[Study of the placebo effect by the pressure profile method].

The effect of one tablet of placebo daily for 8 days on blood pressure was studied in 90 hypertensive patients, using the blood pressure profile method, with a 12-hour recording period (from 8 a.m. to 8 p.m) and readings every 15 minutes (Dinamam 845). Statistical analysis of the results showed no significant difference in diastolic, systolic and mean blood pressure in 50% of the cases. A significant decrease of about 10% in blood pressure without change in variability was observed in more than 20% (complete placebo effect) and in about 30% (total placebo effect) of the cases. A significant in…

Placebo effect in surgery for Ménière's disease. A double-blind, placebo-controlled study on endolymphatic sac shunt surgery.

To investigate the placebo effect in surgery for Ménière's disease, a double-blind controlled study was undertaken, comparing effects of a regular endolymphatic shunt with those of regular mastoidectomy. Thirty patients with typical Ménière's diseases, selected because of unsuccessful medical treatment, participated. Patients completed daily dizziness questionnaires three months before and 12 months after surgery, with registration of nausea, vomiting, vertigo, tinnitus, hearing impairment, and pressure in the ears. Patients were operated on at two universities, and the patients operated on at…

Physiological and Placebo Effects of Beetroot Juice in Virtual Cycling Using the Balanced Placebo Design.

The physiological and placebo effects of beetroot juice remain unclear in settings outside traditional laboratory environments. Using a randomized, between-participant balanced placebo design, we asked 67 trained cyclists to complete 20-min time-trials on a virtual cycling platform with the ingestion of either beetroot juice or placebo (~552 or ~0.2 mg nitrate, respectively). After completing baseline trials, participants were randomized to one of four experimental groups: 1) informed beetroot juice/given beetroot juice (BRJ/BRJ), 2) informed beetroot juice/given placebo (BRJ/PLA), 3) informed…

The placebo effect and white coat effect in isolated systolic hypertension and systo-diastolic hypertension.

It is well recognised from many clinical trials that there is a blood pressure lowering effect when placebo is administered to patients with essential hypertension ("placebo effect"). The reduction in blood pressure, however, may also be partly due to loss of the alerting response ("white coat effect") as a result of familiarisation with the clinical environment. To investigate the hypothesis that there may be a more marked placebo effect and white coat effect in isolated systolic hypertension (ISH) compared with systo-diastolic hypertension (SDH), we studied 78 patients with hypertension: 34 …

Objective approach for fending off the sublingual immunotherapy placebo effect in subjects with pollenosis: double-blinded, placebo-controlled trial.

BACKGROUND: Symptom scoring for the assessment of allergen immunotherapy is associated with a substantial placebo effect. OBJECTIVE: To assess the ability of exhaled breath temperature (EBT), a putative marker of airway inflammation, to evaluate objectively the efficacy of grass pollen sublingual immunotherapy in a proof-of-concept study. METHODS: This was a double-blinded, placebo-controlled clinical trial in 56 subjects (mean ± SD 30 ± 12 years old, 33 men) sensitized to grass pollen. The objective measurements were EBT, spirometry, and periostin and high-sensitivity C-reactive protein in …

A Reliable Effect of Alexithymia, but Not Autism, on Placebo Hypoalgesia.

Perception of pain can be altered by one's expectations of that pain, producing well-known phenomena such as placebo effects. It has been suggested that perceptual inference functions differently in autism, with perception weighted towards bottom-up sensory evidence rather than top-down expectations. This account would predict reduced placebo effects in autism. However, research also indicates that atypical interoception (perception of bodily sensations) in autism may be better explained by co-occurring alexithymia. This latter finding suggests that any differences in perceptual inference in a…

Placebo effects: Rethinking the mechanisms of morphine analgesia.

A new study provides the most comprehensive, mechanistic account to date of how the brain learns to predict and mimic drug effects. After associating morphine injections with a place context, the context alone elicits endogenous opioid release and analgesia via a classic prefrontal-brainstem pathway.

Classical conditioning of antidepressant placebo effects in mice.

