A 25-year-old woman with dermatomyositis suffered a right central retinal vein occlusion (CRVO) with visual acuity of 20/40. Examination of the right eye showed vitreous cells, suggesting inflammation of the central retinal vein leading to a CRVO as the presumed mechanism. She was admitted to hospital, and extensive evaluation was negative. She was maintained on corticosteroids to manage her dermatomyositis. One month later, she had macular edema and elevated intraocular pressure. Both resolved with dorzolamide, timolol, and intravitreal bevacizumab, and vision returned to 20/20 in the right e…

PURPOSE: To investigate and identify hereditary spherocytosis as a possible risk factor for the development of a central retinal vein occlusion in a young, otherwise healthy, adult.
METHOD: This study is a retrospective case description of a single female patient.
RESULTS: A 31-year-old white woman with hereditary spherocytosis after splenectomy was found to have a central retinal vein occlusion. She had no atherosclerotic risk factors and a negative hypercoaguable workup. Her course was complicated by macular edema. She was treated with intravitreal ranibizumab injections with resultant imp…

PURPOSE: To study the pathomorphology of subretinal hemorrhage (SRH) seen in eyes with branch retinal vein occlusion (BRVO) and its association with visual prognosis.
METHODS: We retrospectively reviewed 42 consecutive patients (42 eyes) with BRVO that affected the fovea. Retinal structural changes were examined by spectral domain optical coherence tomography (SD-OCT).
RESULTS: On SD-OCT sections, serous retinal detachment was seen at the fovea in 35 eyes, 18 of which accompanied foveal SRH. While initial detection of foveal SRH had no correlation with initial visual acuity (VA), it was corr…

Risk factors for the occurrence of central retinal vein occlusion are to a certain extent similar to those of cardiovascular diseases (e.g. arteriosclerosis, arterial systemic hypertension, diabetes mellitus, dislipidemia). Hyperhomocysteinemia is an essential risk factor for arteriosclerosis intervening also directly in the local mechanism of causing venous and arterial occlusions. Ocular hypertension and glaucoma are risk factors significantly associated with pathogenesis of central retinal vein occlusion. Therapy with anticoagulants and platelet anti-aggregating agents exposes the patient t…

Neovascularization in branch retinal vein occlusion usually develops 6-12 months after the onset of the disease, although some cases have been reported in which neovascularization and subsequent vitreous haemorrhage developed 2-3 years later. This is a report of nine cases of late appearance of vitreous haemorrhage due to branch retinal vein occlusion, which occurred 3-6 years after the initial onset of the disease. In two of these nine cases the vitreous haemorrhage was very profound and had to be managed by vitrectomy. We have studied the remaining seven cases, which had retinal ischaemia an…

An 8-year retrospective study of patients suffering a central retinal vein occlusion (CRVO) was undertaken to study secondary new vessel formation and whether pathologies known to predispose to CRVO influenced the occurrence of these neovascular complications and their responsiveness to treatment. Seventy-three patients were studied. Ocular neovascularisation had occurred in 60%. More specifically 39% developed neovascular glaucoma. Panretinal photocoagulation produced regression in only 37% of those with established neovascularisation and was unsuccessful in preventing neovascularisation in f…

I have described the vascular changes after retinal branch vein occlusion and have shown progressive changes both in the large retinal vessels and in the capillary bed. The capillary reactions are comparable to those seen in central retinal vein occlusion. Retinal oedema due to capillary dilatation and leakage occurs in both conditions but, whereas this is generalized in central retinal vein occlusion, local changes occur in branch vein occlusion. Capillary closure also occurs in both conditions, but the neovascular response differs. In branch vein occlusion significant new vessel formation is…

BACKGROUND: Retinal vein occlusion, subdivided into central retinal and branch retinal vein occlusion, is one of the most frequent vascular diseases of the retina. Biomarkers of optical coherence tomography (OCT), OCT-angiography and (ultra-widefield) fluorescein angiography are of exceptional importance in the initial diagnosis and also in the treatment of complications associated with retinal vascular occlusion, particularly macular edema.
METHODS: A systematic literature review was carried out in PubMed with the keywords central retinal vein occlusion, branch retinal vein occlusion, biomar…

PURPOSE: To report a case of branch retinal artery occlusion (BRAO) followed by branch retinal vein occlusion (BRVO) and paracentral acute middle maculopathy (PAMM) in a patient with confirmed calciphylaxis.
OBSERVATIONS: A 52-year-old female with a history of BRAO in the right eye one-year prior presented with decreased vision and a new inferotemporal scotoma. Computed tomography angiography of the head and neck demonstrated vascular calcifications at the origin of both ophthalmic arteries, which were otherwise poorly visualized. Ophthalmic examination demonstrated retinal whitening superior…

Paroxysmal nocturnal hemoglobinuria (PNH) is a rare disorder associated with increased risk for thrombosis and reduced life expectancy. Retinal vein occlusion (RVO) is a frequent cause of vision loss but its relationship with PNH has not been studied systematically. Patients followed up for RVO in our ophthalmology department were screened for the presence of a PNH clone in peripheral blood by means of flow cytometry. The presence of other well-documented risk factors for RVO was also analyzed. In a series of 110 patients (54 males, median age of 67) we found no evidence of PNH. Most patients …

Central retinal vein occlusion (CRVO) remains one of the most common retinal vascular disorders that may lead to blindness. The etiology is unknown, however, predisposing factors such as hypertension, diabetes, atherosclerosis and hypercoagulable states have all been described. Local ophthalmic illnesses such as open angle glaucoma, ocular trauma and orbital infections have also been suggested as causative. CRVO can be subdivided into two clinical types, ischemic and non-ischemic. The non-ischemic type comprises the milder form of the disease with partial venous obstruction and good visual out…
