents an endocrine emergency. We report a case of transient hyperthyroidism, possibly a paraneoplastic syndrome presenting as impending thyroid storm in a patient with undiagnosed hepatoblastoma. To our knowledge, this is the first case of this association reported in children. A previously healthy 21-month-old male presented with abdominal pain and unremitting tachycardia. He was managed for thyrotoxicosis and impending thyroid sto

The insidious development of symptomatic secondary hormone syndromes in patients with malignant endocrine tumors.

Endocrine tumors may produce secondary or "ectopic" hormones that cause paraneoplastic syndromes. Such syndromes may be confused with more common complications related to a patient's tumor, and thus escape detection and appropriate treatment. The secondary hormone secretion responsible for these syndromes often occurs late in the course of such diseases and presents in an insidious manner. Two patients are presented that illustrate these points. The first, a woman with medullary carcinoma of the thyroid (MCT), developed a syndrome secondary to ACTH secretion that was confused initially with th

A case of suspected paraneoplastic nerve syndrome associated with prostate cancer or opsoclonus-myoclonus syndrome associated with COVID-19 infection, but symptoms improved after treatment of both.

INTRODUCTION: Paraneoplastic neurological syndrome is a type of neurological syndrome that occurs in patients with cancer; however, it is rarely associated with prostate cancer. We herein report a rare case of this condition. CASE PRESENTATION: A 76-year-old man, treated conservatively for rotatory vertigo due to a subtype of Guillain-Barré syndrome after COVID-19, was referred to our neurology department. Magnetic resonance imaging showed no obvious findings; however, paraneoplastic neurological syndrome was suspected due to opsomyoclonus, and close examination of the primary tumor revealed

Unusual Imaging Findings Associated with Germ Cell Tumors.

Germ cell tumors, because they contain immature and mature elements, can differentiate into different tissue types. They can exhibit unusual imaging features or manifest in a syndromic fashion. The authors describe these features and assign them to one of the following categories: (a) unusual manifestations of metastatic disease (growing teratoma syndrome, choriocarcinoma syndrome, ossified metastases, and gliomatosis peritonei); (b) autoimmune manifestations (sarcoidlike reaction and paraneoplastic syndromes); (c) endocrine syndromes (sex hormone production, struma ovarii, and struma carcinoi
