The authors claim that the critical health reform in Chilean history was the establishment of the National Health Service (NHS) in 1952. The development of modern Chilean health care since the end of the 19th century is discussed both in terms of the prevailing health situation and the subsequent evolution of institutions and policies, with an emphasis on the social and political conditions that led to the creation of the NHS in 1952. From this analysis and from a comparison of infant mortality rates among Latin American countries during the same period, the authors infer that the 1952 health

RESULTS: Physical environments were identified as the primary barriers, while technology and political systems were major facilitators. Individuals with moderate/severe disability (90th percentile WHODAS-B.O) demonstrated significantly higher limitations (median 17.2 vs. 9.4). Despite an expanding literature on Ebola-response, few studies detail or reflect on the responses of diverse systems of care. Little is known about how, why or in what ways, strategies of ill-health management were enacted locally, how health-systems power, authority and hierarchy were perceived and contested, or how other social systems, institutions and relationships shaped the response. This paper presents an interdisciplinary analysis of local responses in two early affected districts in Sierra Leone. Drawing on anthropological theories of social ordering and assemblage, we present an anal

Why do some ambitious social health insurance reforms get enacted, while others stall, even within the same country, and under similar structural constraints? This article compares Kenya's failed 2004 National Social Health Insurance Fund Bill, with the reform enacted in 2023. It combines Kingdon's Multiple Streams Approach (MSA) with political economy and populism theory, drawing on 48 elite interviews, and policy documents. Using outcome-explaining process tracing, the study explains why enactment became possible in 2023 but not 2004. MSA highlights how adoption depends on coupling across pr

During the early years of the People's Republic of China, sexually transmitted diseases (STD) were almost eliminated nationwide for a period. In the STDs eradication campaign, "political correctness" became a ruling policy. During the prevention and treatment of the STDs, the choice of therapy depended on the "political correctness" policy rather than medical effectiveness. With this political background, three competing treatments of STDs emerged (especially for syphilis): penicillin treatment, arsenic and bismuth mixed and intermittent treatment, and traditional Chinese medicine treatments.

Despite compelling evidence on the value of prevention, public health remains chronically underfunded and structurally deprioritised. Spending on prevention has remained stable at around 3% of total health expenditure across Organisation for Economic Co-operation and Development countries for the past two decades, and despite a temporary doubling of spending during the COVID-19 pandemic, this trend has already reversed. The central challenge for the next decade lies not in the absence of evidence, but rather the absence of political will and systems capable of translating evidence into action.

ObjectiveThis narrative review examines the impact of the United States on the war against HIV/AIDS and thus the impact of political ideologies on the financing of the medical sector.DiscussionInitially marked by stigma, particularly in the early years of the epidemic, the US response was transformed with the launch of the Presidential Emergency Plan for AIDS Relief (PEPFAR) in 2003. PEPFAR has partnered with entities such as the Global Fund and the Joint United Nations Programme on HIV/AIDS (UNAIDS), retranslating global health governance. Despite this success, the global response remains dep

Clinical protocols are commonly framed as technical instruments for standardizing care and improving patient safety. Their implementation in practice, however, depends not only on the strength of the evidence that underpins them, but also on the organizational and political conditions that enable their continuity over time. In health-care settings characterized by administrative instability, political interference, patronage-based appointments, and high workforce turnover, adherence to protocols becomes especially fragile. This practice-based analysis examines how such conditions affect clinic

BACKGROUND: Disability-adjusted life years (DALYs) are widely used to prioritize public funding for science, technology and innovation (ST&I) in health. In universal systems such as Brazil's Unified Health System (SUS), however, allocation decisions may also incorporate broader political and social considerations.
OBJECTIVE: To assess whether Brazilian public ST&I funding for neglected tropical diseases (NTDs) and Zika (2006-2019) aligned with disease burden measured by DALYs, and to examine how observed funding patterns reflect political commitments to health equity and innovation in the Glo

This study captures and contextualizes experiences of former patients treated in GDR psychotherapy, focusing on the pre-therapeutic phase, triggers, access, expectations, and political attitudes. It addresses a research gap on the patient perspective.Semi-structured interviews with 15 individuals treated between 1974-1989 were analyzed using qualitative content analysis. Categories included triggers, access pathways, expectations, political attitudes, motivations for change, and gender differences. The most frequently reported reason for seeking psychotherapy was current distress, partly in com

