Spondyloarthritis and Sarcopenia: Prevalence of Probable Sarcopenia and its Impact on Disease Burden: The Saspar Study.

To evaluate the prevalence of probable, confirmed, and severe sarcopenia in spondyloarthritis (SpA), according to the European Working Group on Sarcopenia in Older People 2019 (EWGSOP2) definition. A total of 103 patients (51% women) with SpA, mean age 47.1 ± 13.7 years, were included and compared to 103 age- and sex-matched controls. Grip strength was measured by dynamometry. Body composition was assessed by whole-body densitometry. In SpA patients gait speed was measured by the 4-m-distance walk test and quality of life was evaluated with a specific health-related questionnaire for sarcopeni…

[Correlation between sarcopenia and osteoporosis].

Sarcopenia and osteoporosis are two common metabolic diseases among the elderly population. Studies have found that there was a mutually reinforcing correlation in the process of their onset. Epidemiological research indicated that the incidence rates of sarcopenia and osteoporosis were positively correlated. Genetic studies have discovered genes that could simultaneously control low bone density and low muscle mass. Cell biology research has found that there were cytokines that act on both muscle cells and bone cells, and changes in their content could directly cause a decrease in muscle mass…

Sarcopenia in interstitial lung disease.

BACKGROUND: Interstitial lung disease (ILD) encompasses a heterogeneous group of chronic lung conditions with considerable variability in prognosis and response to treatment. People with reduced muscle mass and function, known as sarcopenia, have a higher risk of mortality and adverse clinical outcomes both in the general population and in other chronic disease states. The importance of sarcopenia across the spectrum of patients with ILD is not well established. OBJECTIVES: In this narrative review, we explore the prevalence and clinical implications of sarcopenia in patients with ILD, evalua…

[Sarcopenia in Chronic Liver Disease].

Sarcopenia is a crucial factor in assessing the nutritional status of chronic liver disease patients and predicting their prognosis and survival. The serum ammonia level is closely associated with sarcopenia regarding ammonia, a key regulator in the liver-muscle axis. In addition, various changes in energy metabolism and hormones are also involved in sarcopenia. The psoas muscle area can represent the overall skeletal muscle mass in liver disease patients. Therefore, measuring the psoas muscle area with computed tomography or magnetic resonance imaging is considered an objective and reliable m…

Lipotoxicity-related sarcopenia: a review.

A body of literature supports the postulation that a persistent lipid metabolic imbalance causes lipotoxicity, "an abnormal fat storage in the peripheral organs". Hence, lipotoxicity could somewhat explain the process of sarcopenia, an aging-related, gradual, and involuntary decline in skeletal muscle strength and mass associated with several health complications. This review focuses on the recent mechanisms underlying lipotoxicity-related sarcopenia. A vicious cycle occurs between sarcopenia and ectopic fat storage via a complex interplay of mitochondrial dysfunction, pro-inflammatory cytokin…

[Sarcopenia - 2021: Pathophysiology, diagnosis, therapy].

Sarcopenia is a progressive, generalized skeletal muscle disease with the loss of muscle mass and function, associated with adverse outcomes and poor prognosis. Sarcopenia first was regarded as an age-related disorder of older people (primary sarcopenia). Later it turned out that it can also occur in young age due to a range of chronic disorders such as cancer, anorexia or malnutrition (secondary sarcopenia). This paper overviews the pathophysiology of sarcopenia and the factors involved in the muscle mass loss, i.e., physical inactivity, insulin resistance, low-grade chronic inflammation, hep…

Muscoloskeletal aging, sarcopenia and cancer.

In the context of the management of the elderly patient with cancer, currently, a key topic may be expressed by sarcopenia. Sarcopenia in its various definitions and ambiguities in the older adult is always related to negative outcomes. A close correlation between sarcopenia and adverse outcomes such as treatment response or toxicity has also been shown in the patient with cancer. For these reasons, it becomes increasingly important to try to understand and therefore differentiate conditions such as the loss of muscle mass linked to normal aging, an independent pathological condition such as s…

Comprehensive approach to sarcopenia treatment.

Sarcopenia is characterized by progressive and generalized loss of skeletal muscle mass and strength with a risk of adverse outcomes such as physical disability, poor quality of life, and death. Primary sarcopenia is considered to be age-related when no other cause is evident, other than ageing itself. Secondary sarcopenia should be considered when one or more other causes are evident, such as activity-, disease-, or nutrition-related sarcopenia. In this narrative review that focused on human studies, we summarize the pharmaceutical therapies (testosterone, dehydroepiandrosterone, estrogen, gr…

Finding good biomarkers for sarcopenia.

The term sarcopenia describes the age-related loss of skeletal muscle mass and function. While this process, in principal, occurs in every adult person and already starts around the age of 40, it is associated with disability, morbidity, and increased mortality in some individuals. In the absence of clear clinical manifestation, we today lack the ability to differentiate between physiological and pathological sarcopenia. In this regard, we need good biomarkers that can be quantified in a reliable, cost-effective manner and that guide diagnosis and therapy of pathological sarcopenia in routine …

Development and validation of the sarcopenia composite index: A comprehensive approach for assessing sarcopenia in the ageing population.

