Inverse psoriasis is clinically defined by chronic inflammatory lesions in intertrigineous areas. Colonisation or infection with Candida ssp. or bacteria is common. The disease-related quality of life is significantly reduced especially regarding sexual behavior. After the exclusion of relevant differential diagnoses, therapy should be adapted to the clinical outcome and potential comorbidities. Substances which are efficacious in psoriasis vulgaris are generally efficacious in inverse psoriasis, but have to be used off-label. Controlled clinical studies are only available for topical ascomyci…

The pustular forms o f psoriasis make up a heterogeneous entity from a clinical point of view. However, the existence of clinically detectable aseptic pustular lesions is common to all these forms. How they are related to plaque psoriasis, also called psoriasis vulgaris, the most frequent form of psoriasis, and the genetic and molecular mechanisms recently updated in some forms, have renewed interest in these pustular forms, resulting in reconsideration of their place within cutaneous and systemic inflammatory diseases.

BACKGROUND: Psoriasis is associated with several extracutaneous manifestations of which ocular complications are common. Signs and symptoms of ocular psoriasis may be subtle and overlooked. The dermatologic literature has generally underaddressed these complications; however, a thorough understanding of ophthalmic involvement is important to the comprehensive care of patients with psoriasis.
OBJECTIVE: We sought to provide a complete and up-to-date clinical guide on the manifestations and diagnostic considerations of ocular psoriasis.
METHODS: PubMed and Google Scholar were used to find prim…

There are conflicting reports in the literature concerning the use of antimalarials in psoriatic patients with arthropathy or coexisting systemic lupus erythematosus. On the basis of a review of 18 publications in English, it was estimated that up to 18% of patients with psoriasis would develop an exacerbation of their disease following antimalarial therapy. In contrast to lithium and beta-blockers, antimalarials do not induce psoriasis de novo, but they only trigger already existing psoriasis, via a pharmacologic mechanism, probably due to an alteration of the activity of enzymes involved in …

Psoriasis is a common skin disease in infants, children, and adolescents. A review of the clinical, epidemiologic, genetic, and therapeutic aspects of childhood psoriasis is presented. Population studies indicate that the first signs of psoriatic lesions occur in the pediatric age group, birth to 18 years of age, and that both genetic and environmental factors interact to precipitate the development of psoriasis. Koebner reactions are the result of external or internal triggering factors, such as physical injury to the skin, low humidity, and certain drugs. The most frequently observed variant…

UNLABELLED: A MULTIFACTORIAL ORIGIN: Psoriasis is an inflammatory dermatosis resulting from abnormal epidermal homeostatis and characterized by hyperproliferation and abnormal keratinocyte differentiation as well as activation of the skinis immune system. This dermatosis is a prototype of multifactorial conditions implicating hereditary and environmental pathogenic mechanisms. TWO TYPES OF PSORISASIS: Familial psoriasis is observed early in life and is strongly associated with the major histocompatibility complex, particularly CW6, DR7, B1, and B57 molecules. The sporadic non-familial form dev…

Pustular psoriasis may appear as localized pustular psoriasis, which runs a chronic course, or, in a more severe state, generalized pustular psoriasis. Precipitating factors influencing both local and generalized forms include various drugs (eg, lithium, hydrochloroquine), irritative topical therapy (eg, coal tar), dental and upper respiratory infections, pregnancy, and solar irradiation. Both adults and children are affected, but infantile and juvenile pustular psoriasis is rare. It is recommended that provoking factors be eliminated as the first line of management. Short-contact anthralin th…

Prevention and early treatment with appropriate antibiotics administered at the onset of upper respiratory infections in children with psoriasis may be able to block the appearance of acute guttate psoriasis. A careful screening for a focus of infection is also indicated, especially in children.

We report 14 infants younger than 2 years of age with psoriasis vulgaris in order to call attention to the existence of infantile psoriasis. Three of the patients were noted to have psoriasis in the neonatal period; the onset may have been prenatal in one. In 13 of the 14 patients the site of onset was determined by the isomorphic effect (Köbners phenomenon). Although nail pitting is usually said to be infrequent in childhood psoriasis, 11 (79%) of our patients had typical psoriatic nail changes. Psoriasis must be considered in the differential diagnosis of papulosquamous disorders in infants.

BACKGROUND: Methotrexate remains a valuable option for the treatment of psoriasis. This report will summarize studies regarding the use of methotrexate since the last guidelines were published in 1998.
OBJECTIVE: A task force of the National Psoriasis Foundation Medical Board was convened to evaluate treatment options. Our aim was to achieve a consensus on new updated guidelines for the use of methotrexate in the treatment of psoriasis.
METHODS: Reports in the literature were reviewed regarding methotrexate therapy.
RESULTS: A consensus was achieved on use of methotrexate in psoriasis inclu…
