The authors present an overview of the technique of pancreatoduodenectomy with pylorus preservation. The main technical variants since the original description by Traverso and Longmire in 1978 are described. Advantages and drawbacks of the technique from a carcinologic and functional point of view are presented and discussed. Pancreatoduodenectomy with pylorus preservation appears to be the procedure of choice in most cases. However, it should be avoided in a few patients with invasion of the proximal duodenum by a periampullary carcinoma for whom a curative resection requires the addition of …

30 cases of pancreaticoduodenectomy with pylorus preservation are reported. The operative mortality has been 6,6%. Technical details of the operation are discussed and the importance of maintaining an optimal vascularization of the duodenal wall is stressed. Long term results (maximum follow-up 2 years) seem to be excellent with a clear improvement of the digestive function as compared to the traditional pancreatectomy with gastric resection. Jejunal ulcerative complications have not been observed. It is believed that pylorus preservation should become the operation of choice in pancreatectomy…

Further clinical observation and laboratory evaluation will be necessary before we can recommend this modification of the usual technique of pancreaticoduodenectomy to preserve the pylorus and distal part of the stomach in patients in whom the resection is performed for benign disease or for carcinoma of the distal part of the duodenum. However, we believe that the early results in the two patients described are encouraging.

A case of chronic pancreatitis localized in the head of the pancreas with pancreas divisum was treated by laparoscopic pylorus-preserving pancreatoduodenectomy. The laparoscopic technique of resection and reconstruction with a gastrojejunostomy, hepaticojejunostomy, and pancreaticojejunostomy is described. The postoperative period was complicated by a jejunal ulcer and delayed gastric emptying necessitating a prolonged hospitalization and intravenous hyperalimentation. No fistulas occurred, a follow-up CT scan revealed no pancreatic abnormalities, and the patient was discharged in good conditi…

Smith and Tuttle described in 1969 for the first time a double pylorus; since then this anomaly has been found repeatedly. The pathogenesis is still discussed; the anomaly may be congenital or acquired. A case report is given of a patient with double pylorric ulcer.

BACKGROUND: Delayed gastric emptying (DGE) is a frequent complication after pylorus-preserving pancreatoduodenectomy. Recent studies have suggested that resection of the pylorus is associated with decreased rates of DGE. However, superiority of pylorus-resecting pancreatoduodenectomy was not shown in a recent RCT. This meta-analysis summarized evidence of the effectiveness and safety of pylorus-preserving compared with pylorus-resecting pancreatoduodenectomy.
METHODS: RCTs and non-randomized studies comparing outcomes of pylorus-preserving and pylorus-resecting pancreatoduodenectomy were sear…

BACKGROUND: The aim of this meta-analysis was to compare the long-term survival, mortality, morbidity and the operation-related events in patients with periampullary and pancreatic carcinoma undergoing pylorus-preserving pancreaticoduodenectomy (PPPD) and pylorus-resecting pancreaticoduodenectomy (PRPD).
METHOD: A systematic search of literature databases (Cochrane Library, PubMed, EMBASE and Web of Science) was performed to identify studies. Outcome measures comparing PPPD versus PRPD for periampullary and pancreatic carcinoma were long-term survival, mortality, morbidity (overall morbidity,…

OBJECTIVE: To determine in a prospective randomized controlled trial (RCT) whether pylorus-resecting pancreatoduodenectomy (PrPD) with preservation of nearly the entire stomach reduces the incidence of delayed gastric emptying (DGE) compared with pylorus-preserving pancreatoduodenectomy (PpPD).
BACKGROUND: Several RCTs have compared PpPD and conventional pancreatoduodenectomy with antrectomy. However, no study has reported the difference between PrPD with preservation of nearly the entire stomach and PpPD.
METHODS: One hundred thirty patients were randomized to preservation of the pylorus ri…

BACKGROUND: The aim of this study was to describe the procedure of laparoscopically assisted pylorus-preserving gastrectomy (LAPPG), and to evaluate intra- and postoperative conditions of the patients.
METHODS: We performed pylorus-preserving gastrectomy under laparoscopic observation, including regional lymph node dissection in early gastric cancer of the lower body. Seven patients were treated (two women and five men) using this procedure between April 1996 and April 1999.
RESULTS: All the patients showed good postoperative recovery, and no signs of recurrence were recognized. There were n…

Pylorus-preserving pancreatododenectomy is the resection of choice for patients with carcinoma of the head of the pancreas and periampullary area and for certain patients with chronic pancreatitis. Preoperative preparation, operative technique, and results are discussed.

Pylorus preserving pancreatoduodenectomy (PPPD) was reintroduced 12 years ago. Since that time, over 400 patients have undergone PPPD with approximately 41 per cent having chronic pancreatitis and 54 per cent having pancreatic and other periampullary malignancies. Reported 5-year survivals in this latter group have been comparable to those achieved by the classic Whipple procedure. The postoperative mortality rate in 339 reported patients has been 3.8 per cent. Postoperative morbidity, including delayed gastric emptying, has been similar to that of the classic Whipple operation. However, PPPD …
