Prosthetic joint infection (PJI) is a devastating complication that results in substantial costs to society and patient morbidity. Advancements in our knowledge of this condition have focused on prevention, diagnosis, and treatment, in order to reduce rates of PJI and improve patient outcomes. Preventive measures such as optimization of patient comorbidities, and perioperative antibiotic usage are intensive areas of current clinical research to reduce the rate of PJI. Improved diagnostic tests such as synovial fluid (SF) α-defensin enzyme-linked immunosorbent assay, and nucleic acid-based test

Over the last several decades, periprosthetic joint infection (PJI) has been increasing in incidence and is occurring in more complex patients. While there have been advances in both surgical and medical treatment strategies, there remain important gaps in our understanding. Here, we share our current approaches to the diagnosis and management of PJI, focusing on frequent clinical challenges and collaborative interdisciplinary care. The more detailed review including diagnosis, surgical considerations, and a detailed antimicrobial discussion is presented in the online version.

The malleable penile implant is often considered an inferior device to the three-piece inflatable penile prosthesis implant. Nonetheless, the malleable prosthesis has its unique advantages such as lower cost, easier to perform and fewer mechanical complications than inflatable prostheses. Furthermore, its role can be extended to patients with issues relating to poor manual hand dexterity, those undergoing a salvage for infection prosthesis and as an emergency surgical measure in patients presenting with acute ischaemic priapism. Over the past few decades, there have been numerous design and te

BACKGROUND: Periprosthetic joint infections (PJI) are among the most severe and complex complications following joint arthroplasty and require a structured interdisciplinary treatment approach. In addition to an adequate surgical strategy, pathogen-directed and patient-specific anti-infective therapy plays a central role in successful management.
METHODS: Based on a comprehensive literature review and current guidelines, followed by a structured consensus process within the interdisciplinary working group of the "German Society for Arthroplasty (Deutsche Gesellschaft für Endoprothetik e. V.)"

Candida species (Candida spp.) infection following total hip arthroplasty (THA) is a rare but potentially devastating complication. The purpose of this study was to provide an evidence-based summarization of the treatment and outcome of Candida spp. infection after THA. We conducted a systemic review of the literature and pooled analysis of Candida spp. infection after THA published between 1950 and 2014. A total of 20 articles with 37 patients were identified. Median time between THA and readmission for Candida spp. infection was 25 months (range 0.5-184 months). Thirty-two patients (87%) und

Orthopaedic implant material can get infected via haematogenous spread from a distant source at any point after implantation. The sources of haematogenous orthopaedic implant infections have been studied only for prosthetic joints. The most common source of infection has varied, but it can be, for example from the skin and soft tissues, cardiovascular system and dental infections. The risk for developing a periprosthetic joint infection (PJI) during bacteraemia is dependent on the pathogen: it is highest for Staphylococcus aureus and beta-haemolytic streptococci, but low for gram-negative bact

Penile prosthetic implantation is a successful procedure for the management of male erectile impotence. However, infection remains the most serious complication requiring removal of the device. Later reinsertion can be difficult due to fibrosis and a shortened penis. We present 3 cases of penile infection with Staphylococcus epidermidis in which a new penile prosthesis was placed after 72 hours of continuous irrigation of the corpora cavernosa with rifamycin. The procedure requires judicious selection of patients and a stable clinical status.

Recurrent periprosthetic joint infection (PJI) is a highly morbid complication following total joint arthroplasty. Despite appropriate medical and surgical management with targeted antimicrobial therapies, many patients who suffer from a PJI experience clinically relevant episodes of recurrence. Though the majority of these recurrent infections are due to microbes that are different from the species initially isolated at the index infection, approximately 15 to 50% of subsequent PJIs are attributed to recurrence with the same causative organism. Treatment resistance, contamination during revis

BACKGROUND: Abdominal aortic graft infections are a major complication following abdominal aortic aneurysm surgery, with high morbidity and mortality rates. They can be treated surgically or conservatively using medical management. The two most common surgical techniques are in situ replacement of the graft and extra-anatomical bypass. Medical management most commonly consists of a course of long-term antibiotics. There is currently no consensus on which intervention (extra-anatomical bypass, in situ replacement, or medical) is the most effective in managing abdominal aortic graft infections.

