PURPOSE OF REVIEW: The present review discusses the current knowledge about exercise capacity and physical activity in children with rheumatic disease and examines the role of exercise in managing these conditions.
RECENT FINDINGS: Recent studies suggest exercise capacity is significantly impaired in a large proportion of children with juvenile idiopathic arthritis and other rheumatic diseases. These deficits are not limited to children with active inflammation. Children, especially girls, with rheumatoid factor positive polyarticular juvenile idiopathic arthritis, have the greatest deficits.…

Patients with autoimmune rheumatic diseases are at increased risk of developing infections. However, concerns about the safety and the immunogenicity of vaccines in these patients limited their use. Most of the data against the use of vaccines come from the reported cases of previously healthy individuals who presented the onset of rheumatic diseases after immunization, nevertheless a causal relationship has not been established. During the past few decades influenza and pneumococcal vaccines, administered to patients with systemic lupus erythematosus, were found to be safe and, generally, ser…

Inflammatory rheumatic diseases other than systemic vasculitides and systemic lupus erythematosus are frequently associated with renal abnormalities, which are clinically less apparent due to the subtle course and the often only moderate impairment of renal function. These abnormalities include vascular, glomerular and tubulointerstitial changes. Renal manifestations in the course of rheumatoid arthritis influence the prognosis of the disease. Renal involvement due to AA amyloidosis following long-standing inflammatory joint disease can lead slowly, over years, to end-stage renal disease. A sc…

Nailfold capillaroscopy (NVC) is a simple, non-invasive, inexpensive and useful method for the analysis of microvascular abnormalities found … named as "scleroderma-like pattern". Capillaroscopy changes have also been found in other systemic rheumatic diseases such as Systemic Lupus Erythematosus, Antiphospholipid Syndrome and Sjögren's Syndrome, further epidemiological and clinical studies are needed to better characterize and standardize nailfold capillaroscopy patterns in these disorders.

The majority of the imaging techniques in cardiology could be applied in rheumatic diseases (RDs), such as echocardiography, single-photon emission computed tomography (SPECT), radionuclide ventriculography, angiography, cardiovascular MRI and CT. Inflammatory pericardial involvement is the most common cardiac manifestation in various forms of RD. Echocardiography is the gold standard for diagnosis of pericardial abnormalities, demonstrating location and amount of pericardial effusion. Cardiac MRI and CT can be used to assess the features of pericardial effusions and pericardial structures. In…

Necrotizing fasciitis is an uncommon but life-threatening complication in immunocompromised hosts. We reported four patients with rheumatic diseases complicated by necrotizing fasciitis and reviewed 14 others from literature search. Most patients were on corticosteroid treatment. Septic shock, disseminated intravascular coagulopathy and acute renal deterioration were common giving rise to an overall mortality rate of 27.8%. Septic arthritis may also complicate the condition. Statistical analysis on the series showed the lack of major surgical debridement as the only risk factor associated with…

Extract: Seasonal variation has been shown in a number of rheumatic diseases (diseases involving the joints and related structures). The incidence of acute gouty attacks (an inflammatory arthritis) is highest in the spring. The onset or exacerbation of rheumatoid arthritis, the onset of Wegener's granulomatosis (chronic tissue inflammation and cellular clumping in the nasal passages, lungs and kidneys), anti-neutrophil cytoplasmic antibodies (ANCA) associated kidney inflammation (glomerulonephritis) and systemic vasculitis are all seen more commonly in the winter. There is a significant increa…

OBJECTIVES: Detailed review of the manifestations of eye involvement in the context of rheumatic diseases.
METHODS: An OVID Medline search of the rheumatology and ophthalmology English literature related to the eye manifestations of human rheumatic diseases from 1966 to the present was conducted by the authors.
RESULTS: Analysis of 300 recent and consecutive rheumatology consultations from a large Veterans Administration Healthcare System shows that 4% are referred for eye manifestations of suspected rheumatic diseases, most commonly, anterior uveitis and keratoconjunctivitis sicca (KCS). Oc…

Ocular manifestations of rheumatic disease are common and varied. The presentation of uveitis in psoriatic arthritis and inflammatory bowel disease differ from typical HLA-B27-associated uveitis. Giant cell arteritis continues to be a common cause of visual loss. Early diagnosis is crucial to limiting visual loss. Once visual loss has occurred, the need for oral versus intravenous steroids is controversial. Chronic uveitis associated with juvenile rheumatoid arthritis may be less prevalent than previously shown. However, poor visual outcomes continue to occur and efforts to identify visual pro

The medical texts published in New Spain between the XVI and XVIII century have not been searched until now for evidence of the existence of rheumatoid arthritis and other rheumatic diseases before the descriptions made by Sydenham. We surveyed most of the medical books written and published in New Spain from the arrival of the Spaniards to the XVIII century, and we divided the diseases with articular manifestations into four groups, according to their main clinical characteristics: pain without swelling in anatomical region; pain without swelling in several joints; pain and swelling in joints…

OBJECTIVE: To review mortality data in published studies of various rheumatic diseases.
METHODS: A MEDLINE search of the literature on the rheumatic diseases, including osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, systemic lupus erythematosus, scleroderma, polymyositis, and vasculitis.
RESULTS: Mortality rates higher than expected have been reported in most rheumatic conditions, considerably higher for inflammatory rheumatic diseases. The mortality rates in patients with systemic lupus erythematosus, scleroderma, polymyositis, and vasculitis are often comparable to mortality…

Contrary to previous belief, there is increasing evidence that a broad spectrum of rheumatic diseases do affect African blacks. Although properly conducted epidemiological studies have yet to be performed, reports of population surveys from a variety of sub-Saharan African countries indicate that diseases such as rheumatoid arthritis (RA), gout, and the connective tissue diseases are observed, although some differences in clinical presentation may occur as a result of cultural, racial, and socioeconomic factors. Rheumatoid arthritis is common in some parts of Africa and less common in others. …

There have been few epidemiological studies of bone and joint diseases in black Africa. Available data were generated by hospital studies which were inevitably flawed by selection bias. They found that the incidence and/or severity of rheumatoid arthritis were reduced in West Africa but not in urban areas of Southern and East Africa, as compared with industrialized countries. Ankylosing spondylitis was infrequent. The human immunodeficiency virus epidemic can be expected to increase the prevalence of spondyloarthropathies despite the fact that few black Africans are HLA B27-positive. Gout was

The ocular complications of the rheumatic diseases are common, frequently insidious and sight threatening, and deserve review to heighten awareness and facilitate earlier diagnosis and treatment.

Laboratory investigations have been perhaps the most important factor in the development of our understanding and classification of rheumatic diseases. Provided they are used and interpreted appropriately, they can assist considerably in the diagnosis and management of these diseases. They are an important adjunct to clinical findings, but not a substitute for them.
