Sensory changes and lower limb reflexes were observed in 35 patients receiving segmental epidural analgesia in the mid-thoracic region. Sensory blockade was confined to the thoracic and upper lumbar segments. The lower limb reflexes changed to an "upper motor neurone' pattern, with the onset and development of segmental blockade. The sequence of changes observed was hypertonic tendon reflexes, then the onset of ankle clonus, and finally the plantar response changed to a positive Babinski sign. With regression of analgesia the reflexes returned to normal in the reverse order. The significance o

The evolution of tone and reflexes from 25 weeks postmenstrual age (gestational age plus chronologic age) to term in a population of 42 surviving infants is described. The infants were born in 1983 at the Johns Hopkins Hospital, had birth weights less than 1300 g, were examined weekly until neonatal intensive care unit discharge, and did not develop cerebral palsy. Lower-extremity flexor tone was first detectable at 29 weeks post-menstrual age by the popliteal angle and heel to ear maneuvers. Flexor tone, recoil, and hyperreflexia were all noted 2 to 3 weeks earlier in the lower extremities (3

The neurological deficits caused by tethered cord syndrome do not necessarily follow a dermatomal pattern of the affected spinal cord from caudal to cephalic fashion. The authors defined these neurological deficits as "skip lesions", and reviewed the neurological deficits in tethered cord syndrome. Among 29 cases, inconsecutive segmental motor weakness in 3 cases, inconsecutive segmental sensory impairment in 6 cases, positive Babinski reflex in 8 cases, supranuclear neurogenic bladder in 3 cases and lumbago or leg pain in 4 cases were observed and they were thought to be compatible with the n

The classification of disorders of cerebral circulation (DCC) was presented on the basis of clinical-neurosonic study of 100 newborn children with the signs of DCC. There were-determined the same degrees of DCC severity as in hypoxic-ischemic encephalopathy (that is light, manifested, severe ones). There weren't observed any signs of focal damages of brain in light form of DCC, although there might sometimes be found the small ischemic damages of brain and injuries of hemorrhagic type in one third of patients. In manifested DCC there were revealed the signs of focal cerebral damages which were

The late 19th/early 20th century heralded a period in neurology when there was claim and counter claim about the founders of new reflex movements of the great toe. The cutaneous plantar reflex is undoubtedly one of the most important signs in clinical neurology, and there have been several methods described of eliciting this sign, each with its own eponymous name. This article reviews the background to these claims and the contribution of the various authors, to the discovery, clinical relevance and ease of eliciting reflex movements of the great toe.

BACKGROUND AND PURPOSE: The hereditary spastic paraplegias (HSP) are characterized by progressive spasticity of the lower limbs, mostly inherited as an autosomal dominant trait. Analyses of large HSP pedigrees could help to better characterize the phenotype due to a single causative mutation. Patients in a seven-generation kindred carrying a large deletion in SPAST/SPG4 are described.
METHODS: Individuals originating from Sardinia were clinically and genetically studied.
RESULTS: Sixty-seven subjects carried a heterozygous deletion encompassing exons 2-17 of SPAST. Fifty patients (53.2 ± 15.

Joseph Babinski (1857-1932) was born on November 17, 1857. He worked in a clinical arena dominated by Charcot and a focus on hysteria. His primary aim was in trying to find the reliable clinical signs to distinguish organic from non-organic disease of the nervous system. He was considered as masterly diagnostician, relying considerably less on neuropathological reports. Babinski's first attention to the reflex of the toes occurred during a chance observation of the contrasting responses between two female patients, one a hysteric and the other a hemiplegic. He first published description of hi

We describe a 41 years old woman who 17 years ago presented hypotonia and proximal muscular weakness in the upper and lower limbs. On neurological examination, the biceps, triceps and Achilles reflexes were absent; the brachioradialis reflexes were decreased and the patellar reflexes were normal. There was bilateral Babinski sign. The remainder of the neurological examination was unremarkable. In the investigation a myopathic pattern was found in the electromyography. The nerve-conduction study was normal; a ELISA method for HTLV-I antibodies was positive in the blood and in the cerebral spina

The offspring of rats fed a diet containing 10% (v/v) of ethanol were tested for developmental differences from controls on a variety of maturational events and reflexes. Results indicated slower development of fur onset and ear (pinnae) opening, more rapid and vigorous righting responses and shorter dowel-grasping times in the offspring of ethanol-fed dams. No differences in rooting reflex, Babinski reflex, or in the visual placing response were found. These findings were seen as consistent with the interpretation that gestational alcohol exposure results in reduction in the ability of subjec…

In 1896, Joseph François Babinski first described his well-known sign of dorsiflexion of the big toe on stimulating the sole of the foot. However, unknown to Babinski, several painters had previously demonstrated this phenomenon in their paintings. Sandro Botticelli (1445-1510), a Florentine Renaissance painter, demonstrated this reflex in his Madonna and Child with Angels 400 years before the publication of Babinski's discovery. Botticelli used live infants as models for his paintings. Gentile da Fabriano (d 1427) in his Adoration of the Kings, demonstrates a similar response of toe extension

The presence or absence of a Babinski sign can be puzzling, but in the light of existing pathological studies it is more fruitful to consider which pyramidal tract fibres release it than whether they release it. This was investigated clinically, by looking for correlations with other reflex changes and with motor deficits in the leg. A survey of 50 patients with a unilateral Babinski sign and six patients who lacked it in spite of other pyramidal tract signs was supplemented with follow-up of the patients who had acute lesions. Appearance of the Bibinski sign proved to depend on the interactio

In this case report, we present a patient with rare diagnosis of amyotrophic lateral sclerosis (ALS). In this patient, four different neurological disorders were diagnosed in a short-time period of 8 months. The first three diagnoses, chronic low back and leg pain, a left frontal gemistocytic astrocytoma WHO grade 2 and suspected Lyme's disease, could not fully explain the signs of physical examination. Finally, the diagnosis of ALS could reduce these signs to the same denominator.

Electromyographic studies of anterior tibial muscles supplemented by simultaneous induction of Babinski's reflex indicated the appearance of bioelectrical activity in the extremities contralateral to the tumour which suggested the generalization of tone alterations and helped determine the laterization of the pathological process even when there were no clinical manifestations of motor deficit. Having analyzed their own and literature data, the authors came to the conclusion that the contralateral response observed on the electromyogram in the form of prolonged bioelectrical activity with simu

BACKGROUND: The Modified Ashworth Scale (MAS) is the most popular clinical measure of spasticity. Other clinical signs of spasticity include hyperactive tendon reflexes, myoclonus and Babinski sign.
PURPOSE: To assess reliability of the MAS with myoclonic and tendon reflex examination (MTR) in patients with spinal cord injury (SCI).
MATERIAL AND METHODS: 30 patients (16 with complete and 14 with incomplete neural deficit) who sustained cervical SCI 4-66 months prior to the study. Mean age 33,9 years (SD=14,7). 6 independent observers rated MAS and MTR in each patient.
RESULTS: Poor interrat

Amyotrophic lateral sclerosis (ALS) is a progressive, degenerative neuromuscular condition. The procedure and difficulties involved in the clinical diagnosis of ALS have been the subject of numerous investigations. The understanding of the genetics and the epigenetics of the disease is still at infancy with several missing links. We present a case report of a 73-year-old woman suffering from bulbar onset ALS with a 4-month history of progressive dysphagia and dyspnea. She displayed tongue fasciculations and muscle atrophy. The bilateral palmomental reflexes, snout reflex, Hoffman, Babinski, di
