Clinical improvement of photophobia with galcanezumab in episodic and chronic migraine.

OBJECTIVE: This study aimed to evaluate changes in photophobia using the Turkish version of Utah Photophobia Symptom Impact Scale-12 (UPSIS-12) in patients with episodic (EM) and chronic migraine (CM) treated with galcanezumab, and to examine the relationship between photophobia, migraine outcomes, disability, and treatment response. BACKGROUND: Photophobia is one of the most bothersome symptoms (MBS) of migraine and may persist during both ictal and interictal periods, leading to substantial impairment in daily functioning. Despite the clear clinical relevance of photophobia, the specific ef…

Beyond migraine: A narrative review of photophobia's clinical spectrum.

INTRODUCTION: Photophobia is a frequent and often disabling symptom encountered in a wide range of neurological and ophthalmological conditions. Particularly prevalent in migraine, but also observed in common eye diseases, photophobia reflects complex interactions between visual and nociceptive pathways. This review provides a comprehensive overview of the clinical features, pathophysiological mechanisms, diagnostic challenges, and therapeutic strategies of photophobia from both neurological and ophthalmological perspectives. MATERIAL AND METHODS: A narrative literature review was conducted u…

Hyperactivity of the medial thalamus in patients with photophobia-associated migraine.

OBJECTIVE: To examine cerebral functional alterations associated with sensory processing in patients with migraine and constant photophobia. BACKGROUND: Migraine is a common headache disorder that presents with photophobia in many patients during attacks. Furthermore, some patients with migraine experience constant photophobia, even during headache-free intervals, leading to a compromised quality of life. METHODS: This prospective, case-control study included 40 patients with migraine (18 male and 22 female) who were recruited at an eye hospital and eye clinic. The patients were divided into…

Intercritical photophobia in the migrainous patient. Proposal for diagnostic criteria.

INTRODUCTION: Photophobia is a symptom of abnormal light intolerance without pain sensation that requires an anamnesis and an examination to diagnose an underlying etiology. BASIC PROCEDURE: This article focuses on 30 clinical cases with isolated intense photophobia and on the review of the literature. OBJECTIVE: The purpose of this article is to establish diagnostic criteria for photophobia. RESULTS: The etiology of photophobia appears to be at the level of the intrinsically photosensitive retinal ganglion cells known as melanopsin cells and at a neurochemical level mediated by calcitonin-…

Blepharospasm With Photophobia Secondary to Midbrain Tuberculoma and Thalamic Infarct.

INTRODUCTION: Blepharospasm is a type of focal dystonia and categorized into primary and secondary forms, based on whether or not a cause can be established. Secondary blepharospasm is uncommon and can be associated with underlying brain lesions. Photophobia is a prominent complaint in blepharospasm patients. We are reporting a case of secondary blepharospasm with photophobia in a patient who had underlying midbrain tuberculoma and thalamic infarcts. This type of presentation has not been reported to the best of our knowledge. CASE REPORT: A 26-year-old man presented to us with the complaint …

Photophobia in primary headaches, in essential blepharospasm and in major depression.

OBJECTIVES: Although photophobia is a well-known symptom in various disorders, it has rarely been studied explicitly and its definition in a clinical setting can be somewhat elusive. Here, we assessed photophobia with a common psychometric tool in different conditions, in which light intolerance is considered part of the syndrome. PATIENTS AND METHODS: A prospective study was undertaken in patients with migraine (MH), cluster headache (CH), tension-type headache (TH), essential blepharospasm (BS) and major depression (MD). Photophobia was assessed by the photophobia questionnaire (range 0-8).…

Modulation of sensory photophobia in essential blepharospasm with chromatic lenses.

PURPOSE: To determine the effect of photochromatic modulation with tinted lenses on the sensory symptoms of photophobia in blepharospasm patients. DESIGN: Nonrandomized case-control study. PARTICIPANTS: Thirty-four subjects (24 benign essential blepharospasm patients and 10 normal controls). METHODS: Subjects were seated in front of a calibrated light source. Beginning at 0%, the intensity of the light source was increased gradually until the patient reported symptomatic photophobia. The intensity of the light source was then measured with a light meter. This procedure was performed first w…

Photophobia as the Presenting Symptom of Internal Carotid Artery Dissection.

A 53-year-old man developed right eye photophobia after a hypertensive crisis, followed by right-sided headache and retro-orbital pain. On examination, a right Horner syndrome was evident and an urgent brain and neck Computed Tomography confirmed the suspicion of an internal carotid artery (ICA) dissection. While photophobia may occur in many neurological disorders, to our knowledge, it has never been described when the ICA is involved. Possible hypotheses of photophobia caused by carotid artery dissection are discussed.

Further delineation of the ichthyosis follicularis, atrichia, and photophobia syndrome.

We describe an 18-month-old male infant suffering from the ichthyosis follicularis, atrichia, and photophobia (IFAP) syndrome and further delineate the clinical phenotype. Severe retardation of growth and psychomotor development, chill-like seizures, bronchial asthma, urticaria, a proneness to skin infections and transient nail dystrophy observed in our patient are non-obligatory manifestations of this disorder. Histological examination of the atrichia revealed poorly developed, shortened hair follicles and a complete absence of sebaceous glands. The sex ratio of published cases suggests an X-…

Correlation between higher-order aberration and photophobia after cataract surgery.

Cataract surgery impinges on the spatial properties and wavelength distribution of retinal images, which changes the degree of light-induced visual discomfort/photophobia. However, no study has analyzed the alteration in photophobia before and after cataract surgery or the association between retinal spatial property and photophobia. Here, we measured the higher-order aberrations (HOAs) of the entire eye and the subjective photophobia score. This study investigated 71 eyes in 71 patients who received conventional cataract surgery. Scaling of photophobia was based on the following grading syste…

Tryptophan depletion increases nausea, headache and photophobia in migraine sufferers.

Sensitivity to light was investigated 5 and 8 h after consumption of an amino acid drink which contained L-tryptophan (balanced amino acid c … s and controls. These findings suggest that a reduction in brain synthesis of serotonin intensifies photophobia and other migrainous symptoms and thus might contribute to the pathogenesis of migraine.

Ichthyosis follicularis, alopecia and photophobia (IFAP) syndrome treated with acitretin.

We describe a 3-year-old male patient with the ichthyosis follicularis, alopecia and photophobia (IFAP) syndrome, who developed cutaneous and ocular involvement in infancy. In addition, he had growth retardation and borderline intelligence; no other systemic involvement was found on detailed investigation. A moderate response to acitretin therapy (1 mg/kg) administered for 6 months was observed, with improvement in cutaneous features and corneal erosions and no change in alopecia or photophobia.

Cerebellar Ataxia With Extreme Photophobia Associated With Anti-SOX1 Antibodies.

Anti-SOX1 antibodies are associated with diverse neurological syndromes, targeting both the central (paraneoplastic cerebellar degeneration) and peripheral nervous systems (Lambert Eaton myasthenic syndrome, paraneoplastic neuropathy). Although the pathogenic role of these antibodies remains unclear, their strong association with underlying neoplastic disease (mainly small-cell lung cancer) has designated them as onconeural antibodies. Here, we present a case of cerebellar ataxia with marked photophobia, with severe atrophy of the cerebellum and brain stem, associated with anti-SOX1 antibodies…
