Restless legs syndrome variants: A systematic review.

INTRODUCTION: Restless Legs Syndrome (RLS) is a clinical syndrome that may manifest itself in non-leg parts of the body as well, called RLS variant, which is considered a distinct entity by some researchers. In this systematic review, we tried to evaluate various clinical features and effective treatments of RLS variants and compare them with that of typical RLS. METHODS: This study was conducted following the PRISMA guideline. The primary search was performed in the data resources of Medline (PubMed), Web of Science, and Scopus, as well as the Google Scholar search engine. The required data …

Restless legs syndrome. Pathophysiology, diagnosis and treatment.

Restless legs syndrome is the most prevalent sleep-related movement disorder, affecting 5-10% of the population, characterized by an urge to move that appears during rest or is exacerbated by rest, primarily in the lower extremities, that occurs in the evening or night and that disappears during movement or is improved by movement. Restless legs syndrome significantly impacts sleep, mood, and quality of life. Its pathophysiology involves a complex interplay of genetic and environmental factors, as well as comorbidities, leading to alterations in brain iron resulting in dysfunction in dopaminer…

The Molecular Genetics of Restless Legs Syndrome.

Restless legs syndrome (RLS) is a common sensorimotor trait defined by symptoms that interfere with sleep onset and maintenance in a clinically meaningful way. Nonvolitional myoclonus while awake and asleep is a sign of the disorder and an informative endophenotype. The genetic contributions to RLS/periodic leg movements are substantial, are among the most robust defined to date for a common disease, and account for much of the variance in disease expressivity. The disorder is polygenic, as revealed by recent genome-wide association studies. Experimental studies are revealing mechanistic detai…

Is preeclampsia associated with restless legs syndrome?

OBJECTIVE: Restless legs syndrome (RLS) is a common neurologic disorder. Secondary RLS includes pregnancy and iron deficiency. Prevalence of RLS in pregnancy ranges from 11% to 27%. We aimed to assess the frequency and characteristics of RLS in pregnancy in a Peruvian population and to evaluate the possible pregnancy or delivery complications due to RLS. METHODS: We assessed 218 consecutive expectant mothers at the inpatient clinic of the Hospital San Bartolome in Lima, Peru. Assessment was performed by using the standard diagnostic criteria for RLS and by using a clinical and diagnostic inte…

Restless legs syndrome in multiple myeloma patients.

BACKGROUND: Restless legs syndrome (RLS) is a common neurological condition characterized by uncomfortable and unpleasant sensations in the legs that are relieved by movement. It is frequently idiopathic, sometimes associated with specific disorders such as malignancies. Because there is no study relevant to RLS in Multiple Myeloma (MM), we aimed to evaluate the frequency of RLS in MM patients during chemotherapy and examined the relationship between presence of RLS and depression and anxiety in these patients. METHODS: We enrolled a population of 62 adult MM patients for RLS features. RLS wa…

Association between restless legs syndrome and migraine.

BACKGROUND: An association between restless legs syndrome and migraine has been reported recently. The clinical correlates and impact of comorbidity of restless legs syndrome (RLS) are not fully described in patients with migraine. OBJECTIVES: To investigate the frequency of RLS among different primary headache disorders, and its impact and clinical correlates in migraine patients. METHODS: Consecutive patients with migraine, tension-type headache (TTH) and cluster headache (CH) were recruited in a headache clinic. Each patient completed the Migraine Disability Assessment (MIDAS) questionnai…

Restless legs syndrome in hereditary spastic paraparesis.

This study was designed to investigate the prevalence and determinants of the association of restless legs syndrome (RLS) and hereditary spastic paraparesis (HSP). Therefore, 132 patients with HSP were evaluated concerning the symptoms of RLS by a standardized questionnaire. RLS was supposed when patients met all of the established four essential criteria of RLS defined by the International RLS Study Group. In addition, we studied the relationship between RLS and age at HSP symptom onset and evaluated the severity of RLS symptoms. Out of 59 responses, RLS was found in 27 HSP patients (15 male,…

Restless legs syndrome associated with primary hyperparathyroidism.

Restless legs syndrome (RLS) is a poorly understood sleep-related movement disorder which can be primary or associated with other conditions, most commonly iron deficiency, uremia and peripheral nerve disease. We present a case of RLS with an unusual secondary cause: primary hyperparathyroidism with hypercalcemia. This patient experienced complete and sustained relief of RLS symptoms immediately after parathyroidectomy, with normalization of her serum parathyroid hormone (PTH) and calcium levels. Early recognition and treatment of this uncommonly detected underlying cause is important because …

Restless legs syndrome in patients on dialysis.

The past decade has seen an explosion of interest in both idiopathic and secondary restless legs syndrome (RLS). Secondary RLS occurs in patients with uremia, pregnancy, and iron deficiency. Patients experience an irresistible urge to move the legs that is worse during inactivity and at night. RLS affects 6.6% to 62% of patients on long-term dialysis therapy and is associated with a greater mortality risk. The wide range of reported prevalence is explained in part by variations in methods of diagnosis. The International Restless Legs Syndrome Study Group defined diagnostic criteria that have i…

Rheumatologic serologies in secondary restless legs syndrome.

