Papillary Thyroid Carcinoma, Cushing Disease, and Adrenocortical Carcinoma in a Patient with Li-Fraumeni Syndrome.

BACKGROUND/OBJECTIVE: Li-Fraumeni syndrome (LFS) is an inherited sequence variant in TP53 characterized by the early onset of various core m … with ACC. DISCUSSION: There is limited literature on the relationship between LFS and thyroid and pituitary neoplasms. Genetic testing has suggested that TP53 sequence variants may play a role in tumorigenesis in thyroid and pituitary neoplasms; however, most of the current literature is based on evidence of somatic rather than germline sequence variants. CONCLUSI …

Two Pituitary Neuroendocrine Tumors (PitNETs) with Very High Proliferation and TP53 Mutation - High-Grade PitNET or PitNEC?

We report two pituitary neuroendocrine tumors (PitNETs) with very high Ki67 labeling indices, many mitoses and TP53 mutation (nearly all tumor cell nuclei were positive for p53). One of the tumors had bone and liver metastases. One was a corticotroph cell tumor; the other was a lactotroph tumor. The classification of these tumors is the subject of this discussion. Traditionally, pituitary carcinomas are only diagnosed by demonstration of metastases according to the 2017 WHO classification. In contrast, neuroendocrine neoplasms of the gastrointestinal tract and pancreas are classified as either

Update on the Genetics of Pituitary Tumors.

Pituitary adenomas are common intracranial neoplasms, with diverse phenotypes. Most of these tumors occur sporadically and are not part of genetic disorders. Over the last decades numerous genetic studies have led to identification of somatic and germline mutations associated with pituitary tumors, which has advanced the understanding of pituitary tumorigenesis. Exploring the genetic background of pituitary neuroendocrine tumors can lead to early diagnosis associated with better outcomes, and their molecular mechanisms should lead to novel targeted therapies even for sporadic tumors. This arti

[Pituitary tumors].

Pituitary adenomas have been reported to be the most common benign intracranial neoplasms. Recent radiological advancement has resulted in incidental diagnosis of pituitary tumors such as pituitary adenomas and Rathke's cleft cysts. The most common location of pituitary adenomas is intrasellar region with or without suprasellar extension. Pituitary tumors in unusual site, however, have been reported previously, including the sphenoid sinus, suprasellar region, clivus, nasal cavity, or something like that. The sources of ectopic pituitary tumors have been thought to be migration of pituitary ce

Surgical management of pituitary adenomas.

Pituitary adenomas are a diverse group of benign neoplasms. The hormonally active tumours present with well-recognised syndromes and include acromegaly (growth hormone adenoma), Cushing's disease (corticotropin adenoma), and amenorrhea-galactorrhea (prolactin adenoma or prolactinomas). The hormonally inactive, or clinically non-secreting, adenomas generally come to clinical attention secondary to local mass effect or pituitary deficiency. With the exception of the prolactinomas, transsphenoidal surgery remains the first-line therapy for most pituitary adenomas. The current diagnosis and surgic

The Optimal Time for Postoperative Magnetic Resonance Imaging of the Sella in Patients With Pituitary Adenoma.

INTRODUCTION: Magnetic resonance imaging (MRI) is the preferred neuroradiologic tool for evaluating the sellar region. Pituitary adenomas account for about 15% of primary intracranial tumors. The optimal time for postoperative MRI of central nervous system neoplasms is 48 hours after surgery. Nevertheless, controversy exists regarding the timing of postoperative MRI in the sellar region. This study analyzed the sellar MRI findings of patients with pituitary adenoma at different times before and after surgery. Finally, we suggest the optimal time for postoperative sellar MRI imaging in patients

Neuropathology of Pituitary Adenomas and Sellar Lesions.

The pituitary gland is the site of numerous neoplastic and inflammatory processes. The overwhelmingly most frequent tumors arise from cells of the anterior lobe, the pituitary neuroendocrine tumors (PitNETs). Immunohistochemistry assay staining for pituitary hormones is the core tool for classifying PitNETs, resulting in the diagnosis of somatotroph PitNETs, lactotroph PitNETs, and so on. For cases showing no hormonal expression, the updated WHO classification system now considers the assessment of several transcription factors: PIT-1 (pituitary-specific POU-class homeodomain transcription fac

MicroRNAs-based network: a novel therapeutic agent in pituitary adenoma.

