Genetic ablation of parathyroid glands reveals another source of parathyroid hormone.

The parathyroid glands are the only known source of circulating parathyroid hormone (PTH), which initiates an endocrine cascade that regulates serum calcium concentration. Glial cells missing2 (Gcm2), a mouse homologue of Drosophila Gcm, is the only transcription factor whose expression is restricted to the parathyroid glands. Here we show that Gcm2-deficient mice lack parathyroid glands and exhibit a biological hypoparathyroidism, identifying Gcm2 as a master regulatory gene of parathyroid gland development. Unlike PTH receptor-deficient mice, however, Gcm2-deficient mice are viable and ferti…

[The thyroid gland, the parathyroid gland and the skin].

Hormones of the thyroid gland (thyroxine, tri-iodothyronine) control the metabolism of cells and tissue of the body, while parathormone and calcitonine are balancing the intra- and extracellular levels of calcium and phosphorus by governing some metabolic functions of bones, kidney and small intestine. Growth, maturation and metabolic homeostasis of the organism depend, among other intrinsic factors, on a normal production and secretory rate of both thyroidal and parathyroidal hormones. Clinical conditions of hyperthyroidism induce 1. increased metabolic turnover of the body with transcutaneou

The parathyroid glands and parathyroid hormone: Insights from PTH gene mutations.

Nine mutations have been discovered in the parathyroid hormone (PTH) gene since it was initially sequenced in 1983. An autosomal dominant C18R mutation in the signal peptide was first reported in 1990, followed by an exon skipping mutation, leading to loss of exon 2 in 1992; the latter mutation prevents PTH biosynthesis, as exon 2 contains the initiation codon. The S23P and S23X mutations affecting the same residue were reported in 1999 and 2012, respectively, while in 2008, the somatic mutation, R83X, was detected in a parathyroid adenoma tissue sample from a patient with overt hyperparathyro

Leptin Is Produced by Parathyroid Glands and Stimulates Parathyroid Hormone Secretion.

OBJECTIVE: We asked if leptin and its cognate receptor were present in normal and diseased parathyroid glands, and if so, whether they had any functional effects on parathyroid hormone (PTH) secretion in parathyroid neoplasms. BACKGROUND: The parathyroid glands acting through PTH play a critical role in the regulation of serum calcium. Based on leptin's recently discovered role in bone metabolism, we hypothesized these glands were the sites of a functional interaction between these 2 hormones. METHODS: From July 2010 to July 2011, 96 patients were enrolled in a prospective study of leptin an…

Parathyroid Pathology Uncovered: Tiny Glands, Big Questions.

Parathyroid neoplasms encountered in routine practice are most often diagnostically straightforward and can usually be classified on the basis of conventional histomorphology without the need for extensive biomarker work-up. This relative simplicity has contributed to the perception that pathologic assessment of parathyroid tumors is largely uncomplicated. Yet this view rapidly changes when a lesion exhibits atypical features beyond those expected in a conventional adenoma, particularly when the differential diagnosis includes parathyroid carcinoma, an exceptionally rare but potentially fatal

[Hyperplasia and tumors of the parathyroid glands].

Hyperparathyroidism is mainly caused by hyperplasia or adenomas of the parathyroid glands. Formerly, the therapeutic strategy depended on the morphological diagnosis (preferentially already made intraoperatively by examination of frozen sections). Today, however, the extent of parathyroid surgery is in many cases guided both by substantial improvements in the preoperative clinical diagnosis as well as the intraoperatively determined decrease in the serum parathyroid hormone level. Nevertheless, the pathologist should be familiar with the differential diagnostic criteria of parathyroid hyperpla…

Abnormal parathyroid glands: high-resolution MR imaging.

