BACKGROUND: Childhood sun exposure is associated with development of future skin cancers. Sunscreens are an important tool to prevent harmful ultraviolet rays.
OBJECTIVES: The aims of the study are to evaluate sunscreens targeted to children and to analyze cost, marketing claims, ingredients, and allergens to help consumers select products.
METHODS: The top 50 pediatric sunscreens across retailers were analyzed for their cost, marketing claims, ingredients, vehicles, and containers. Ingredients were compared with the American Contact Dermatology Society 2020 Core Allergen List.

There are many anatomical and functional differences between the skin of young children and adult skin. As a consequence, the skin is more easily irritated by topical agents. There is also increased transcutaneous absorption; the latter effect is amplified by underlying conditions such as ichthyoses and atopic dermatitis with defects of the epidermal barrier. Common topical agents such as salicylic acid and lactic acid can cause life-threatening intoxications. The relevance of transcutaneous absorption of "hidden" ingredients such as polyethylene glycol and preservatives is unknown at present.

In recent years, we have observed several "ring-shaped" positive allergic patch test reactions to allergens dissolved in a liquid vehicle, a more intense response at the periphery of the site of application than in the central part. The occurrence of such reactions was evaluated for formaldehyde, Kathon CG, hydrocortisone and hexamidine diisethionate. Possible explanations for such reactions include pressure and/or a capillary effect.

Treatment of common warts with topical keratolytic solutions is generally estimated to result in cure rates of 60% to 80% in 12 weeks. Problems with keratolytic solutions include irritation of surrounding normal skin and poor patient compliance. A product containing 26% salicylic acid in a novel polyacrylic vehicle was evaluated in 27 patients and found to cure or provide much improvement in 22 (81%) after only two weeks of treatment. The penetration enhancement of sodium diclofenac and diclofenac by alcohols with various alkyl chains (C8 to C14) was evaluated by the steady state flux of diclofenac through rat abdominal skin. Propylene glycol is a commonly used vehicle for topical preparations. Although it is well suited for this purpose, it is capable of producing both primary irritant skin reactions and allergic sensitization. The literature on propylene glycol is reviewed, with particular attention to the nature of these adverse cutaneous effects. Guidelines for patients sensitive to propylene glycol are discussed.

Two preparations of diazepam, Valium Roche mixed Micelles and Diazemuls, were compared as intravenous premedicants in 119 patients undergoing minor gynaecological surgery. There were no differences between the formulations with regard to anxiolytic effects and systemic tolerance. There were no symptoms typical of thrombophlebitis in the Valium Roche Mixed-Micelles group and only one case in the Diazemuls group. Over 700 patients were screened by patch testing for sensitivity to the components of a vehicle tray from July 1974 through June 1976 at the Mayo Clinic. A modified version of the vehicle tray originally suggested by Fisher et al. (1971) was used, containing 14 substances commonly found in vehicles. Patches were applied for 24 rather than the usual 48 hours. Four studies carried out in normal volunteers assessed the characteristics and acceptability of the matrix of a soluble sustained-release delivery system. Fourteen volunteers in the first study received crossover single applications of a rod- and oval-shaped soluble unit at 8 AM.
