OBJECTIVES: The pathophysiology of tinnitus is obscure and its treatment is therefore elusive. Significant progress in this field can only be achieved by determining the mechanisms of tinnitus generation, and thus, histopathologic findings of the cochlea in presbycusis with tinnitus become crucial. Temporal bones, brains, and kidneys of 40 patients over 50 years of age were studied histopathologically, paying special attention to angiosclerotic changes. The histopathologic findings were correlated with audiometric and manometric records obtained while the patients were alive. A close relation existed among the lumen narrowing of the internal auditory artery, spiral ganglion atrophy, and hearing loss. The angiosclerotic changes of the Willis' circle, encephalomalacia, and hearing loss were also related.

Age-related hearing impairment, or presbycusis, is a degenerative condition not currently treatable by medication. It is therefore significant that the author, as a patient, experienced a reversal of high-frequency hearing loss during a 2-day period following abdominal surgery with general anesthesia. OBJECTIVE: A proposed mechanism for presbycusis is a significant increase in oxidative stress in the cochlea. The enzymes glutathione S-transferase (GST) and N-acetyltransferase (NAT) are two classes of antioxidant enzymes active in the cochlea. In this work, we sought to investigate the association of different polymorphisms of GSTM1, GSTT1, and NAT2 and presbycusis and analyze whether ethnicity has an effect in the genotype-phenotype associations.

In a longitudinal and an age cohort comparing study the influence of aging and occupational noise exposure on hearing sensitivity was studied. The participants of the longitudinal study were studied at 70, 75, and 79 years of age. Seventy year old men exposed to occupational noise had 10 to 15 dB poorer hearing in the high frequency range than nonexposed men. The difference in hearing acuity decreased with increasing age. Presbycusis is the progressive hearing loss caused by histologic changes due to aging. Schuknecht described four histopathologic types of presbycusis and correlated these findings to audiometric data. In a retrospective study, some strict criteria were set to distinguish the "pure" cases of presbycusis in a group of 1,181 subjects, 50 years of age and older, seen in our Audiology Clinic over one year; 91 subjects were found to meet these criteria.