Pituitary adenomas are common benign intracranial neoplasms representing about 10-25% of all intracranial neoplasm. Significant morbidity can occur along with pituitary adenomas due to hormonal dysfunction and mass effects. The pathogenesis of pituitary adenoma is unclear, however, etiologic factors include genetic events, hormonal stimulation, and growth factors [1], all of which promote cell proliferation and transformation in the tumor. However, genetic events play the most important role in tumorigenesis. MicroRNAs (miRNAs), a class of non-coding RNAs, not only have function in pituitary c

[Aggressive pituitary tumors and carcinomas: modern classification, advances and prospects in treatment].

Despite slow growth of most pituitary tumors and high rates of total resection and/or effective therapy, pituitary neoplasms are characterized by aggressive behavior with high growth rate, frequent relapses and resistance to standard treatments in 10% of cases. In modern WHO classifications of tumors of the central nervous system, endocrine and neuroendocrine tumors, the authors propose the definition «pituitary neuroendocrine tumor» instead of previous «pituitary adenoma» and «metastasizing pituitary neuroendocrine tumor» instead of «pituitary carcinoma». Currently, there are no effective pro…

Metastasis of an occult gastric carcinoma suggesting growth of a prolactinoma during bromocriptine therapy: a case report with a review of the literature.

The treatment of a slowly growing invasive prolactinoma with bromocriptine for 8 months resulted in a substantial decrease in plasma prolactin levels despite rapid suprasellar tumor expansion. On exploration, this uncommon observation could be attributed to hematogenous metastasis from an occult gastric adenocarcinoma to the pituitary tumor. Apart from infiltration of neighboring parts of the hypothalamus, autopsy revealed no other hematogenous metastases. This extraordinary type of neoplasm-to-neoplasm metastasis was not shown by computed tomography. This possibility should be considered when

Non-adenomatous sellar lesions: single-centre 10-year experience.

OBJECTIVE: A minority of lesions found in the sellar region are non-adenomatous neoplastic, inflammatory, or cystic masses. Our study aims to describe the prevalence and characteristics of these lesions in a multidisciplinary pituitary outpatient clinic. DESIGN: We conducted an observational study which included 36 patients (15.9% of those followed up in this outpatient clinic between 2006 and 2016 who had pituitary surgery) submitted to pituitary surgery with histological results showing a non-adenomatous sellar lesion. We evaluated clinical, radiological, and biochemical (pituitary function

NG2 targets tumorigenic Rb inactivation in Pit1-lineage pituitary cells.

The proteoglycan neuron-glial antigen 2 (NG2) is expressed by oligodendrocyte progenitors, pericytes, and some cancerous cells where it is implicated in tumor development. We examined mice with NG2-driven pRb inactivation. Unexpectedly, NG2-Cre:pRb(flox/flox) mice developed pituitary tumors with high penetrance. Adenohypophysial neoplasms developed initially as multifocal lesions; by 1 year, large tumors showed brain invasion. Immunohistochemistry identified these as Pit1-lineage neoplasms, with variable immunoreactivity for growth hormone, prolactin, thyrotropin, and α-subunit of glycoprotein

Benign and Malignant Tumors of the Pituitary Gland.

Pituitary gland tumors represent approximately 10-15% of all brain tumors and the most common neoplasms of the sellar region. Among them, pituitary adenomas are the widespread accounting for more than 80%. Recently, the fourth edition of the World Health Organization (WHO) 2017 classified pituitary tumors focusing on histopathologic and molecular genetics features and introduced new entities like pituitary blastoma. Most of pituitary gland neoplasms occur sporadically, whereas 5% are related to familial syndromes. They present with several clinical manifestations including signs and symptoms r

Carcinogenicity of heptachlor and heptachlor epoxide.

Heptachlor and its metabolite heptachlor epoxide are unequivocally carcinogenic in rats and mice. The chemicals induced carcinomas of the liver, which were highly significant. There were neoplasms at other sites in rats. Neoplasms at all sites, as well as malignant tumors, were increased in heptachlor-treated male rats. There were similar increases in benign and malignant neoplasms of endocrine organs, particularly in female rats. Neoplasms of the thyroid and pituitary were increased in male rats and neoplasms of the reproductive system, including the ovary and uterus, in female rats given hep

MRI Imaging of Double Pituitary Microadenoma: A Rare Preoperative Diagnosis.

Pituitary adenomas are benign, single, monoclonal slow-growing neoplasms usually related to chemical overproduction. A pituitary adenoma is the third most common intracranial tumor, with the first two being glioma and meningioma. Double pituitary adenoma is an infrequently occurring case and is characterized as occurrence of two adenomas in the single pituitary gland, both having typical immunohistochemical and histopathological highlights. In most of the cases, pituitary adenomas occurring as double adenomas are associated with Cushing's disease. It is further categorized into contiguous and
