Prurigo nodularis imitators unmasked.

Prurigo nodularis (PN) is characterized by chronic, nodular lesions on the extensor surfaces of the extremities and back. For experienced clinicians, the recognition of this diagnosis is usually not difficult; however, it is essential to consider differential diagnoses and to identify PN imitators. We report five patients with severe chronic pruritus who presented with skin lesions resembling PN. They were treated accordingly but were subsequently diagnosed with a different underlying disease, including nodular marginal zone lymphoma (1 case), delusional parasitosis (1 case), well-differentiat…

[Prurigo pigmentosa during pregnancy].

INTRODUCTION: Prurigo pigmentosa is a rare inflammatory dermatosis characterized by pruritic and reticulate papules on the trunk leaving hyperpigmentation. This dermatosis has been rarely described outside Asia. The pathophysiology remains obscure. CASE REPORT: We report the case of a 21-year-old North-African woman presenting with a highly pruritic eruption with numerous erythematous macules and papules coalescing in a reticular pattern on the trunk. The eruption occurred during the first trimester of pregnancy, which was marked by severe vomiting resulting in weight loss of 13kg with ketone…

Comorbidities in prurigo nodularis.

Prurigo nodularis (PN) is a chronic inflammatory skin condition characterized by intense pruritus and hyperkeratotic nodules commonly found on the trunk and extremities. The pathogenesis of PN involves neuroimmune dysregulation, mediated by various inflammatory cytokines and neuropeptides. PN significantly impacts patients' quality of life. There is greater disease severity, increased itch intensity, and a more pronounced reduction in quality of life in PN compared with other individuals with chronic pruritus. PN is associated with a range of systemic comorbidities, including chronic kidney di…

Update on Chronic Prurigo.

Chronic prurigo is itself a common condition, but it can also occur secondary to a large number of diseases. Management is challenging as historically chronic prurigo has been poorly defined and very few treatments are available. Clinically, it presents as excoriated, hyperkeratotic lesions. When chronic prurigo is suspected, a comprehensive differential diagnosis is essential. New diagnostic criteria have appeared in recent years and new drugs have been developed. Although no truly effective treatment is yet available, patients will benefit from a greater understanding of this condition.

Histopathology of actinic prurigo.

Actinic prurigo (AP) is an idiopathic familial photodermatosis seen in American Indians. We report on 17 patients; 16 had dermatitis and one had actinic cheilitis. Ten patients had acute dermatitis and six had chronic dermatitis. The histologies of acute AP and polymorphous light eruption (PLE; limited concept) are eczematous and indistinguishable. Both show spongiosis, superficial (and sometimes deep) perivascular lymphocytic infiltrates, and papillary dermal edema. Both also show vacuolar degeneration of the basal layer. In contrast, the chronic lichenified AP lesions are associated with mar…

Prurigo nodularis and uremia.

Patients with uremia frequently have generalized pruritus that is usually related to a disturbance of calcium and phosphorus metabolism. This report describes three patients with uremia who had prurigo nodularis due to localized scratching and rubbing. The diagnosis of uremia in one case was made when the patient was hospitalized for evaluation of skin lesions.

Cryotherapy improves prurigo nodularis.

A 55-year-old black woman with an eight-year history of prurigo nodularis was treated successfully with blistering cryotherapy. Cryotherapy resulted in smooth macule formation and afforded extended relief of pruritus. Several standard modalities of treatment had been used previously and had resulted in only partial relief of symptoms and minimal reduction of nodule size. This case demonstrates that cryotherapy can be a useful therapeutic alternative for prurigo nodularis.
