Pellagra is a nutritional disease caused by a deficiency of niacin. It may lead to death if not identified and treated timely. We review the literature and report a female patient presented with clinical features of pellagra as a complication of Crohn's disease.

Pellagra is caused by a deficiency of niacin. It is endemic in Africa and Asia where the staple diet of the many refugees or displaced people is corn-based and, therefore, nicotinic acid-deficient. It is also frequently seen in developed countries among chronic alcoholics or, occasionally, those suffering from anorexia nervosa. The usual manifestations are diarrhoea, dementia and dermatitis (the 3Ds), which if left untreated may lead to death. BACKGROUND: In the United States, pellagra is infrequently reported. Yet this disorder does occur among malnourished persons.
OBJECTIVE: The author seeks to clarify diagnosis and treatment.
METHOD: The author describes various presentations and effects of this disorder.
RESULTS: Knowledge of classic and atypical presentations can assist in making the diagnosis. The author presents two cases of pellagra that exemplify the classic and atypical presentations.
CONCLUSION: The typically robust response of the disorder to physiologic doses of niacin can assist in confirming the diagnosis.

Pellagra is caused by deficiency of niacin or its precursor tryptophan. While cutaneous lesions are the most prominent feature of the disease, gastrointestinal, neurological and psychiatric signs and symptoms are the other characteristics of the disease. In this case report, we present a 29-year-old female patient with discoloration of hands and feet diagnosed with pellagra.

A 67-year-old man was admitted to our hospital in May 2006 because of gait disturbance, delirium and myoclonus along with dermatitis and diarrhea. Those symptoms became worse in 3 months. BACKGROUND: While pellagra appears to be a rare entity currently, it may still develop. It is important to recognize how the disease manifests to ensure adequate and timely treatment.
CASE PRESENTATION: We present a case of pellagra secondary to anorexia nervosa in a 28-year-old woman. We observed the classical signs: erythema in the neck region, diarrhea, and neurologic symptoms. Diagnosis was made on a clinical basis, and the patient had a rapid recovery after undergoing therapy with nicotinamide and tryptophan.
