We encountered a case of tuberculosis of a popliteal cyst in a 76-year-old man. He visited our department for treatment of the left knee pain which had not responded to treatment over the previous ten months. At first examination, local rubor, swelling and tenderness on a popliteal cyst were noted. Therefore, curettage of the lesion, including resection of the cyst, was performed. Six weeks later, an abscess had formed in the subcutaneous area over the lateral aspect of the knee, which was cleaned out. The abscess recurred in the same area four months later. At the third operation, curettage o…

Popliteal cysts, often referred to as Baker's cysts, are a common occurrence in the adult knee. Although controversy exists as to the exact indications for treatment, these structures can cause extreme discomfort and morbidity, including pain from rupture and symptoms from neurovascular compromise. Prior to the development of the arthroscope, open treatment of popliteal cysts was not uncommon. Complications such as poor wound healing, cyst recurrence, and knee flexion contractures were reported after such treatment. Owing to the presence of a valve-type structure, also called the posterior tra…

Baker's cyst, or popliteal cyst, is a fluid-filled mass that is a distention of a preexisting bursa in the popliteal fossa, most commonly the gastrocnemio-semimembranosus bursa. This bursa is unique in that it communicates with the knee joint, unlike other periarticular bursae, via an opening in the joint capsule posterior to the medial femoral condyle. Many have theorized that this opening creates a valve-like mechanism in the presence of effusion that contributes to the formation of these cysts in adults. Popliteal cysts rarely manifest alone and are most often found in conjunction with othe…

OBJECTIVE: To review the epidemiology, clinical presentation, pathogenesis, imaging, differential diagnosis, complications, and treatment of popliteal cysts.
METHODS: References were taken from MEDLINE from 1985 to 1998 under the subject "Popliteal Cyst" with subheadings of Radiography, Ultrasonography, and Radionuclide Imaging. Other pertinent references were used. Childhood cysts were excluded.
RESULTS: Depending on the studied population and the imaging technique, 5% to 32% of knee problems may have these cysts, with 2 age-incidence peaks of 4 to 7 years and 35 to 70 years. In older patie…

Popliteal cysts and their complications (rupture, expansion and pseudothrombosis) all of which may simulate deep venous thrombosis of the lower extremity may be diagnosed reliably by ultrasonography. Even small cysts can be detected, their extension can be defined and rupture with leakage of fluid into the surrounding muscles can be diagnosed. Phlebography can be avoided in most cases. Arthrography rarely gives additional information. Ultrasonography of the erect patient is recommended if rupture is suspected.

A 12-year-old boy with recurrent complaints of knee pain and swelling for six months, but without diagnosed rheumatological disease, presented with calf swelling and knee pain of several days duration. Examination revealed findings consistent with juvenile rheumatoid arthritis and a ruptured Baker's cyst. This is the sixth reported case of ruptured Baker's cyst reported in a child less than 14 years old and should alert emergency physicians that calf pain and swelling secondary to a ruptured popliteal (Baker's) cyst may be an early manifestation of juvenile rheumatoid arthritis.

We report five observations of popliteal cysts responsible for clinical features of phlebitis of the calf. In each case, thrombosis of deep veins was outruled by phlebography. Clinical manifestations, which did not respond to efficiently dosed anticoagulant therapy, consistently resolved quickly after treatment of the cyst. Rheumatoid arthritis was the most frequent etiology. These observations induce us to look for a popliteal cyst in patients with signs of phlebitis. The negative phlebography rectifies the clinical diagnosis, thus avoiding prolonged anticoagulant therapy.

Forty-one patients with popliteal cysts were examined radiographically. All cysts consisted of a dilated gastrocnemio-semimembranosus bursa. A valve mechanism at the site of their communication with the joint was demonstrated by a specific radiographic technique in the majority of cases. The radiographic findings in popliteal cyst ruptures simulating deep venous thrombosis are described.

Ultrasound examination of 9 patients with suggested popliteal cyst was performed and the results are described and discussed. Ultrasonography is a diagnostic alternative which may replace arthrography.

Ultrasonography was used to examine 42 patients who were complaining of popliteal and suprapatellar pain. When clinically indicated, patients also underwent phlebography, arthrography, and posterior synovectomy. In 17 patients, Baker's cysts were demonstrated by ultrasonogram. Of these 17 patients, four had confirmation of the cysts by arthrography. One patient had simultaneous deep venous thrombosis, proved by phlebography and arthrography. Five patients underwent posterior synovectomy, and the surgical specimens closely correlated with the dimensions predicted by ultrasonography. Internal ec

Synovial-lined cysts in the popliteal space associated with rheumatoid arthritis or meniscal tears may compress the popliteal vein and produce signs and symptoms suggestive of thrombophlebitis. Because of a presumptive diagnosis of thrombophlebitis, venography may be performed initially and demonstrate compression of the popliteal vein by an extrinsic mass. The correct diagnosis of a popliteal cyst can easily be made with B-mode ultrasonography, and, if desired, confirmed by arthrography. Four cases are presented which emphasize these features.

Popliteal vein compression caused by popliteal cyst was detected in 53-year-old man with venostasis of the lower limb. Deviation of the popliteal artery was also demonstrated by arteriography. The cyst communicating with the knee joint and partly covered by the lining of synovial cells was totally excised without damage to the vein. An analysis revealed hyaluronic acid to be the major component of glycosaminoglycan in the cyst and was 3 or 4 times in excess of that in the synovial fluid. The postoperative course was uneventful and symptoms were almost relieved.
