Differences among Research Domain Criteria score trajectories by Diagnostic and Statistical Manual categorical diagnosis during inpatient hospitalization.

With brief psychiatric hospitalizations, the extent to which symptoms change is rarely characterized. We sought to understand symptomatic changes across Research Domain Criteria (RDoC) dimensions, and the extent to which such improvement might be associated with risk for readmission. We identified 3,634 individuals with 4,713 hospital admissions to the psychiatric inpatient unit of a large academic medical center between 2010 and 2015. We applied a natural language processing tool to extract estimates of the five RDoC domains to the admission note and discharge summary and calculated the chang

Evaluation of the extent of under-reporting of serious adverse drug reactions: the case of toxic epidermal necrolysis.

INTRODUCTION: Toxic epidermal necrolysis (TEN) is a life-threatening adverse drug reaction (ADR) that is primarily the result of drug exposure (incidence 0.4-1.3 per million person-years). Life-threatening ADRs such as TEN should be reported to ADR monitoring programmes, which collect reports for suspected ADRs and alert the public and medical practitioners to new drug hazards. In Canada, reports are made to the Canadian Adverse Drug Reaction Monitoring Program (CADRMP). OBJECTIVE: To examine the extent of under-reporting for TEN in Canada. DESIGN: A retrospective case series design was used

Outcomes and Safety of Same-Day Discharge Following Minimally Invasive Abdominal Surgery: A Systematic Review.

Same-day discharge (SDD) following minimally invasive abdominal surgery has emerged as a paradigm shift in perioperative care, aiming to enhance patient comfort and optimize healthcare efficiency. This systematic review evaluates the safety and outcomes of SDD after minimally invasive abdominal procedures. A comprehensive literature search was conducted using PubMed, Google Scholar, and the Cochrane Library for studies published between January 2015 and December 2025. The quality of randomized controlled trials and non-randomized studies was assessed using the Cochrane Risk of Bias 2.0 tool an

Hospital initiation of benzodiazepines and Z-drugs in older adults and discontinuation in primary care.

OBJECTIVE: To examine factors associated with continuation of hospital-initiated benzodiazepine receptor agonists (BZRAs) among adults aged ≥65 years, specifically instructions on hospital discharge summaries. METHODS: This retrospective cohort study involved anonymised electronic record data on prescribing and hospitalisations for 38,229 patients aged ≥65 from forty-four GP practices in Ireland 2011-2016. BZRA initiations were identified among patients with no BZRA prescription in the previous 12 months. Multivariate regression examined whether instructions on discharge messages for hospital

Comparison of physician-authored and artificial intelligence-generated after-visit summaries: A blinded comparative study.

BACKGROUND: After-visit summaries (AVSs) are essential for a safe hospital discharge, yet are often written above literacy levels, omit key information, and are produced under substantial clinical time pressure. Large language models (LLMs) offer a potential solution, while performance and safety in real clinical workflows remain uncertain. OBJECTIVE: To compare the quality, understandability, actionability, and safety of LLM-generated versus physician-authored after-visit summaries for hospitalized patients. METHODS: We conducted a retrospective, blinded comparison of physician-authored AVS

An evaluation of copy and paste events in electronic notes of patients with hospital acquired conditions.

BACKGROUND: The increased use of the copy and paste function (CPF) in Electronic Health Records (EHRs) has raised concerns about possible clinician miscommunication and adverse patient outcomes. OBJECTIVE: This study investigated the prevalence and extent of CPF in the EHRs of patients diagnosed with Hospital-acquired Conditions (HACs). We also examined the association between the use of CPF and patient characteristics. MATERIALS AND METHODS: The prevalence and extent of CPF were investigated using electronic clinical notes of 50 patients hospitalized with HACs between 2017 and 2021 at a lar

Large language models for optimizing clinical trial recruitment in ICUs: application to ventilator-induced diaphragm dysfunction.

BACKGROUND: Ventilator-induced diaphragm dysfunction (VIDD) is a frequent and under-recognized consequence of prolonged mechanical ventilation in intensive-care unit (ICU) patients. Identifying eligible candidates for clinical trials targeting VIDD remains a major operational challenge. This study evaluates the use of large language models (LLMs) to automate patient prescreening from ICU discharge summaries and estimate recruitment capacity for a future phase 2 trial. METHODS: We developed an LLM-based prescreening pipeline to assess trial eligibility criteria from ICU discharge summaries, wh

Assessment of discharge treatment prescribed to women admitted to hospital for hyperemesis gravidarum.

AIMS: Prescribing drug treatment for the management of hyperemesis gravidarum (HG), the most severe form of nausea and vomiting in pregnancy, remains controversial. Since most manufacturers do not recommend prescribing antiemetics during pregnancy, little is known regarding which treatments are most prevalent among pregnant patients. Here, we report for the first time, evidence of actual treatments prescribed in English hospitals. METHODS: A retrospective pregnancy cohort was constructed using anonymised electronic records in the Nottingham University Hospitals Trust system for all women who

Dot Phrase Utilization and Opioid Prescription at Discharge for Subarachnoid Hemorrhage Patients.

BACKGROUND AND PURPOSE: Subarachnoid hemorrhage (SAH) frequently causes headaches that debilitate patients in the intensive care unit and persist for many patients in follow-up. Current literature suggests that opioids are ineffective for treating SAH pain despite their widespread use and perceived efficacy. There is evidence that implementation of dot phrases in note templates and electronic medical record alerts can positively influence prescriber behavior, including with reducing opioid prescriptions in the primary care setting. METHODS: We assessed rates of opioid prescription in our SAH

Patient-Representing Population's Perceptions of GPT-Generated Versus Standard Emergency Department Discharge Instructions: Randomized Blind Survey Assessment.

BACKGROUND: Discharge instructions are a key form of documentation and patient communication in the time of transition from the emergency department (ED) to home. Discharge instructions are time-consuming and often underprioritized, especially in the ED, leading to discharge delays and possibly impersonal patient instructions. Generative artificial intelligence and large language models (LLMs) offer promising methods of creating high-quality and personalized discharge instructions; however, there exists a gap in understanding patient perspectives of LLM-generated discharge instructions. The effect of the Shorter Stays in Emergency Departments health target on the quality of ED discharge summaries.

OBJECTIVE: Time targets for ED stays are used as a policy instrument to reduce ED crowding. There is debate whether such policies are helpful or harmful, as focus on a process target may divert attention from clinical care. The objective of this study is to investigate whether the Shorter Stays in Emergency Departments target in New Zealand was associated with a change in the quality of ED discharge information provided to primary care providers. METHODS: The quality of discharge summaries was assessed retrospectively over time using chart review. Logistic regression was used to account for s
