Polytetrafluoroethylene grafts have been used extensively for infrainguinal vascular reconstruction either as the conduit of choice or as a substitute when saphenous vein is unavailable. A randomized, double-blind clinical trial was designed to assess the effect of aspirin (ASA) alone or in combination with dipyridamole (DIP) on the patency rates of expanded PTFE grafts placed in the infrainguinal position. Despite well established methods of vascular surgery, the elderly patient has not been considered a candidate for operation because of a high operative risk and a limited life expectancy. Simplified surgical procedures and the use of new graft material now permit vascular reconstruction with minimal injury to the tissues, little blood loss, and a short operative time. Despite the introduction of new procedures such as pharmacologic reduction of portal pressure and endoscopic sclerotherapy, the role of surgery in portal hypertension must be reconsidered. The aim of this retrospective study was to evaluate the long-term results of mesocaval interposition shunting in the treatment of recurrent variceal bleeding after sclerotherapy failure or in patients with intractable ascites in whom optimal medical treatments failed. Various nonresorbable implants are currently used worldwide for medialization thyroplasty in patients with unilateral vocal cord paralysis. The Gore-Tex (expanded polytetrafluoroethylene) implant was introduced in the late 1990s. At our institution, 27 patients with unilateral laryngeal nerve paralysis had medialization thyroplasty with a Gore-Tex implant during the years 1998 to 2002. The current report documents our first case of endolaryngeal extrusion of a Gore-Tex implant.
