Beyond High-Stakes Testing: Learner Trust, Educational Commodification, and the Loss of Medical School Professionalism.

With ever-growing emphasis on high-stakes testing in medical education, such as the Medical College Admission Test and the United States Medical Licensing Examination Step 1, there has been a recent surge of concerns on the rise of a "Step 1 climate" within U.S. medical schools. The authors propose an alternative source of the "climate problem" in current institutions of medical education. Drawing on the intertwined concepts of trust and professionalism as organizational constructs, the authors propose that the core problem is not hijacking-by-exam but rather a hijackable learning environment …

METHODS: A cross-sectional questionnaire-based study was conducted in first and final year medical students at an Irish medical school. The following instruments were administered: (a) Jefferson Scale o…

Seeing health and social care work through the lens of 'connective professionalism': Implications for interprofessional education.

The field of interprofessional education provides educational preparedness for collaborative work. In this cross-cutting edge paper, we argue that aligning educational offerings to diverse current and future configurations of collaborative work is both a goal and a challenge for educators. To continue to move towards this goal, we draw from recent debates in the fields of organizational studies and sociology of professions. In response to the historical focus on protective professionalism, theorists in the intersections of those two fields have begun to debate the concept of 'connective profes…

[De-professionalization or 'para-professionalization'?. The professional development of the midwife and her place in the obstetrics of the 19th century].

The present article deals with the standing and the tasks of midwives in 19th century Württemberg. After outlining the legal framework of obstetrics, special emphasis was given to the education and activities of contemporary midwives, and to their numbers. At the beginning of the 18th century, obstetrics was almost exclusively in the hands of women. After 1800, a growing number of barber-surgeons and physicians began to practise obstetrics. At the same time, vocational training and qualification of women-obstetricians increased considerably. Schools for midwives and lying-in hospitals were fou

METHOD: This retrospective, cross-sectional study used P…

James Edmund Reeves (1829-1896) and the contentious 19th century battle for medical professionalism in the United States.

During his life, Dr James E Reeves was a national figure in the US. His work included multiple professional publications, civic and professional leadership positions, and the drafting of a landmark law that confirmed the right of states to regulate the medical profession. While much of Reeves' work supported the successful struggle of 19th century regular physicians to gain control of the practice of medicine, he challenged his colleagues when their self-interests conflicted with his perception of the public good. He was frequently lauded for this work by physicians and the public but he also

Flourishing under constraint: character education and virtue ethics as the foundation of professionalism in medical education and clinical practice.

Contemporary biomedical ethics has been shaped largely by principle-based and outcome-oriented frameworks, yet these approaches insufficiently account for the role of character in clinical judgement and professional formation on which their fruitful use depends. This paper develops a systems-aware virtue-ethical framework for medical education in which professionalism is understood as a sustained mode of professional agency arising from the coordinated exercise of cultivated moral and epistemic character, clinical competence, and practical wisdom within institutional conditions. Drawing on neo…
