Refractive enhancements for residual refractive error after cataract surgery.

PURPOSE OF REVIEW: Advances in cataract surgery have allowed surgeons to achieve superior refractive outcomes but have also led to higher patient expectations. Despite ever-evolving technology, residual refractive errors still occur. Postcataract refractive enhancements may be required to deliver satisfactory visual outcomes. This review aims to discuss the potential causes of residual refractive errors and the various enhancement modalities to correct them. RECENT FINDINGS: A thorough preoperative workup to detect and address underlying pathologic causes of impaired vision should be performe

Polynomial decomposition method for ocular wavefront analysis.

Zernike circle polynomials are in widespread use for wavefront analysis because of their orthogonality over a circular pupil and their representation of balanced classical aberrations. However, some of the higher-order modes contain linear and quadratic terms. A new aberration series is proposed to better separate the low- versus higher-order aberration components. Because its higher-order modes are devoid of linear and quadratic terms, our new basis can be used to better fit the low- and higher-order components of the wavefront. This new basis may quantify the aberrations more accurately and

Cataract surgery on the previous corneal refractive surgery patient.

Cataract surgery in cases with previous corneal refractive surgery may be a major challenge for the ophthalmologist. The refractive outcome of the case deserves special attention in the preoperative planning process, which should be tailored for the type of prior refractive procedure: incisional, ablative under a flap, or on the corneal surface. Avoiding refractive surprise after cataract surgery in these cases is principally dependent on the accuracy of the intraocular lens calculation, together with the selection of the appropriate biometric formula for each case. Modern techniques for catar

Laser refractive surgery.

BACKGROUND: Refractive errors; myopia, hypermetropia and astigmatism, have traditionally been corrected by glasses and more recently contact lenses. For nearly 100 years surgical techniques to treat such disorders have also been developed. The latest of these are based upon the use of the excimer laser to ablate the corneal stroma either directly under the corneal epithelium (surface PRK) or under a flap of superficial stroma (LASIK). OBJECTIVE: To look at the development of laser refractive surgery and to put this in the context of appropriate and inappropriate indications for usage. DISCUS

Toric implantable collamer lens for keratoconus.

Keratoconus is a progressive non-inflammatory thinning of the cornea that induces myopia and irregular astigmatism and decreases the quality of vision due to monocular diplopia, halos, or ghost images. Keratoconus patients unfit for corneal procedures and intolerant to refractive correction by spectacles or contact lenses have been implanted toric posterior chamber phakic intraocular lenses (PC pIOLs) alone or combined with other surgical procedures to correct the refractive errors associated with keratoconus as an off label procedure with special informed consent from the patients. Intracorneal ring segments in ectatic corneal disease - a review.

The purpose of this review is to collect and summarize all the scientific literature regarding the use of intracorneal ring segments (ICRS) in corneal ectatic disease. These implants, initially designed to correct myopia in normal eyes, are implanted in the deep corneal stroma with the aim of achieving modifications to the corneal curvature and subsequently refractive adjustments. Colin et al. in 2000 were the first to report the efficacy of these implants in reducing the refractive error and corneal steepening in keratoconus eyes. Two main types of ICRS have been developed and used for the tr

Biomechanically Informed Patient-Specific in Silico Models for Laser Refractive Surgery.

PURPOSE: Corneal biomechanics plays a key role in the planning and outcomes of laser refractive surgery. This study presents a validated methodology for simulating patient-specific refractive treatments, focusing on the optomechanical effects of the three most commonly performed procedures: PRK, LASIK, and SMILE. METHODS: For the first time, patient-specific mechanical properties of the cornea were incorporated into finite element simulations. These properties were estimated using an artificial neural network trained on in silico data from fluid-structure interaction models of non-contact ton

Current applications of the Corneal Modeling System.

The ability of the Corneal Modeling System to incorporate curvature and thickness data into a single mathematical construct of the cornea is an important achievement. As experience with this and similar instruments is gained, a number of important issues in corneal topography can be addressed. Snellen visual acuity, refraction, and keratometric changes do not fully reveal the effects of refractive surgical procedures. Detailed analysis of corneal topography will prove necessary to understanding the quality of vision resulting from surgery, as it can explain the disparity between uncorrected vi

Some possible longer-term ocular changes following excimer laser refractive surgery.

While the short- and medium-term refractive and acuity results of excimer laser refractive surgery may now be generally satisfactory, the relatively brief history of the procedures involved (around 10 years or less) means that those concerned with eye care must remain vigilant to the possibility of longer-term problems. This paper reviews some relevant studies of potential post-surgical effects, including imperfect corneal healing and recovery of innervation, reduced corneal sensitivity and dry eye problems, changes in corneal rigidity leading to slow refractive change, possibly misleading red

Impact of corneal refractive surgery on the precorneal tear film.

Corneal refractive surgeries are one of the commonly performed procedures for correction of refractive errors. Tear film abnormality is the most common postoperative complication of corneal refractive surgeries. Consequently, these procedures represent a clinically significant cause of dry eye disease. The mechanisms which lead to dry eye disease include corneal sensory nerve dysfunction, ocular surface desiccation, glandular apoptosis and ocular surface inflammation. Although transient tear film abnormalities occur in almost all patients following surgery, patients with pre-existing dry eye s
