PURPOSE: For the treatment of thyrotoxicosis, alternative treatment modalities may be necessary if anti-thyroid drugs cannot be used due to side effects, inefficiencies, or there is a need to start a rapid action such as thyroid storm. By using therapeutic plasma exchange (TPE), it is possible to effectively and rapidly remove the increased thyroid hormones. We evaluated our results and experience on a rapid, effective, and reliable alternative treatment modality in thyrotoxic patients.
METHODS: TPE was performed in 46 thyrotoxic patients at the Adult Therapeutic Apheresis Center.

Therapeutic plasmapheresis has been used for intractable diseases that cannot be cured by conventional drug therapy. Currently, the use of therapeutic plasmapheresis has been approved for 27 diseases by Japan's National Health Insurance system and is mainly categorized into three modalities: plasma exchange (PE), double-filtration plasmapheresis (DFPP), and plasma adsorption (PA). BACKGROUND: Double-filtration plasmapheresis (DFP) removes high-molecular-weight molecules semiselectively, thereby minimizing albumin loss, but with subsequent need for fluid substitution. The purpose of this study was to establish the overall incidence of complications during DFP and to analyze the possible contributory factors.

In 10 patients with Guillain-Barré syndrome the level of globulins and immunoglobulins before and after plasmapheresis was investigated. As a plasma substitute either PPL (in 8 patients) or a plasma substitute solution rich in immunoglobulins (in 2 patients) was used. The kinetic model of continuous gravitation plasmapheresis on preoperative preparation of patients with thyrotoxicosis has been developed and fried on 41 patients who had failed thyrostatic drugs or were intolerant to them. The model takes into account the process of plasma mixing. The prognostic value of the model in all cases exceeded 0.83; being in sonne cases > 0.09. The prognostic accuracy of the model was higher if the plasma volume and thyroid hormones concentration were considered.

Despite the wide number of diseases currently or previously treated with plasma exchange and plasmapheresis, the clinical effectiveness of these treatments has been established by large, controlled clinical trials only in few clinical conditions. The firmly accepted and the possible indications for these techniques in critically ill patients are reviewed and discussed, as well as their complications and possible side effects.

The authors present their experience with therapeutic plasmapheresis (TPE) using membrane filters at the pediatric dialysis unit of a referral center. Between January 2006 and December 2010, 486 sessions of TPE were performed in 39 patients (range 6-17 y), chiefly for atypical hemolytic uremic syndrome (HUS, n = 22), crescentic glomerulonephritis (n = 8) and focal segmental glomerulosclerosis (n = 5).