Diagnostic criteria for restless legs syndrome (RLS) have been established; however, the pathophysiology of this common condition remains elusive. Several secondary forms of RLS potentially include renal failure, iron deficiency, pregnancy, and neuropathy. RLS has also been reported in approximately 25% of patients diagnosed with rheumatoid arthritis and Sjögren's syndrome. We performed clinical and serologic evaluations on 68 patients diagnosed with RLS to determine how many may have concurrent rheumatologic disease that could be causing their RLS symptoms. We compared these with other postul…

Cryoglobulinaemic neuropathy manifesting with restless legs syndrome.

In a series of 12 patients with essential mixed cryoglobulinaemia (EMC) and peripheral neuropathy as main feature of the disease, restless legs syndrome (RLS) was a major manifestation in four women, aged 55-65 years. In one patient RLS was a presenting manifestation of the disease, and in another patient the diagnosis of EMC was made investigating RLS and polyneuropathy, although prior rheumatological symptoms were retrospectively recognized. All patients with RLS had symmetrical sensory polyneuropathy, but non-RLS patients had also other forms of peripheral neuropathy, and symmetrical sensor…

Restless legs syndrome in childhood and adolescence.

Restless legs syndrome (RLS) is believed to be a condition primarily of middle to older age. However, it can have its onset in childhood. Five illustrative case histories with an autosomal dominant mode of inheritance are described. A mother and her 3 children (age: 6 1/2, 4 and 1 1/2 years) as well as a 16-year-old patient from a second family have typical RLS signs of leg discomfort (paresthesias) and motor restlessness prevalent at night and at rest, with temporary relief by activity. Polysomnography or videotaping revealed periodic limb movements in sleep (PLMS) and, in some cases, involun…

Restless Eyes at Night: A Novel Variant of Restless Legs Syndrome.

INTRODUCTION: Restless legs syndrome (RLS) is a common sensorimotor disorder that primarily affects the lower extremities. This condition is characterized by unpleasant sensations and an irresistible urge to move the affected body regions, typically during periods of rest or at night. While RLS most commonly involves the legs, atypical variants affecting other body parts, including the arms, abdomen, face, and even the head, have increasingly been reported. This has led to the emergence of a broader diagnostic concept termed "restless X syndrome". However, no prior cases involving the ocular r…

Validation of an algorithm for the diagnosis of Restless Legs Syndrome: The Restless Legs Syndrome-Diagnostic Index (RLS-DI).

BACKGROUND: There is a need for structured methods to improve sensitivity and specificity of diagnostic decision making in Restless Legs Syndrome (RLS). We present the RLS-Diagnostic Index (RLS-DI), a diagnostic algorithm which combines essential and supportive diagnostic criteria from patient interviews, polysomnography and neurological examination in an adaptive procedure. METHOD: The RLS-DI consists of 10 items which are related to the essential diagnostic criteria established by the International RLS Study Group (five items) as well as their supportive criteria (3 items) and features asso…

Restless legs syndrome during pregnancy and puerperium.

Pregnancy is a significant risk factor for the development of restless legs syndrome (RLS). Apart from its negative impact on sleep and general quality of life, RLS is also associated with pregnancy complications and negative outcomes. To elucidate the prevalence, trajectory, severity, and risk factors of RLS throughout pregnancy and the postpartum period, we analyzed data from the multicenter Life-ON Cohort Study (NCT02664467), which recruited 439 pregnant women in the 10th-15th gestational week and followed them up until 12 months after delivery. About 26.03% of women (n = 113, 95% CI = 22.0…

Restless Legs Syndrome and Periodic Limb Movements.

OBJECTIVE: This article describes the diagnostic criteria of restless legs syndrome (RLS) and periodic limb movements. The pathophysiology of RLS and its management are discussed. The approach to address RLS augmentation is also reviewed. LATEST DEVELOPMENTS: In the 2025 American Academy of Sleep Medicine RLS treatment guidelines, long-term dopamine agonist therapy for RLS was downgraded because of the accumulating evidence of augmentation. In this context, it is important to optimally use IV iron therapy in patients with RLS. Ferric carboxymaltose is more likely to cause hypophosphatemia com…

Clinical Features of Childhood Restless Legs Syndrome.

Childhood restless legs syndrome is a sensori-motor disorder characterized by an irresistible urge to move the lower or upper extremities. It is usually accompanied by uncomfortable and unpleasant sensations in the legs which are worse at night. Symptoms worsen during periods of rest and improve with voluntary movement. The diagnosis can be facilitated by asking leading questions about symptoms, assessing their impact on sleep and daytime function, and by reviewing patient drawings of leg discomfort. Early recognition and management can improve daytime functioning and overall quality of life.

Striatal mechanism of the restless legs syndrome.

STUDY OBJECTIVES: Brain iron deficiency has been reported to be associated with the restless legs syndrome (RLS). However, 30%-50% of RLS patients do not respond to iron therapy, indicating that mechanisms other than brain iron deficiency may also participate in this disease. The striatum is known to be involved in the modulation of motor activity. We speculated that dysfunction of the striatum may induce RLS. METHODS: Two groups, wild-type (WT) and iron-deficient (ID) rats were used. Each group was divided into two subgroups, control and N-methyl-d-aspartate striatal-lesioned. After baseline…
