The operation technique and the results after occipital correction in children with plagiocephaly and scaphocephaly are reported. The indication for the intervention is provided by aesthetic and functional considerations. The outcome is very good; the growth of the skull follows the same range of percentiles as preoperatively and normal mental development is seen.

INTRODUCTION: The diagnosis and treatment of posterior plagiocephaly is one of the most controversial aspects of craniofacial surgery.
PATIENTS AND METHODS: The purpose of this study is to describe a recent increase in the incidence of occipital plagiocephaly without synostosis in our hospital during the last 6 months. The shift in the referral patterns is roughly contemporaneous with the American Academy of Pediatrics recommendations regarding infant sleep position. The temporal coincidence of this increase with the recommendation to avoid the prone sleeping position, to reduce the risk of s

An asymmetrical head (plagiocephaly) may be due to premature closure of the sutures of the skull (craniosynostosis) or to prolonged pressure on one side of the head in the pre- peri- or postnatal period. The latter deformity may be part of the squint baby syndrome. The diagnosis is based on a careful history and physical examination. X-rays of the skull and/or the cervical vertebral column are seldom necessary to confirm the diagnosis. Physiotherapy in an early stage, directed toward plagiocephaly and associated disorders, results in a complete disappearance of symptoms in a high percentage of

Plagiocephaly patients generally undergo corrective surgery in the first years of life, but during their growth malformative sequelae become apparent and can have a negative psychological and social impact. Volumetric filling techniques have been used to improve social balance and minimize such negative effects. We present a case of a 25 years old male with a history of plagiocephaly corrected by 8 months of age. He kept a bilateral frontotemporal defect and we performed a lipofilling of the affected areas. The aesthetic results were very satisfactory and 6 months and 1 year after the procedure

UNLABELLED: : media-1vid110.1542/5972296741001PEDS-VA_2018-2373Video Abstract BACKGROUND: Studies have revealed an association between positional plagiocephaly and/or brachycephaly (PPB) and development, although little is known about long-term outcomes. We examined cognition and academic achievement in children with and without PPB, testing the hypothesis that children who had PPB as infants would score lower than controls.
METHODS: We enrolled 187 school-aged children with a history of PPB and 149 controls. Exposures were the presence or absence and severity of infancy PPB (mild, moderate t

The typical asymmetry in infancy is recognized 2-4 weeks after birth and follows the development of a plagiocephaly. The unilateral flattening of the occiput leads to a permanent rotation of the head to one side, shortening of neck muscles and a triggering of the asymmetric tonic neck reflex (ATNR), also known as fencer response. Subsequently a muscle tone increase is forming on the side of the face while a decrease in tone is seen at the occiput side. Clinical signs like increasing neck stiffness, flexible positional scoliosis and pelvic obliquity may ultimately develop. Management includes e

OBJECTIVE: Treatments of unilateral lambdoid synostosis and unilateral coronal synostosis.
METHODS: Approach to coronal, double frontal flaps were rotated with orbit advancement and anteversion of the frontal to enlarge the volume of ante cranial fossa., and reconstruct the contour of orbit and the frontal. The gap was between the frontal and parietal to reconstruct the coronal suture after fixation and reconstruction of the frontal os. For post plagiocephaly, double rotated occipital flap and plum-plasty flap were performed to reconstruct the occiput. The gap between the parietal and occiput

Over the past decade there has been a dramatic increase in referrals to specialty clinics, craniofacial centers, plastic surgeons, and neurosurgeons for assessment and treatment of deformational plagiocephaly (DP). Though considered a medically benign condition, preliminary reports suggest that DP may be associated with developmental problems. However, mechanisms to account for this association have not been hypothesized or empirically tested. Although treatment justifications often center on prevention of atypical appearance, little is known about the cosmetic outcomes of treated and untreate

Positional plagiocephaly deformities have increased dramatically in all craniofacial clinics in the United States. There are multiple methods for evaluating the degree of deformity, all of which are expensive, time consuming, and have poor reproducibility. We present a clinical classification of plagiocephaly deformities that we have employed since 1998. The classification allows us to quantitate the degree of deformity in these children at any given time, to reliably determine quantitative changes from evaluation to evaluation. The technique is highly reproducible, cost effective and readily

The pathology, diagnosis and management of posterior plagiocephaly remains highly controversial. While the rationale for surgical management of true lambdoid synostosis is undisputed, opinions vary greatly on how to manage severe, unresolving, non-synostotic cases. We reviewed 39 cases of posterior plagiocephaly, 37 of which were treated conservatively. Of these, 34 patients had a significant improvement over the following year with sleep posture modification and/or physiotherapy. While only eight cases returned to complete normality, the remainder had deformities that were deemed mild by both

INTRODUCTION: The American Academy of Pediatrics Back-to-Sleep Campaign significantly reduced infant mortality from sudden infant death syndrome. As a result of prolonged supine positioning, the incidence of deformational plagiocephaly has also risen 5-fold since its adoption. We aimed to improve the current educational paradigm for new parents with the goal of reducing the incidence of plagiocephaly within the confines of the Back-to-Sleep Campaign. We hypothesized that the early addition of plagiocephaly focused education for parents would reduce cephalic index, the ratio of head width to le

OBJECT: The increase in the prevalence of nonsynostotic occipital deformational plagiocephaly in infants, which resulted from the American Academy of Pediatrics' 1992 recommendation to have healthy infants sleep supine, has been accompanied by significant controversy in diagnosis and management. The controversy was exacerbated by the 1998 FDA classification of cranial orthotic devices as Class II devices requiring premarket notification, and the subsequent increase in treatment-associated costs.
METHODS: Two independent reviews of the literature were conducted to clarify the objective evidenc

OBJECTIVE: The management of infants with posterior plagiocephaly has been controversial both because of widely differing estimates in the literature of the relative frequencies of true lambdoidal synostosis vs positional molding and because of divergent approaches to treating this problem in different institutions. Based on our experience, we hypothesized that the vast majority of children with posterior plagiocephaly did not have true synostosis and that the cosmetic impairment in such patients could be effectively treated with nonsurgical modalities.
METHODS: Between 1992 and 1995, we pros

A classification of anterior plagiocephaly is proposed by the authors on the basis of the clinical and radiological findings analyzed in a series of 28 subjects less than 1 year old. Three main patterns of the anomaly have been identified. Type I anterior plagiocephaly is characterized by unilateral flattening of the frontal bone and elevation of the superior orbital ridge without deviation of the nasal pyramid. The vomer and petrous bones maintain a normal position. In type II anterior plagiocephaly, the frontal and orbital anomalies are accompanied by contralateral deviation of the nasal pyr
