Lower extremity bilateral explosive strength asymmetry predicts non-contact low back injuries in mine rescue workers: a prospective cohort study.

OBJECTIVE: The purpose of this study was to investigate the relationship between bilateral lower extremity explosive strength asymmetry and non-contact low back injuries in mine rescue workers, a specialized occupational group. METHODS: A single-leg countermovement jump (SLCMJ) test was performed on 101 rescue members who had participated in frontline rescue missions, using a force platform. The collected force platform data were used to calculate lower limb asymmetry. Injury incidence was measured by recording all non-contact low back injuries occurring during routine training or rescue oper

Effectiveness and safety of techniques for cervical spine immobilization in mountain rescue.

BACKGROUND: Cervical spine injuries in alpine sports require immediate immobilization at the site of the accident to avoid possible secondary damage caused by transportation. Using special sensor technology, this study investigated whether a cervical spine orthosis (cervical collar, Stifneck collar (Laerdal Medical GmbH, Puchheim, Germany)) provides greater stability than a vacuum mattress alone. METHODS: Using one male test person, we simulated transporting a patient with a spinal injury in steep alpine terrain. A wireless motion capture system (Xsens Technologies, Movella™ Inc., Henderson,

Evolution of Pararescue medicine during operation Enduring Freedom.

This article highlights recent advances made in U.S. Air Force Pararescue Medical Operations in relation to tactical evacuation procedures. Most of these changes have been adopted and adapted from civilian medicine practice, and some have come from shared experiences with partner nations. Patient assessment includes a more comprehensive evaluation for hemorrhage and indications for hemorrhagic control. Ketamine has replaced morphine and fentanyl as the primary sedative used during rapid sequence intubation and procedural sedation. There has been an increasing use of the bougie to clear an airw

Hurricane Katrina response and guidance for health-care providers, relief workers, and shelter operators.

Hurricane Katrina struck the coastal areas of Alabama, Florida, Louisiana, and Mississippi on August 29, 2005, causing substantial numbers of deaths among both humans and animals, infrastructure damage, and flooding. Affected areas continue to experience shortages of essential services, including electricity, potable water, food, and fuel; damage to health-care and public health systems; and disrupted communications. CDC/ATSDR, local and state health departments, other federal agencies, and other partners are supporting public health and medical-care functions for persons in affected areas and

The Effectiveness of Debriefing on the Mental Health of Rescue Teams: A Systematic Review.

Background: Rescue teams and emergency services face high levels of mental health problems due to their frequent exposure to traumatic situations. Critical incident stress debriefing (CISD) is widely used as a psychological intervention for emergency responders and military personnel exposed to traumatic events. However, its effectiveness remains controversial, with systematic reviews yielding mixed results and some evidence of negative and harmful outcomes. This systematic review, conducted according to PRISMA guidelines, evaluates the evidence on the efficacy of CISD in mitigating psychologi

Stories behind the symptoms: a qualitative analysis of the narratives of 9/11 rescue and recovery workers.

A qualitative study of the experiences of rescue and recovery workers/volunteers at Ground Zero following the terrorist attacks of 9/11/01 is reported. Information was extracted from a semi-structured clinical evaluation of 416 responders who were the initial participants in a large scale medical and mental health screening and treatment program for 9/11 responders. Qualitative analysis revealed themes that spanned four categories- occupational roles, exposures, attitudes/experiences, and outcomes related to the experience of Ground Zero. Themes included details regarding Ground Zero roles, gr

Medical response of a physician and two nurses to the mass-casualty event resulting in the Phi Phi Islands from the tsunami.

The Phi Phi Islands are isolated islands located about one hour by ship from the mainland in Krabi province of Thailand. There is a small medical facility where the director is the one physician that provides care to residents and tourists. This small medical facility faced an enormous mass casualty incident due to the 2004 Tsunami. The hospital was damaged by the Tsunami wave and was not functional, one crew member died and another was injured. Medical care and evacuation posed a unique problem in the Phi Phi Islands due to remoteness, limited medical resources, lack of effective communicatio

The EMS response to the Oklahoma City bombing.

This is a descriptive study of the Emergency Medical Services response to a bombing of a United States Federal Building in Oklahoma City, Oklahoma on 19 April 1995. The explosion emanated from a rented truck parked in the front of the building. The force of the explosion destroyed three of the four support columns in the front of the building and resulted in a pancaking effect of the upper floors onto the lower floors. There were three distinct phases of the medical response: 1) Immediately available local EMS ambulances and staff; 2) Additional ambulances staffed by recalled, off-duty personn

Excessive Daytime Sleepiness and Associated Factors in Military Search and Rescue Personnel.

BACKGROUND: Abnormal excessive daytime sleepiness (EDS) has been reported worldwide, but too little is known about EDS and its determinants in Search and Rescue (SAR) populations. We aimed to determine the prevalence of abnormal EDS and contributing factors among Royal Norwegian Air Force (RNoAF) SAR helicopter personnel.METHODS: In this cross-sectional study, a total of N = 175 RNoAF SAR personnel completed an electronic survey including socio-demographic and lifestyle questions. The Epworth Sleepiness Scale (ESS) was used as both a continuous and categorical outcome variable to measure EDS.R

RDSP: Rapidly Deployable Wireless Ad Hoc System for Post-Disaster Management.

In post-disaster scenarios, such as after floods, earthquakes, and in war zones, the cellular communication infrastructure may be destroyed or seriously disrupted. In such emergency scenarios, it becomes very important for first aid responders to communicate with other rescue teams in order to provide feedback to both the central office and the disaster survivors. To address this issue, rapidly deployable systems are required to re-establish connectivity and assist users and first responders in the region of incident. In this work, we describe the design, implementation, and evaluation of a ra

Contributing Causes of Injury or Death in Grain Entrapment, Engulfment, and Extrication.

OBJECTIVES: Grain entrapments and engulfments are one of most common hazards associated with grain storage facilities, with over 1,140 such entrapments/engulfments documented since the 1970s. The objective of the study was to determine the factors that contribute to injury or death in grain entrapment, engulfment, and extrication cases. METHODS: A literature review, including data contained in the Purdue Agricultural Confined Spaces Incident Database (PACSID), was conducted to determine the conditions that the body experiences during an entrapment or engulfment in grains and during extricatio

Complex alpine extrication: case report of mountain and speleological rescue cooperation.

Mountain sporting activities are an increasingly popular practice that exposes mountaineers to a high risk of adverse events. This report describes a unique case of recovery in an austere environment that involved explosives. In June 2012, a 52-year-old man ascended a cliff tower in the Eastern Alps, Italy. A landslide occurred, and a boulder crushed the climber against a large stone located farther down the cliff, causing compression of the lower limbs and the pelvis with consequent severe musculoskeletal trauma. The National Alpine and Cave Rescue Unit (NACRU) arrived and proceeded with stab