Infective endocarditis (IE) is an uncommon clinical entity that, if unrecognized, leads to serious morbidity and mortality. Approximately 15,000 new cases of IE occur in the United States each year. Despite advances in early diagnosis, antimicrobial treatment, and surgical techniques, reported mortality from referral centers has changed little throughout several decades. Early recognition of IE requires understanding of its epidemiology, risk factors, clinical presentations, physical examination signs, microbiological associations, and electrocardiographic and chest radiographic findings. We present the case of a 77-year-old woman who developed an Actinomyces naeslundii infection of a hip prosthesis. The isolate grew well aerobically with 5% CO(2). Possible diagnostic problems may arise in the microbiological laboratory because aerobic growth is not sufficiently accounted for in some of the traditional identification schemes and commercial test kits. Therefore, besides presenting an unusual pathogen in this setting, this report focuses on possible diagnostic problems in the microbiological laboratory.

PURPOSE: Aortoduodenal fistulas are the most frequent aortoenteric fistulas. They may be primary (occurring after aneurysms of the native aorta) or secondary (occurring after aortic prosthesis). Aortoduodenal fistulas are a rare complication of aortic prostheses. They may be caused by prosthesis infection or due to inadequate prosthesis.
METHODS: We report seven observations that emphasize issues pertaining to either diagnosis or therapy.
RESULTS: The delay of occurrence is variable, with a mean of 3 years as reported in the literature. Clinical picture includes upper digestive tract hemorrh

AIMS: The aim of this study was to evaluate the overall mortality rate in patients with periprosthetic joint infection (PJI) after total knee arthroplasty (TKA), the overall mortality over time, and mortality after treatment.
METHODS: Ovid MEDLINE, Embase, CINAHL, Cochrane Library, Scopus, and Web of Science, were searched for studies reporting the rate of mortality in patients with TKA PJI until December 2023. Full-length studies with extractable data dealing with mortality in these patients were included. Studies were excluded if they involved TKAs which were undertaken for oncological or t

Prosthetic vascular infections are rare, occurring in less than 3 % of cases. If left untreated within the required timeframe, serious complications such septic shock and anastomotic rupture with hemorrhage may occur and lead to the patient's death. Targeted and prolonged intravenous or oral antibiotic therapy is necessary but insufficient without surgical revision and removal of the infected prosthetic material. Once the infected prosthetic material is removed, blood continuity must be restored to ensure downstream vascularization and avoid amputation. There are two types of repair: extra-ana

BACKGROUND: The debridement, antibiotics, and implant retention (DAIR) procedure is an established therapeutic option for periprosthetic knee infections (PKI). However, the efficacy and the indication for this procedure are still controversial.
METHODS: All the relevant literatures were systematically reviewed and analyzed. The present study aimed to assess the success rate of DAIR in the management of PKI, identify the factors associated with prognosis of DAIR, and establish a simple algorithm for predicting a high success rate of DAIR.
RESULTS: Totally, 33 studies with 1266 cases were incl

Although the clinical results of total joint arthroplasty are usually excellent, some implants develop loosening and require revision. Implants usually fail by a combination of mechanisms, but different basic designs tend to show different dominant mechanisms of failure. Infection causes failure of about 1-5% of cases of primary arthroplasty. Clues to the presence of infection include clinical signs, a periosteal reaction, a positive culture of aspirated joint fluid, and acute inflammation identified in tissue around the implant. There are several different mechanisms and modes of implant wear

INTRODUCTION: Refractory prosthetic joint infections (PJI) of the hip remain a major challenge in orthopedic surgery. The Girdlestone procedure, also known as resection arthroplasty, was originally described for native hips but has since been adopted as a salvage option for complex and recurrent prosthetic infections. The main objective of this study was to evaluate the effectiveness of this procedure in treating prosthetic hip infections in this context.
MATERIALS AND METHODS: Eighty-eight patients (91 hips) who underwent femoral resection arthroplasty for refractory hip prosthesis infection

Computed tomographic (CT) scans of 19 patients with 10 right and nine left orbital implants were reviewed. Except for orbital soft-tissue swelling due to recent surgery or infection, CT scans obtained with both the implant and prosthesis in place showed relative symmetry of the postoperative side and the native globe. Benign air collections were often associated with either the implant or prosthesis interface or with seating of the prosthesis in the conjunctival fornices. Six patients had either cartilage, silicone, or glass beads placed surgically along the orbital floor to elevate the implan
