We report the first case of sino-orbital mucormycosis caused by Rhizopus rhizopodiformis. Therapy consisted of conservative surgical drainage and intravenous amphotericin B. The patient, an insulin-dependent diabetic, survived her infection with minimal residual deformity. This case and a literature review suggest that infection due to R. rhizopodiformis in diabetics might be more amenable to treatment than mucormycosis caused by other species of fungi. Speciation of such fungi would permit the determination of species specific mortality rates, and may be of therapeutic and prognostic value.

Mucormycosis is a fungal infection primarily afflicting immunocompromised or diabetic patients. Its presentation ranges from rhino-orbito-cerebral infections to disseminated mucormycosis with angioinvasion. We present a patient who developed a bowel obstruction one month after bone marrow transplant and was diagnosed with Rhizopus arrhizus diverticulitis despite antifungal therapy since transplantation. Mucormycosis was produced in rabbits with sustained, severe leukopenia and granulocytopenia induced by repeated injections of nitrogen mustard. Initially, these animals developed extensive fungus lesions at the site of inoculation which later became granulomatous and tended to heal. While patients undergoing treatment for hematologic malignancies are at risk for a variety of infections, gastrointestinal mucormycosis is a rare and feared complication. Diagnosis requires a high index of suspicion and timely evaluation. Prompt treatment improves patient outcomes.

Posaconazole is a triazole antifungal with activity against Rhizopus, but data on its use and pharmacokinetics in preterm infants are scarce. In this case, a 24 4/7-week neonate's Rhizopus infection is successfully treated with debridement and combination antifungal therapy with amphotericin B, micafungin and enteral posaconazole. This is the first reported posaconazole use in a preterm neonate with Rhizopus.

Mucormycosis is an invasive fungal infection caused by filamentous fungi of the Mucoraceae family. The genera most commonly responsible are Mucor or Rhizopus. The accepted standard for treatment of zygomycetes is amphotericin B and surgical debridement, however recent data suggest that combined treatment modalities may be optimal. Newer anti-fungal agents show success in vitro and in animal models. We present the case of a 10-year-old boy with invasive Rhizopus microsporus var. rhizopodoformis who was successfully treated with a combination of modalities.

A rapidly enlarging leg ulcer appeared in a 54-year-old woman with systemic lupus erythematosus receiving aggressive immunosuppressive therapy. Skin biopsy revealed proliferation of hyphae in the midst of a neutrophilic abscess. Culture yielded Rhizopus azygosporus. As no organ involvement was detected by thorough examination, the patient was diagnosed as having primary cutaneous mucormycosis.
