A 68-year-old woman presented dizziness whenever she put her finger into the right ear and also complained of water-streaming tinnitus, which indicated she would have been suffering from perilymph fistula. CONCLUSIONS: Cyclo-oxygenase-2 (COX-2) enzyme would not appear to be constitutively expressed in human perilymph while it is always induced in the perilymph of patients affected by sensorineural hearing loss (SNHL). To improve understanding of the pathophysiology of perilymph fistulas, a predictable animal model of a chronic fistula was developed. Our findings suggest that guinea pig fistulas do not remain patent for prolonged periods. By extrapolating these findings to humans, we postulate that the symptoms and signs of perilymph fistula are possibly due not to one prolonged constant fistula, but rather a series of "blowouts" from an inherent congenital or posttraumatic weak spot in either the round or oval window. In experiments using guinea pigs, the middle ear and perilymphatic pressures were simultaneously registered in response to pressure change in the external ear canal. In the first experiment, pressure was slowly loaded in the ear canal in the range of 200 mm H2O to -200 mm H2O. Management of 10 cases of primary perilymph fistula is described. The modified technique of stapedectomy described in this article, seems to prevent primary perilymph fistula; time will tell if it will prevent secondary fistula. The hearing gain obtained with this technique is slightly inferior to the gain obtained with Gelfoam-wire prosthesis, but most significant is the fact that discrimination score is excellent in all cases. The author is convinced that the extra time and dexterity required to perform this procedure are well justified by the safety provided and the results obtained.

Noise-induced hearing loss (NIHL) is significantly greater in rats fed a magnesium-deficient diet than in rats on a magnesium-rich diet. The hearing loss was found to be negatively correlated with the magnesium concentration of the perilymph. It is suggested that also in man, the magnesium concentration in the perilymph may be of importance in determining susceptibility to NIHL.

The clinical entity characterized by episodic vertigo and a positive Hennebert's sign in patients without hearing loss is described. The history of perilymph fistulas is reviewed. Minute perilymph fistulas of the oval window were documented surgically in five consecutive patients with this syndrome. All experienced relief of their symptoms after surgical correction.

In 1978, I reported on the diagnostic criteria and therapy based on the treatment of 34 cases of perilymph fistula. Of that group, 20 patients had no hearing loss associated with the fistula. Since metronidazole is very effective against anaerobic bacteria, we studied its pharmacokinetics in perilymph, cerebrospinal fluid, aqueous humour, and serum. 1, 2, 4, 6, 10 and 16.5 hours after intraperitoneal injection of 200 mg/kg metronidazole to six groups of six guinea pigs each, we determined the drug concentrations by high-pressure liquid chromatography.
