Twenty-four insulin-dependent diabetic children and 114 normal control children, all between the ages of 6 to 16 years, were investigated for riboflavin deficiency. The method used was a measurement of erythrocyte glutathione reductase activity and the results were expressed as the activity coefficient (AC). None of the children had received vitamin supplementation. The percentage of diabetic children with riboflavin deficiency was 4 fold greater than in non diabetics. Supplementation with daily oral riboflavin quickly returned all AC values to normal.

Riboflavin, in its cofactor forms flavin adenine dinucleotide (FAD) and flavin mononucleotide (FMN), plays fundamental roles in energy metabolism, cellular antioxidant potential, and metabolic interactions with other micronutrients, including iron, vitamin B6, and folate. Severe riboflavin deficiency, largely confined to low-income countries, clinically manifests as cheilosis, angular stomatitis, glossitis, seborrheic dermatitis, and severe anemia with erythroid hypoplasia. The finding of tomacula, focal areas of sausage-shaped hypermyelination in peripheral nerves, is reported for the first time in avian riboflavin deficiency. Day-old, meat-type chickens were fed a riboflavin-deficient diet (1.8 mg/kg) and were killed on postnatal days 6, 11, 16, and 21, while control chickens were fed a conventional diet containing 5.0 mg/kg riboflavin. Tomacula were found in sciatic and brachial nerves from day 11 onward, became more frequent and prominent with increasing time, and preceded the onset of segmental demyelination.

We have evaluated the effects of treatment with riboflavin in five patients with a mitochondrial myopathy, associated with a complex I (NADH dehydrogenase) deficiency. Two patients suffered from a clinically pure myopathy and the other patients presented with encephalomyopathic features. Treatment with riboflavin resulted in a clear clinical improvement in the two patients with the myopathic form of complex I deficiency. Preliminary data on the functional riboflavin deficiency in patients with malignant skin melanoma are discussed. In 50% of 36 patients with skin melanoma the glutathione reductase activation coefficient was higher than 1.21 and in 28% of the patients--above 1.31. Oral administration of high doses of riboflavin normalized the riboflavin status.