High-resolution magnetic resonance (MR) images of the neck were obtained in 23 patients with hyperparathyroidism. Surgical confirmation was available in all cases. MR imaging allowed accurate localization of 14 of 17 parathyroid adenomas, two of two parathyroid carcinomas, and five of eight hyperplastic glands, the latter from three patients. MR depicted nine of 11 (82%) abnormal glands that were 0.5-1.0 cm in size and 12 of 14 (85.7%) glands larger than 1 cm in diameter. In all but one case, abnormal glands had signal intensity greater than or equal to that of fat on spin-echo images with lon

Retrieval of the parathyroid glands during thyroidectomy.

A recommended technique of intraoperative search of the removed thyroid lobe or entire gland for attached parathyroid glands is presented. Diligent examination of the thyroid capsule and folds of the thyroid gland is made in the sterile operative field. Any recovered parathyroid gland or glands are then finely sectioned and reimplanted in a location which is not likely to be violated in any future surgical procedure. This technique has significantly reduced the incidence of permanent hypoparathyroidism after total thyroidectomy.

Beta adrenergic receptors in the parathyroid glands.

The location of beta-adrenoceptors in human parathyroid gland was studied using an immunohistochemical method. Frozen sections of human parathyroid glands, taken from surgical samples, were treated with (-)-alprenolol, washed and exposed to (-)-alprenolol antibodies conjugated with fluorescent dyes. The (-)-alprenolol was bound to the parathyroid principal cells and to the main blood vessels. On the contrary, adrenergic nerve fibres, demonstrated with formaldehyde fluorescence technique, were only located within the walls of main blood vessels. The findings are discussed.

Mast cells in parathyroid glands of hyperparathyroidism.

Parathyroid glands from cases of hyperparathyroidism have been examined with particular attention to mast cells. In ;normal' glands accompanying an adenoma they were common, although there was a wide range in their frequency, whereas in pathological tissue, both adenoma and hyperplasia, mast cells were scarce. This difference between glands was examined with respect to alterations in gland size, to variations in the amount of interstitial tissue, to degrees of overactivity, and to anatomical development of the glands. No conclusions can be drawn with regard to the significance of mast cells in…

THE ANTAGONISM BETWEEN THYROID AND PARATHYROID GLANDS.

From the facts stated in this paper it is evident that the thymus gland of mammals contains a substance which is capable of producing tetany when fed to the larvae of certain species of salamanders (Ambystoma opacum and Ambystoma maculatum). As long as the larvae have not developed their own thymus glands, they are able, by means of some mechanism, to counterbalance the tetanic action of the thymus substance introduced in their food. When, however, the secretion from their own thymus glands is added to the thymus material introduced with the food, this mechanism of preventing tetany becomes in

[Surgery of the parathyroid gland].

The introduction of various techniques for minimally invasive parathyroidectomy (MIP) has changed both the conceptual and surgical approach to parathyroid disease. The perceived advantages of minimally invasive surgery among both clinicians and patients have been a major factor in the development of new surgical techniques, as well as refinement of preoperative localization techniques such as high-sensitive ultrasound and technetium sestamibi scanning. MIP for primary hyperparathyroidism has become an accepted part of endocrine surgical practice worldwide. In recent years, medical management o

[Imaging of the parathyroid gland].

Diseases of the parathyroid gland are rare and are especially in case of hyperparathyroidism associated with hyperacalcaemia. Enlarged glands are, therefore, generally not detected with imaging modalities. The aim of imaging studies is to locialize abnormal glands. Ultrasound is the modality of choice for the primary investigation followed by nuclear medicine studies. Computed tomography or angiography with vein sampling should only be performed if the results of other investigations are equivocal. Magnetic resonance imaging is performed rarely in Austria with its high prevalence of endemic go

Parathyroids: the forgotten glands.

Symptomatic calcium disorders--most notably, hypocalcemia--are common problems in preterm and certain term infants. Many factors predispose susceptible neonates to hypo- or hypercalcemia, but in many cases, the root of the problem is altered function of the parathyroid glands. Parathyroid gland dysfunction may affect calcium homeostasis and alter physiologic functioning, resulting in significant clinical symptoms. A review of how the parathyroid glands and parathyroid hormone affect calcium balance and of the problems that result from altered function promotes a better appreciation of the impo…
