Porencephaly and transverse limb defects following severe maternal trauma in early pregnancy.

A 13 year old boy with mental retardation, hemiatrophy and left hemiplegia, right hemifacial microsomia and transverse digital amputations is reported. He is moderately mentally retarded, experiences grand mal seizures and has evidence of a porencephalic cyst in the right frontotemporal region on CT scan. At 52 days post conception his mother was involved in a road traffic accident and suffered extensive bruising of the abdomen and concussion. It is suggested that the patient's congenital anomalies are a consequence of hypoxia and thrombo-embolic events sustained during early pregnancy.

A case of tubulinopathy presenting with porencephaly caused by a novel missense mutation in the TUBA1A gene.

BACKGROUND: Tubulinopathies include a wide spectrum of disorders ranging from abnormal ocular movement to severe brain malformations, and typically present as diffuse agyria or perisylvian pachygyria with microcephaly, agenesis of the corpus callosum, and cerebellar hypoplasia. They are caused by the dysfunction of tubulins encoded by tubulin-related genes, and the TUBA1A gene encoding alpha-1A tubulin is most frequently responsible for this clinical entity. Porencephaly is relatively rare among patients with the TUBA1A mutations. Mild case of tubulinopathy associated with porencephaly caused …

Biallelic Variants in the COLGALT1 Gene Causes Severe Congenital Porencephaly: A Case Report.

OBJECTIVE: We describe a third patient with brain small vessel disease 3 (BSVD3), being the first with a homozygous essential splice site variant in the COLGALT1 gene, with a more severe phenotype than the 2 children reported earlier. METHODS: Analysis of whole exome sequencing (WES) data of the child and parents was performed. We validated the missplicing of the homozygous variant using reverse transcription PCR and Sanger sequencing of the mRNA in a lymphocyte culture. RESULTS: The patient presented antenatally with porencephaly on ultrasound and MRI. Postnatally, he showed a severe develo…

Magnetic Resonance and Computed Tomographic Findings in a Case of Encephaloclastic Porencephaly in a Sulawesi Crested Macaque.

A 3-year-old intact female Sulawesi crested macaque (Macaca nigra) was referred for progressive abnormal behavior and ataxia in all four limbs. Computed tomography (CT) and magnetic resonance imaging revealed cystic lesions located in the occipital lobes and in the right cerebellar culmen, together with the presence of communication between the subarachnoid space and lateral ventricles. The lesions showed no contrast enhancement on CT. These imaging findings were consistent with porencephaly, which was confirmed on necropsy and histopathology. An inflammatory meningoencephalitis was diagnosed.

Intracranial hemorrhage progressing to porencephaly as a result of congenitally acquired cytomegalovirus infection--an illustrative report.

OBJECTIVE: To report ultrasound and magnetic resonance imaging (MRI) findings in a fetus with intracranial hemorrhage and porencephaly, presumed secondary to intrauterine cytomegalovirus (CMV) infection. METHODS: A 20-year-old, G2, P1 woman presented at 28.6 weeks' gestation after ultrasound examination demonstrated apparently isolated fetal ascites. Evaluation included maternal serology, amniocentesis, and repeated ultrasound examinations. Fetal MRI evaluation was also performed. The infant was born at 35 weeks' gestational age. RESULTS: Maternal serology was positive for CMV IgG. Intrauter…

Psychosis Associated with Acquired Porencephaly-Cause or Incidental Finding? Case Report and Review of Literature.

Porencephaly, a rare disease affecting the central nervous system, is represented by a cerebrospinal fluid-filled cavity in the brain. There are two types of porencephalic cavities: congenital and acquired. Porencephaly is mainly associated with neurological and developmental consequences. Associated psychotic symptoms were reported in a few cases, and due to this fact, there is a knowledge gap regarding the diagnostic and therapeutic approach to such cases. We present the case of a 32-year-old male diagnosed with a psychotic disorder associated with acquired porencephaly. The porencephalic cy…

X-linked hydrocephalus masquerading as spina bifida and destructive porencephaly in successive generations in one family.

The authors report a case of X-linked hydrocephalus which presented as a destructive porencephaly. There was asymmetric dilatation of the ventricles of prenatal onset, and neuro-imagining studies were suggestive of infection or haemorrhage. The child was profoundly handicapped but did not have adducted thumbs. Two of his mother's brothers had been stillborn, and postmortem reports revealed that the diagnosis had been isolated hydrocephalus and not spina bifida as reported by the family. Despite serial ultrasound scans, recurrence of X-linked hydrocephalus in the mother's subsequent pregnancy w…

Polymicrogyria without porencephaly/schizencephaly. MRI analysis of the spectrum and the prevalence of macroscopic findings in the clinical population.