The dynamics of the spreading, and the identity of a virulent epidemic, similar to bubonic plague or typhus, which hit Ancient Egypt in the middle of the Bronze Age, are hereby presented. Documented in medical papyri as well as archaeological findings, and re-echoed in biblical texts, a plague entered Egypt's main harbor, Avaris, around 1715 BC. As a result, the country was severely weakened at a time when it was already facing serious sociopolitical issues: crumbling central government, immigration, foreign influence, and the rise in power of the army and of warlords. The Hebrews, one of the

Within the context of the U.S. occupation of Hawai'i, State-sponsored and Hawaiian-led health initiatives cannot be reduced to a binary of "colonial/unhealthy" versus "Hawaiian/healthy." Instead, they operate within overlapping and often conflicting paradigms structured by what we term the Ideology of Occupation. This ideology obscures a central reality: kaumaha-the deep, collective malaise affecting the Hawaiian psyche ( Rezentes, 1996)-is the result of a disruption in the kū'auhau (genealogy) of Hawaiian public health, in which sovereign systems of care were replaced by a bureaucratic appara

Despite increasing evidence about the value and importance of breastfeeding, less than half of the world's infants and young children (aged 0-36 months) are breastfed as recommended. This Series paper examines the social, political, and economic reasons for this problem. First, this paper highlights the power of the commercial milk formula (CMF) industry to commodify the feeding of infants and young children; influence policy at both national and international levels in ways that grow and sustain CMF markets; and externalise the social, environmental, and economic costs of CMF. Second, this pa

BACKGROUND: Spiritual care is an essential component of palliative care, supporting the existential and spiritual wellbeing of patients with serious illnesses. Yet, its effective implementation remains challenging in many healthcare systems. This study examines the experiences and practices of palliative care clinicians in Lebanon, a country characterized by religious diversity, high religiosity, and a complex political landscape.
METHODS: A qualitative focus group study rooted in constructionist ontology. Palliative care clinicians were recruited from community and acute settings in Lebanon.

Political fragmentation continues to block urgently needed power integration in Latin America, despite the region's vast renewable energy potential. Using the Multiple Streams Approach, this study argues that the escalating climate crisis now acts as a focusing event, aligning a pressing energy problem, viable integration solutions, and growing political will. This alignment signals that a policy window is open. This study combines the Multiple Streams Approach with system modeling to quantify overlooked political costs of inaction, specifically, geopolitical tensions and erosion of trust, off

BACKGROUND: Despite comprising more than five million practitioners and standing as society's most trusted profession, nurses remain structurally marginalized within the policy and governance systems that shape the care they deliver (National Academy of Medicine, 2021). The profession stands closest to human suffering yet furthest from the tables where that suffering is addressed.
PURPOSE: This expert commentary examines the paradox of nursing's proximity to need and its distance from power, advancing a framework that converts the profession's collective frustration into strategic action and

The COVID-19 pandemic demonstrated the capacity of the world to rapidly mobilize resources and political will when faced with an immediate, visible global threat. The accelerated development of effective vaccines inspired the Coalition for Epidemic Preparedness Innovations to promote the 100 Days Mission (100DM), an initiative to enable deployment of medical countermeasures within 100 days of identifying a pandemic threat. The success of the 100DM in uniting stakeholders highlights the power of framing challenges to inspire collective action. On the other hand, antimicrobial resistance (AMR) h

The study examined how macro-environmental factors influenced the preparation of the Ukrainian national Olympic team across two distinct crisis contexts-the Tokyo 2020 and Paris 2024 Olympic cycles. A PEST-based expert survey was applied to assess political, economic, socio-cultural, and technological factors. Two validated questionnaires were used. Twenty-four high-performance athletes who participated in the Olympic Games were involved as experts. The structure of macro-environmental influence differs between the two cycles. In the Tokyo cycle, economic factors had the greatest weight (43%),

The World Health Organisation initiated the Global Polio Eradication Initiative in the year 1988. With the large-scale application of routine and mass vaccinations in children under the age of 5 years, polio disease has become restricted to only 3 endemic countries (Afghanistan, Pakistan and Nigeria) by today. However, since the beginning of the 21st century, increasing numbers of secondary polio epidemics have been observed which were triggered through migration, political turmoil and weak health systems. In addition, there emerged serious technical (e. g., back-mutations of oral vaccine viru