INTRODUCTION: Placebo effects in human clinical trials for depression treatment are robust and often comparable to drug effects. Placebo effects are traditionally difficult to study in rodents due to the slow-onset action of classical antidepressant drugs. We hypothesized that the rapid antidepressant actions of ketamine would allow modeling antidepressant placebo effects in rodents. METHODS: Male and female CD-1 mice received either ketamine or saline injections with concomitant exposure to specific environmental conditioning stimuli, for a total of three drug/conditioning sessions each 2 we…

Placebo effects in treating migraine and other headaches.

Evaluating the efficacy of a drug therapy in the acute and prophylactic treatment of migraine requires the conduct of placebo-controlled, randomized clinical trials. In order to plan and conduct these studies in the most appropriate manner, it is desirable to know which factors influence the placebo response. This editorial overview discusses the factors that influence placebo response in trials for migraine and other headaches; such factors include expectation, blinding, route of drug administration, patient age and gender, and geographic distribution of the trials. The placebo response rates

Classic conditioning and placebo effects in crossover studies.

OBJECTIVE: To find evidence of classically conditioned placebo effects in a placebo-controlled crossover drug study. Specifically, we tested a prediction of the conditioning model that the placebo response will be greater after drug exposure than before. METHODS: Twenty-four patients with mild to moderate essential hypertension and no contraindications to atenolol participated in the study. The study design required randomized assignment to one of three groups: placebo followed by 50 mg atenolol daily, followed by no treatment, each for 1 week; atenolol followed by placebo; and atenolol follo…

Effects of terfenadine, diphenhydramine, and placebo on skills performance.

The behavioral effects of the antihistamines terfenadine and diphenhydramine were compared in a placebo-controlled, double-blind crossover study. Performance of skills deemed important in safe man-machine interactions was objectively assessed one, three, and five hours after administration of test substances. The results indicate that diphenhydramine use resulted in various degrees of impairment of the ability to perform a visual-search task, a tracking task, a divided-attention task, and a vigilance task. Terfenadine users performed as well as or better than placebo-treated subjects. No diffe

Rediscovering the ritual technology of the placebo effect in analytical psychology.

Technology, viewed more generally, is a collection of skills and methods that are used to accomplish an objective of some kind. Modernity has produced many kinds of ever-expanding new technologies, but it is also evident that technologies can be lost or fall out of use. A cross-cultural survey of ritual reveals a rather startling observation: that while developed nations often exceed other cultures in terms of material technology, they often pale by comparison in their use of ritual technology. In this essay we will see how ritual is a powerful sort of technology that developed nations have mo

Placebo-controlled assessment of somnolence effect of cetirizine: a meta-analysis.

BACKGROUND: It has been found that the most common adverse reaction which occurs more frequently on cetirizine than on placebo is somnolence. However, the somnolence rate varied widely among different studies. The objective of this study was to assess the somnolence effect of cetirizine 10 mg daily compared to placebo in patients aged 6 years and older using meta-analysis and explore the sources of heterogeneity among different studies. METHODS: Databases were searched for randomized controlled trials (RCTs) of cetirizine published before 2015. Overall risk differences (RDs) were determined b

Low-Dose Tricyclics for Esophageal Hypersensitivity: Is it all Placebo Effect?

Limsrivilai et al. report on a randomized control trial (RCT) testing the efficacy of imipramine for treating esophageal hypersensitivity and functional heartburn, the first RCT to test this therapy in this indication. Among 43 functional heartburn and esophageal hypersensitivity patients randomized to treatment with 25 mg qhs imipramine and 40 randomized to matched placebo, the response rates, judged by a 50% reduction in gastroesophageal reflux disease symptoms, were 37.2% and 37.5%, respectively, with no observed difference between patients with hypersensitivity and those with functional he

Effect of amantadine in essential tremor: a randomized, placebo-controlled trial.

There is a need for new medication for essential tremor (ET). Preliminary evidence suggests that amantadine may be effective in the treatment of ET. We studied the effects of amantadine in a double-blind, cross-over, placebo-controlled trial in ET patients. Sixteen patients with ET received amantadine 100 mg b.i.d. and placebo for 15 days, with a 1-week wash-out period between treatments. Major evaluation outcomes consisted of a tremor clinical rating scale, accelerometric recordings, and a self-reported disability scale obtained before drug intake and on study days 1 and 15 of each treatment
