Does pulmonary vasoconstriction play an important role in patients with primary pulmonary hypertension? A skeptic's view of vasodilator therapy.

Despite isolated reports of dramatic hemodynamic and clinical improvement over the past 30 years, most patients with primary pulmonary hypertension fail to benefit from treatment with vasodilator drugs and many develop serious adverse reactions. The failure of this approach strongly suggests that pulmonary vasoconstriction does not play an important role in the pathogenesis of this disorder.

A novel fusion suicide gene yeast CDglyTK plays a role in radio-gene therapy of nasopharyngeal carcinoma.

To investigate a novel suicide gene for nasopharyngeal carcinoma (NPC) therapy, the yCDglyTK gene was constructed by fusing yeast cytosine deaminase (CD) and herpes simplex type 1 thymidine kinase. The expression of the yCDglyTK gene was detected by RT-PCR and Western blotting, and its bioactivity was demonstrated by an MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide) assay. An animal study was carried out in which BALB/C nude mice bearing yCDglyTK gene-modified tumors were treated with prodrugs and radiation. Our results revealed that the yCDglyTK gene could be expressed in

Silver-doped MoOx NPs with phytochemicals for photodynamic therapy application: Playing with the sequence of surface modification steps.

Photodynamic therapy (PDT) is a promising strategy for cancer treatment, employing photosensitizers activated by light to induce cytotoxicity. Hybrid organic-inorganic nanocomplexes, which integrate the unique properties of plasmonic metals and organic photosensitizers, are currently under intensive investigation. In this study, we propose the development of a novel photoactive nanoplatform for PDT based on a composite system comprising molybdenum oxide, silver, and phytochemicals from Hypericum perforatum and Chelidonium majus. To evaluate how the sequence of silver and phytochemicals deposit

A systematic review on the applications of resting-state fMRI in Parkinson's disease: Does dopamine replacement therapy play a role?

Functional neuroimaging techniques provide important insight into the pathophysiology of neurodegenerative disorders such as Parkinson's disease (PD) in-vivo. Recently, resting-state functional magnetic resonance imaging (rs-fMRI) has been applied as a non-invasive tool in many studies to assess functional abnormalities observed in PD without the effects of particular motor or cognitive tasks. In this review, we summarized 50 original PD rs-fMRI studies and subdivided them based on the medication status of the patients to highlight the impact of dopamine replacement therapy (DRT) when rs-fMRI

Sitting Together and Reaching to Play Physical Therapy Affects Dyadic Emotional Availability in Children With Neuromotor Delay and Their Families.

OBJECTIVE: Emotional availability (EA) describes caregiver-child emotional attunement and is critical to fostering adaptive development. Although early physical therapist interventions adopt a family centered approach, the impact of interventions on families is often not quantified. This study compares EA in dyads receiving usual care-early intervention (UC-EI) versus sitting together and reaching to play (START-Play) in addition to UC-EI. METHODS: Data were drawn from 106 children with neuromotor delay who were 7 to 16 months old at baseline (mean = 10.5 months) and from their caregiver (91%

The effect of family therapy on the changes in the severity of on-line game play and brain activity in adolescents with on-line game addiction.

We evaluated whether a brief 3-week family therapy intervention would change patterns of brain activation in response to affection and gaming cues in adolescents from dysfunctional families who met criteria for on-line game addiction. Fifteen adolescents with on-line game addiction and fifteen adolescents without problematic on-line game play and an intact family structure were recruited. Over 3 weeks, families were asked to carry out homework assignments focused on increasing family cohesion for more than 1 hour/day and 4 days/week. Before therapy, adolescents with on-line game addiction demo

Post-traumatic stress disorder following traumatic limb amputation in an 8 year-old female child and its management through play therapy.

Posttraumatic mental health issues following traumatic limb amputations in children have been commonly reported worldwide. Traumatic limb amputationsarecatastrophic injuries/events and often have various long term psycho-social impacts on children. We report a case of 8 year-oldfemale child presenting with symptoms of post-traumatic stress disorder (PTSD) following a traumatic amputation of right hand. A non- directive approach in play therapy was used for management of her symptoms. At 12 months follow up, the child is doing well with no impairment in her overall functioning.

