The diagnosis of pleural effusions.

Pleural effusions arise from a variety of systemic, inflammatory, infectious and malignant conditions. Their precise etiological diagnosis depends on a combination of medical history, physical examination, imaging tests and pertinent pleural fluid analyses; including specific biomarkers (e.g., natriuretic peptides for heart failure, adenosine deaminase for tuberculosis, or mesothelin for mesothelioma). Invasive procedures, such as pleuroscopic biopsies, may be required for persistently symptomatic effusions which remain undiagnosed after the analysis of one or more pleural fluid samples. Howev…

Managing pleural effusions.

Malignant pleural effusions result from advanced metastatic disease and can have a devastating effect on patients and their families. The insertion of a tunneled pleural catheter, such as a Tenckhoff catheter, is a treatment option for this patient population. Nurses play a significant role during the patient's journey with the disease process, providing the skills necessary to promote self-care and autonomy, resulting in improved quality of life. In this article, the authors discuss the nursing care of patients who have a Tenckhoff catheter.

Fetal pleural effusions.

The clinical course of primary fetal hydrothorax is unpredictable. Whereas smaller unilateral effusions might remain stable or even regress, this is rarely the case with larger collections. Bilateral effusions, hydrops, preterm delivery and the lack of antenatal therapy are all associated with poor outcome. Once structural and chromosomal anomalies have been excluded, optimal management depends on gestational age, rate of progression, the development of hydrops and associated maternal symptoms. For very large effusions with mediastinal shift, hydrops and/or hydramnios, or when there is rapid e

Eosinophilic pleural effusions.

We present 2 cases of eosinophilic pleural effusion (EPE) seen recently at Straub Hospital. One occurred in a patient with pneumococcal empyema; the 2nd patient had 2 episodes of EPE secondary to malignant histiocytic lymphoma. Eosinophilic pleural effusion (EPE) is defined as having eosinophils exceeding 10% of the pleural fluid WBC differential. EPE is usually exudative, typically accounting for 1% to 8% of all pleural effusions. Up to 30% to 35% of EPE are idiopathic, while other frequent causes include air in the pleural space (30%), and pulmonary infections (10%). Collagen vascular diseas…

Malignant pleural effusions.

Various diseases of the gastrointestinal tract at times are accompanied by an exudative pleural effusion. The exudative pleural effusions resulting from esophageal perforation, pancreatic disease, subphrenic abscess, intrahepatic abscess, splenic abscess, abdominal operations, and diaphragmatic hernia are discussed in this article.

Primary Effusion Lymphoma Presenting As Parapneumonic Pleural Effusion.

Primary effusion lymphoma (PEL) is a rare form of high-grade non-Hodgkin's lymphoma that usually occurs in patients with compromised immunit … retroviral therapy (ART). The emphasis is to investigate immunocompromised patients presenting with pleural effusion for PEL, as it is a rare ailment with a high mortality rate.

Nonmalignant Pleural Effusions.

Although the potential causes of nonmalignant pleural effusions are many, the management of a few, including complicated pleural infections and refractory heart failure and hepatic hydrothoraces, can be challenging and requires the assistance of interventional pulmonologists. A pragmatic approach to complicated parapneumonic effusions or empyemas is the insertion of a small-bore chest tube (e.g., 14-16 Fr) through which fibrinolytics (e.g., urokinase and alteplase) and DNase are administered in combination. Therapeutic thoracenteses are usually reserved for small to moderate effusions that are…

[Pleural lymphatics and effusions].

The pleural lymphatic system has a great absorption capacity. Its most known function is fluid resorption. The pleura which cover the lungs (visceral pleura), the mediastinum, diaphragm and thoracic wall (parietal pleura) are formed by a mesothelial cell layer (mesothelium). This permeable layer is in direct contact with the vascular endothelium. The mesothelium is based over a connective tissue (interstitium) containing the blood and lymphatic vessels. The primary lymphatic vessels drain interstitium but are also in direct contact with pleural space by the stoma or openings, situated in the l

Lipid pleural effusions.

Traditionally, a lipid pleural effusion has been described as milky or turbid appearing. However, lipid effusions may have varied presentations making a diagnosis by appearance problematic. Distinguishing between a chylothorax and a cholesterol effusion, the 2 types of lipid effusions, is essential. A chylothorax develops after injury or obstruction of the thoracic duct, leading to a chyle leak into the pleural space that is characterized by an increased triglyceride concentration and the presence of chylomicrons. In contrast, a cholesterol effusion is a long-standing effusion associated with …

Pleural effusions in the neonate.

PURPOSE OF REVIEW: This review highlights the pathophysiological mechanisms, incidence, clinical features, as well as the diagnosis and treatment of pleural effusions in the neonate. RECENT FINDINGS: Pleural effusions are rare except in hydropic neonates. Elevated pleural fluid/serum immunoglobulin G ratio may be a diagnostic marker for congenital chylothorax in utero. Chylothorax may be congenital or acquired. Hydrothoraces may appear at any time during the neonatal period and are related to infectious and noninfectious aetiologies. Haemothorax is defined as the presence of blood in the pleu…

Pleural effusions in the neonate.

INTRODUCTION: Pleural effusions are rare in the neonate and may be associated to several clinical conditions. Only a few series of pleural effusions in the fetus and newborn are described in the literature. AIM: This study was undertaken to determine more accurately the causes and prognostic significance of pleural effusions in a population of high-risk neonates. MATERIALS AND METHODS: A retrospective chart review of 62 neonates admitted to the neonatal intensive care unit of six medical centers in the north of Portugal, between 1997 and 2004, that presented the diagnosis of pleural effusion…

Eosinophilic pleural effusions.

Eosinophilic pleural effusions, defined as a pleural effusion that contains at least 10% eosinophils, may be caused by almost every condition that can cause pleural disease. Eosinophilic pleural effusion occurs most commonly during conditions associated with the presence of blood or air in the pleural space, infections, and malignancy. Drug-induced pleural effusions, pleural effusions accompanying pulmonary embolism, and benign asbestos pleural effusions are also among the common causes of eosinophilic pleural effusion. No etiology is found in as many as one third of patients. Because studies …

Malignant pleural effusions.

The estimated annual incidence of malignant pleural effusions in the United States is 150,000 cases. Patients most commonly present with dyspnea, initially on exertion and later at rest. Chemical pleurodesis is the most common modality of therapy for patients with recurrent, symptomatic, malignant pleural effusion. Talc is the most successful pleurodesis agent, and talc poudrage and slurry have equal efficacy. Although a number of cases of acute respiratory failure have been associated with talc pleurodesis, the incidence is < 1% and many of these episodes cannot be clearly attributed to talc …

Pleuritis and pleural effusions.

A variety of diseases either directly or indirectly affect the pleura, resulting in the accumulation of pleural fluid. A pleural effusion develops whenever the influx of fluid into the pleural space is greater than the efflux. It is now clear that the parietal pleura has the primary role in the reabsorption of pleural fluid normally and during pathologic conditions. Recently, models of experimental pleuritis have demonstrated the importance of inflammatory cytokines in the pathogenesis of both asbestos- and endotoxin-induced pleural effusions.