Although the diagnosis of polymicrogyria currently depends largely on non-invasive imaging, no large imaging-based studies of polymicrogyria have been reported. Previous anatomic studies of polymicrogyria have been based on autopsy studies and most of the cases in those series were associated with porencephaly or schizencephaly. This retrospective MRI analysis of a group of patients with polymicrogyria, without associated porencephaly or schizencephaly, was conducted to elucidate gross morphological findings of polymicrogyria in a clinical population. Seventy-one patients with polymicrogyria d…

De novo and inherited mutations in COL4A2, encoding the type IV collagen α2 chain cause porencephaly.

Porencephaly is a neurological disorder characterized by fluid-filled cysts or cavities in the brain that often cause hemiplegia. It has been suggested that porencephalic cavities result from focal cerebral degeneration involving hemorrhages. De novo or inherited heterozygous mutations in COL4A1, which encodes the type IV α1 collagen chain that is essential for structural integrity for vascular basement membranes, have been reported in individuals with porencephaly. Most mutations occurred at conserved Gly residues in the Gly-Xaa-Yaa repeats of the triple-helical domain, leading to alterations…

Bilateral porencephaly, cerebellar hypoplasia, and internal malformations: two siblings representing a probably new autosomal recessive entity.

We report on 2 sibs with bilateral porencephaly, absence of the septum pellucidum, and pancerebellar hypoplasia including absence of the vermis. Situs inversus and tetralogy of Fallot was present in one, and an atrial septal defect in the other. This constellation of findings is discussed against the background of familial porencephalies and schizencephalies, familial cerebellar hypoplasias, and asplenia/polysplenia syndromes. It is concluded that the described constellation of findings constitutes a new entity of probably autosomal recessive inheritance.

Neonatal alloimmune thrombocytopenic purpura and congenital porencephaly in two siblings associated with a "new" maternal antiplatelet antibody.

We report a brother and sister, both of whom have porencephaly, hydrocephalus, optic atrophy, severe mental retardation, and spastic quadriplegia. In the younger child, abnormal intracranial structure was demonstrated by sonography at 32 weeks' gestation and was suspected earlier. Both children had transient severe thrombocytopenia as newborns. The mother is healthy and has never had purpura or other bleeding symptoms. However, her serum was found to react strongly with platelets from the father and from both children. The antibody in the mother's serum is platelet-specific but does not appear…

The origin of hypsarrhythmia and tonic spasms in West syndrome: evidence from a case of porencephaly and hydrocephalus with focal hypsarrhythmia.

We report on a 3-year-old girl with West syndrome and with focal hypsarrhythmia. The left hemisphere of the patient was virtually completely defective and continuous hypsarrhythmia was only seen in the residual right frontal cortex, where an interictal single photon emission computed tomography (SPECT) showed hyperperfusion. Despite a focal epileptic pattern, the tonic spasms were quite symmetrical. In our patient, spasms might not require the sensorimotor cortex, but the brainstem containing the descending pathways that control spinal reflexes and other infratentorial structures seem to be es

Morning Glory fundus anomaly, coloboma of the optic nerve, porencephaly and hydronephrosis in a newborn infant: MCPH entity.

The association of Morning Glory anomaly with intracranial pathology has been rarely described and seems to be a distinct one. A 41-week full-term infant presenting with Morning Glory anomaly and multiple major and minor anomalies with a normal karyotype is described. Right Parieto-occipital porencephaly with mild hydrocephaly of lateral ventricle, right brain atrophy and enlarged thalami were observed by repeated ultrasound, computed tomography and magnetic resonance imaging examinations. Fundoscopy, visual evoked response and electroretinography revealed bilateral colobomas of the optic nerv…

Case Report: Imaging and pathological features of compartmentalized subdural empyema and pachymeningitis within pre-existing porencephaly in a dog.

A 2-year-old Golden Retriever was presented with progressive intracranial signs. Neurological examination was consistent with multifocal brain disease. Magnetic resonance imaging (MRI) revealed encephaloclastic porencephaly lesions, which were worse on the right side, as well as changes within the fluid compartments, including incomplete fluid-attenuated inversion recovery (FLAIR) signal suppression, ring-like T1-weighted (T1W) contrast enhancement, and increased post-contrast FLAIR hyperintensity without restricted diffusion. Susceptibility-weighted imaging (SWAN) susceptibility was noted thr…