The Calcitonin Receptor Plays a Major Role in Glucose Regulation as a Function of Dual Amylin and Calcitonin Receptor Agonist Therapy.

Amylin treatment improves body weight and glucose control, although it is limited by a short action and need for high doses. Dual amylin and calcitonin receptor agonists (DACRAs) are dual amylin and calcitonin receptor agonists with beneficial effects beyond those of amylin. However, to what extent the additional benefits reside in their higher potency or their targeting of the calcitonin receptor remains unclear. Here we deconstruct the receptors involved in the effects of a DACRA, KBP-088, by comparing it to rat amylin (rAMY), rat calcitonin (rCT), and their combination in obese high-fat die

Integrin alpha1beta1 and transforming growth factor-beta1 play distinct roles in alport glomerular pathogenesis and serve as dual targets for metabolic therapy.

Alport syndrome is a genetic disorder resulting from mutations in type IV collagen genes. The defect results in pathological changes in kidney glomerular and inner-ear basement membranes. In the kidney, progressive glomerulonephritis culminates in tubulointerstitial fibrosis and death. Using gene knockout-mouse models, we demonstrate that two different pathways, one mediated by transforming growth factor (TGF)-beta1 and the other by integrin alpha1beta1, affect Alport glomerular pathogenesis in distinct ways. In Alport mice that are also null for integrin alpha1 expression, expansion of the me

An elastase-responsive "plug-and-play" quercetin-loaded engineered extracellular vesicle platform anchored with anti-PRDX1 antibody for severe acute pancreatitis therapy.

Severe acute pancreatitis (SAP) is a lethal inflammatory disease driven by a severely disregulated redox state, in which Peroxiredoxin 1 (PRDX1) acts as a key but paradoxical regulator. PRDX1 is a protective intracellular antioxidant, however, it acts as a pro-inflammatory DAMP extracellularly. To address this, we developed a "plug-and-play" therapeutic platform. Considering the microenvironment in SAP, we first engineered HEK-293 T cells to overexpress a CD63-Z domain fusion protein, yielding extracellular vesicles (EVs) capable of high-affinity binding to the Fc region of various antibodies.

18FDG-positron emission tomography (PET) has a role to play in the diagnosis and therapy of infective endocarditis and cardiac device infection.

In recent years, molecular imaging with positron emission tomography (PET) and more recently, PET coupled with computed tomography (CT) have made a valuable impact in various clinical arenas, primarily within the field of oncology, but also in cardiovascular medicine, particularly in detecting coronary artery disease and myocardial viability. More recently, PET imaging has been proven useful in the diagnosis and evaluation of inflammation and infection at different organ sites. Application of PET in the case of Infective Endocarditis (IE) is still in its infancy and indeed the value of this ap

Lattice Radiation Therapy Plays a Synergistic Role in the Radical Treatment of Bulky Cervical Cancer: A Case Report and Literature Review.

BACKGROUND: An increasing number of reports showed patients with bulky tumors after lattice radiation therapy (LRT) treatment achieved good local control. However, in these reports, LRT was previously used primarily for palliation. We reported a case that LRT plays a synergistic role in the radical treatment of locally advanced bulky cervical cancer (LABCC) combined with INTERLACE study protocol. METHODS: The patient was a 54-year-old female with LABCC and treated with LRT combined with the INTERLACE study protocol. She received three fractions of 3 Gy each to the gross tumor volume (GTV) and

Radiation Therapy Plays an Important Role in the Treatment of Atypical Teratoid/Rhabdoid Tumors: Analysis of the EU-RHAB Cohorts and Their Precursors.

PURPOSE: Atypical teratoid/rhabdoid tumor (AT/RT) is a rare malignancy of the central nervous system in young children with a dismal prognosis. Prognostic markers have been extensively investigated but have not been validated. The role of radiation therapy (RT) remains controversial. We evaluated the impact of RT as part of multimodality treatment by analyzing data of a European AT/RT cohort. METHODS AND MATERIALS: We retrospectively analyzed data of the European Registry for Rhabdoid Tumors and its precursors. Primary endpoints were progression-free survival (PFS) and overall survival (OS).
