Solid evidence exists that fluid therapy must be started as a first-line treatment in all patients with septic shock as soon as hypotension is detected, with the goal of rapidly restoring tissue perfusion. Crystalloids or colloids can be used for initial fluid therapy, and albumin should be reserved for patients with patent or supposed hypoalbuminemia. Once fluid administration is started, its effect must be carefully monitored. In the early stages, appropriate monitoring should ensure that fluid resuscitation actually increases cardiac preload, mean arterial pressure, and tissue oxygenation. …

Treatment of severe hemorrhage offers few theoretical problems, but in practice, severe blood loss usually occurs out of hospital, often in more or less inaccessible scenarios. Controversy rages over ideal fluid, ideal volume, and minimum O2 carrying capacity, but all agree that pre-hospital, isotonic resuscitation is unfeasible. The effects of highly hypertonic 7.5% NaCl (HS) was first described in 1980, when we showed that it induced immediate and long lasting hemodynamic restoration. The addition of 6% dextran-70 to (HSD) significantly enhances the duration and intensity of volume expansion…

Although pharmacologic intervention has traditionally been a mainstay in the treatment of circulatory collapse and cardiac arrest, a critical reappraisal of the relevant literature does not support this stance in every case. During cardiac arrest, medications take second place to other forms of intervention: initiation of basic life support with ventilation [100% oxygen!], chest compression and defibrillation if indicated. Epinephrine is still the drug of choice in all types of cardiac arrest. Atropine is recommended for asystole and pulseless electrical activity [PEA] with bradycardia, while

Calcium ions are essential for myocardial excitation-contraction coupling and for increasing contractility of the heart. Because of these physiological effects, use of calcium has been recommended in the treatment of electromechanical dissociation, asystole and ventricular fibrillation. Recently, however, the use of calcium during cardiac arrest has come into dispute, as there are no controlled clinical trials supporting a favourable haemodynamic effect. It has rather been suggested that, in resuscitation, calcium may have detrimental effects. In order to evaluate the role of calcium in cardio…

Birth asphyxia is frequent and often severe, occurring in about 10% and 1% respectively of all births; in a third it is unexpected. Delivery rooms must be organised and equipped and trained staff readily available so as to provide appropriate and timely resuscitation of the newborn. Simple procedures designed to prevent hypothermia, maintain a patent airway, improve oxygenation and ventilation are sufficient for the majority of babies. Circulatory support and biochemical resuscitation will be needed in a few. In the absence of other abnormalities, the long term prognosis for newborns who respo…

The resuscitation of a 21-year-old man after submersion in fresh water for at least 5 min is described. On admission to hospital the patient appeared dead with fixed dilated pupils, deep cyanosis and asystole. Twenty min later the patient was transferred to the intensive care unit with a recordable blood pressure. On arrival there he was breathing spontaneously and so was not ventilated. A few hours later he developed pulmonary oedema and died.

Resuscitation should always be attempted in a patient who has at least a theoretical chance of survival. This assumes that there are patent cerebral, coronary and pulmonary vessels, a reasonable time from cardiac arrest has not been exceeded, and cardiac arrest did not occur as a result of a terminal condition caused by an untreatable disease. During resuscitation, medical personnel may face two dilemmas: when to start CPR, and when (and how) to stop it. Apart from various medical conditions, possible outcome and will of a victim has to be taken into consideration. CPR is frequently started wi…

BACKGROUND: Extracorporeal cardiopulmonary resuscitation (ECPR) is increasingly used in adults with refractory cardiac arrest. However, post-resuscitation care in this population remains heterogeneous, evidence-based recommendations are lacking, and most evidence is derived from conventional cardiopulmonary resuscitation. We conducted a scoping review to describe the existing evidence on post-resuscitation care after ECPR.
METHODS: We systematically searched PubMed and Embase from inception to May 19, 2025, for studies reporting on post-resuscitation care in adults treated with ECPR following…
