The treatment of pterygium.

The treatment of pterygium is still quite controversial, with various treatments being advocated in the scientific literature. Unfortunately, there are very few well-conducted controlled clinical trials of treatments. However, years of anecdotal and noncontrolled studies have confirmed that some methods, such as bare scleral closure, are no longer acceptable in the treatment of pterygium and that other methods are likely to be more useful. In the future it will be important to develop a grading system, and surgeons will need to be conservative in the treatment of pterygium until such time as a…

Pathogenesis of pterygium.

This paper reviews the histological and epidemiological characteristics of pterygium which suggest that chronic exposure to ultraviolet radiation plays an important role in the pathogenesis of this disorder. However, there is conflicting evidence indicating that chronic inflammation from other causes can induce the disease. The results of two epidemiological surveys undertaken in Southern Africa show that pterygium is not closely linked to other chronic actinic disorders such as pinguecula and climatic droplet keratopathy. Pterygia differ from these disorders by their vascularity which is prob…

Pterygium in Qatar.

The incidence of pterygium in Qatar is 6.2%, which is less than expected. Laying stress on the climatic conditions, the etiological factors have been discussed. 450 cases of pterygia were operated on by a modified bare scleral technique followed by beta-irradiation. No recurrence occurred in 90% following the first operation. The final good result (98.8%) is in favour of the adopted surgical technique and post-operative care.

The management of pterygium.

A rationale for the management of pterygium, based on theory and clinical experience, is presented in this review. Pterygium is classified as primary, secondary (recurrent), or pseudopterygium. Empirical data and proposed theories of pterygium pathogenesis and recurrence are discussed. The treatment of pterygium based on this information is divided into four phases: diagnosis, medical therapy, surgery, and postoperative adjunctive measures. The indications, techniques, results, and complications of the postoperative adjunctive measures used to lower recurrence are considered. Guidelines for th…

Pterygium: new insights.

Pterygia are common conjunctival degenerations with well-documented risk factors but an unclear pathogenesis. Better understanding of the pathogenesis of pterygium could lead to improved surgical outcomes and decreased postoperative recurrence. Currently, pterygium excision with conjunctival autograft remains the preferred surgical technique to decrease pterygium recurrence. Many adjuvant therapies have been used in pterygium surgery to varying degrees of success. Topical cyclosporine, an immunosuppressive medication, in conjunction with conjunctival autograft was found to be most successful i…