INTRODUCTION: The diagnosis of sarcopenia relies on key indicators such as handgrip strength, walking speed and muscle mass. Developing a composite index that integrates these measures could enhance clinical evaluation in older adults. This study aimed to standardise and combine these metrics to establish a z score for the sarcopenia composite index (ZoSCI) tailored for the ageing population. Additionally, we explore the risk factors associated with ZoSCI to provide insights into early prevention and intervention strategies. METHOD: This retrospective study analysed data between January 2017 …

The Conceptual Definition of Sarcopenia: Delphi Consensus from the Global Leadership Initiative in Sarcopenia (GLIS).

IMPORTANCE: Sarcopenia, the age-related loss of muscle mass and strength/function, is an important clinical condition. However, no international consensus on the definition exists. OBJECTIVE: The Global Leadership Initiative in Sarcopenia (GLIS) aimed to address this by establishing the global conceptual definition of sarcopenia. DESIGN: The GLIS steering committee was formed in 2019-21 with representatives from all relevant scientific societies worldwide. During this time, the steering committee developed a set of statements on the topic and invited members from these societies to participa…

Machine Learning Applications and Sarcopenia.

The objective of this review was to investigate the variety of machine learning (ML) methods and their ability to detect individuals at risk of or suffering from sarcopenia. Sarcopenia is a progressive disease characterized by a significant decline in muscle mass, muscle strength, and physical performance, primarily affecting older adults. PubMed databases were searched from July to August 2024 for trials related to the sarcopenia prognosis and artificial intelligence. The keywords used were "sarcopenia," "artificial intelligence," "machine learning," and "risk factors." Eleven studies were in…

Hormonal dimorphism in sarcopenia disease.

Sarcopenia, affecting over 60% of individuals above age 80, represents a critical challenge for aging populations worldwide. Despite formal recognition as a disease by the WHO in 2016, therapeutic approaches remain limited to exercise and nutritional interventions, with no approved pharmacological treatments. Current management strategies follow a universal paradigm that assumes similar pathophysiological mechanisms across all patients, yet clinical outcomes demonstrate marked variability that may reflect fundamental sex-specific differences in muscle-aging biology. This review interrogates se…

Sarcopenia and aortic valve disease.

Valvular heart disease, including calcific or degenerative aortic stenosis (AS), is increasingly prevalent among the older adult population. Over the last few decades, treatment of severe AS has been revolutionised following the development of transcatheter aortic valve replacement (TAVR). Despite improvements in outcomes, older adults with competing comorbidities and geriatric syndromes have suboptimal quality of life outcomes, highlighting the cumulative vulnerability that persists despite valve replacement. Sarcopenia, characterised by loss of muscle strength, mass and function, affects 21%…

Acute Sarcopenia: Mechanisms and Management.

BACKGROUND: Acute sarcopenia refers to the swift decline in muscle function and mass following acute events such as illness, surgery, trauma, or burns that presents significant challenges in hospitalized older adults. METHODS: narrative review to describe the mechanisms and management of acute sarcopenia. RESULTS: The prevalence of acute sarcopenia ranges from 28% to 69%, likely underdiagnosed due to the absence of muscle mass and function assessments in most clinical settings. Systemic inflammation, immune-endocrine dysregulation, and anabolic resistance are identified as key pathophysiolog…

[Sarcopenia in chronic liver diseases].

BACKGROUND: Sarcopenia is a progressive and generalized disorder of the skeletal musculature that involves the loss of skeletal muscle mass and function. Patients with chronic liver disease frequently have sarcopenia in advanced stages of the disease; however, there is an increased prevalence of sarcopenia not only in liver cirrhosis but also in earlier stages of disease, e.g., in non-alcoholic fatty liver disease (NAFLD). RESULTS: Sarcopenia is an independent prognostic risk factor for morbidity and mortality in patients with liver cirrhosis. The pathogenesis of sarcopenia is multifactorial …

[The Actual Sarcopenia Definition 2019].

Sarcopenia is characterized by a generalized loss of muscle function, strength and mass and is codable in Germany since 2018 in the ICD-10-GM (M62.50). For screening in primary care, it is possible to determine muscle function and strength by means of a sarcopenia questionnaire (SARC-F) as a self-filler with 5 questions of restrictions. With an increased score of 4 and higher, an examination of the musculature and a determination of the skeletal muscle mass index should be performed via dual energy X-ray absorption measurement (DXA) or bioelectric impedance analysis (BIA).If hand and/or leg st…

Sarcopenia - Endocrinological and Neurological Aspects.

Sarcopenia in geriatric patients is often associated with or even caused by changes of the endocrine and nervous system. The multifactorial pathogenesis of sarcopenia and additional multimorbidity in geriatric patients makes it difficult to study distinct pathogenic pathways leading to sarcopenia. Patients suffering from diabetes, Cushing's syndrome, chronic kidney disease, Klinefelter's syndrome or motor neuron diseases, such as amyotrophic lateral sclerosis for example are known to have impaired muscle property and reduced physical performance. These patients are typically younger and suffer…
