[Pyogenic sacroiliitis in children].

Pyogenic sacroiliitis is a rarely occurring disease. In recent years more reports have been published about this topic. We report on a case of infectious sacroiliitis in a young female patient. The multivarious clinical symptoms, the problems of diagnosis, differential diagnosis, treatment and prognosis of this disease will be described.

[Imaging of infectious sacroiliitis].

About 11 cases of various infectious sacroiliitis, the authors discuss the clinical, biological and radiological findings. Acute and subacute infectious sacroiliitis can be associated with another arthritis and are often accompanied by soft tissues lesions. Radiological investigations are very useful for the diagnosis, because of the unspecific clinical and biological findings. Radiography shows advanced osteoarticular remodelling but the subtle lesions are often unapparent. Bone scintigraphy shows a focal hypercaptation but is unspecific. Tomography and CT point out the erosions. With CT, sof…

Sacroiliitis and acne conglobata.

A 24-year old man developed a bilateral sacroiliitis in association with acne conglobata. The acne conglobata was followed after the use of prednisone during 9 months. The prednisone was taken for a severe form of sarcoidosis. This may be a new instance of a relatively uncommon and poorly understood relationship between a particular form of acne and associated arthritis.

Tomography for evaluation of sacroiliitis.

The radiographs and charts of 18 patients were reviewed to determine if sacroiliac joint tomography clarified the equivocal or clinically inconsistent findings of other imaging procedures. In six of eight patients with proven sacroiliitis, tomography detected joint erosions or sclerosis that were not detected on routine pelvis radiographs. In the other 10 patients, tomography excluded sacroiliitis which had been initially suggested by plain radiographs (four patients), quantitative sacroiliac scintigraphy (three patients), or clinical findings (three patients). The estimated average skin radia…

HL-A antigens and sacroiliitis.

Lymphocyte typing for HL-A B27 is useful under conditions in which data based on history and examination are suggestive but not diagnostic of a seronegative spondyloarthropathy. The presence of HL-A B27 would increase the probability of a patient's having one of these entities, but its absence does not rule out such a diagnosis. Furthermore, tissue-typing allows one to predict the probability that a patient with inflammatory bowel disease will develop AS and the likelihood that the family member of a patient with AS will develop a related disease.

HL-A antigens and sacroiliitis.

The histocompatibility antigen W27 has been found to have a high incidence in ankylosing spondylitis (96%), Reiter syndrome (74%), and other forms of arthritis that may involve the axial skeleton. In patients with Reiter syndrome, there was no correlation between the presence of W27 antigen and the extent or duration of the symptoms. Sacroiliitis was predominantly found in those patients with W27 antigen in all the axial arthropathies. Tissue-typing techniques may be of value in detecting early or atypical disease.

Tuberculous sacroiliitis. Four cases.

Tuberculous sacroiliitis with no local abscesses or tuberculosis at other sites occurred in four patients, three men and one woman with a mean age of 42 years. Slow progression characterized this uncommon variant of joint tuberculosis. Two patients reported contact with family members known to have tuberculosis. Mean time from symptom onset to presentation was 14 months. Surgical biopsy showed epithelioid and giant-cell granulomas with caseous necrosis; culturing on Lowenstein-Jensen medium recovered the tubercle bacillus. Treatment was with antitubercular drugs, and protection from weight bea…

[Septic sacroiliitis: case report].

Bacterial pyogenic sacroiliitis is a relatively rare infection that may be difficult to diagnose. A delay in the diagnosis may be associated with marked toxemia and demanded surgical drainage of the septic joint. We report clinical and laboratorial findings in a 12-year-old white girl with onset low back and leg pain two weeks before admission to the hospital. Bone scintigraphy showed increased uptake in the right sacroiliac joint and radiologic changes were typical of sacroiliitis. Oxacilin-sensitive S. aureus was cultured from fluid of surgical biopsy. Antibiotic treatment leads to complete …
