For the last two decades the most common method of radiographic study of the venous system of the lower limbs has been ascending phlebography, by injection of contrast material in a superficial vein on the dorsum of the foot. We report our own experience with another approach, using a large field technique on recumbent patients, with elevation of the limbs, thus taking advantage of gravity. The degree of pain produced by Amipaque 260 and Isopaque 260 at intraosseous phlebography in patients with varicose ulcerations in the ankle region was compared. Amipaque produced less pain than Isopaque. No complications or side-effects occurred during the observation period of about one year.

From a series of 400 cases of lumbar spine phlebography by selective catheterisation, 4 serious thrombo-embolic episodes were observed, including 2 cases of severe pulmonary embolism which responded favourably to medical treatment. In 3 cases, risk factors were found (prolonged immobilisation in bed past history of phlebitis). One case developed severe phlebitis of the lower limbs despite prophylactic treatment with low dose calcium heparin. In the other cases, no anticoagulant therapy was prescribed before or during the examination.

Seventy-eight limbs of forty-eight patients undergoing total hip replacement were studied by roentgenographic phlebography, cuff-impedance phlebography, and 125I fibrinogen scanning. The authors compared the adequacy of phlebography and color-Doppler US in the diagnosis of subclavian-axillary thrombosis, or Paget-Schroetter syndrome. Ten patients with subclavian-axillary thrombosis (8 men and 2 women, aged 16 to 55 years, mean age: 30 years) were examined over a two-year period.
