Coadministered Cagrilintide and Semaglutide in Adults with Overweight or Obesity.

BACKGROUND: Semaglutide at a dose of 2.4 mg has established weight-loss and cardiovascular benefits, and cagrilintide at a dose of 2.4 mg has shown promising results in early-phase trials; the efficacy of the combination (known as CagriSema) on weight loss in persons with either overweight and coexisting conditions or obesity is unknown. METHODS: In a phase 3a, 68-week, multicenter, double-blind, placebo-controlled and active-controlled trial, we enrolled adults without diabetes who had a body-mass index (BMI; the weight in kilograms divided by the square of the height in meters) of 30 or hig…

The Impact of Semaglutide on Metabolic Syndrome: A Case Report.

This case describes a 44-year-old male with poorly controlled type 2 diabetes mellitus, dyslipidemia, and hypertension - hallmarks of metabolic syndrome - who demonstrated significant clinical improvement following the initiation of semaglutide, a glucagon-like peptide-1 receptor agonist (GLP-1 RA), after discontinuing sitagliptin. The patient had previously been treated with multiple oral hypoglycemic agents and basal insulin, yet continued to have inadequate glycemic and lipid control. Upon replacing sitagliptin with semaglutide, he experienced marked reductions in glycated hemoglobin, low-d…

The Therapeutic Effects of Semaglutide in Congenital Linear Scleroderma.

Congenital linear scleroderma (CLS) is a rare connective tissue disorder characterized by unilateral linear induration that can lead to restricted mobility and limb deformities. Various treatments have been proposed, including D-penicillamine, phototherapy, oral vitamin D, and immunosuppressants such as methotrexate (MTX), cyclosporine, and interleukin-6 (IL-6) inhibitors. We present the case of a 14-year-old female patient with refractory CLS, whose condition continued to worsen despite treatment with tocilizumab, mycophenolate mofetil (MMF), and MTX. She was subsequently started on a glucago

Semaglutide-associated hyposalivation: A report of case series.

RATIONALE: Obesity and diabetes of different types are considered global health risks with rising prevalence. In addition to low-calorie diet and daily exercise, several treatment options have been introduced to help patient in needs. Semaglutide (Ozempic) is one popular agent, which attracted the attention of both physicians and patients due to its positive outcome in improving glucose control and weight loss. However, no reports on the effect of semaglutide use on the oral cavity and specifically xerostomia are available in the literature. We are reporting 3 cases for patients who were using…

Rhabdomyolysis Associated With Semaglutide Therapy: A Case Report.

This report describes the case of a 47-year-old woman with myalgias, weakness, and elevated creatine kinase associated with semaglutide therapy prescribed for weight loss. Her symptoms and laboratory markers were consistent with rhabdomyolysis and resolved after discontinuation of semaglutide. Upon rechallenge at a lower dose, symptoms recurred, and urinalysis was consistent with myoglobinuria. Symptoms again rapidly resolved upon discontinuation of the medication. It is imperative for physicians to recognize semaglutide as a possible cause of myalgias and rhabdomyolysis in clinically suspecte…

Semaglutide and cancer: A systematic review and meta-analysis.

BACKGROUND: French national health care insurance system database has suggested 1-3 years use of glucagon like peptide-1 receptor agonists (GLP1RA) (exenatide, liraglutide and dulaglutide) may be linked with increased occurrence of thyroid cancer. Similar data on semaglutide is not-available. Hence, we undertook this systematic review to look at the safety of semaglutide focussing on different cancers. METHODS: Databases were searched for randomized controlled trials (RCTs) and real-world studies involving patients receiving semaglutide in the intervention-arm. Primary outcome was to evaluate…

Semaglutide-induced transient intestinal ischemia: A case report.

Obesity is a global health issue that impacts millions of people worldwide. Semaglutide is a glucagon-like peptide-1 receptor agonist that was initially developed as a treatment for type 2 diabetes mellitus under the brand name Ozempic. Additional studies showed semaglutide at larger doses (marketed as Wegovy) to be an effective medication for weight management. Wegovy for managing obesity is typically well tolerated; however, there have been reports of gastrointestinal adverse effects. Only 1 case of Wegovy-induced transient intestinal ischemia has been documented, making it extremely unusual…

Semaglutide Bioavailability: Limitations, Formulation Innovation and Future Opportunities.

Semaglutide, a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist, improved the type 2 diabetes mellitus treatment and obesity by its glucose-lowering and weight loss effects. Although the subcutaneous route provides high bioavailability and stable pharmacokinetics, the development of oral semaglutide makes a significant advancement in peptide therapeutics. However, oral delivery remains limited by an absolute bioavailability of only approximately 0.4-1% under optimized fasting conditions with restricted water intake, whereas systemic exposure is essentially absent in the fed state b…

Acute pancreatitis following semaglutide therapy: a case report.

BACKGROUND: Acute pancreatitis (AP) is a potentially life-threatening condition that can result from various aetiologies. While gallstones and alcohol consumption are the most common causes, medications can also induce AP. In the absence of a clear cause, drug-induced acute pancreatitis (DIAP) may pose challenges for clinicians. This is especially true when addressing unusual and rare causes such as glucagon-like peptide-1 receptor agonists. CASE PRESENTATION: A 41-year-old Saudi man was admitted to the hospital with epigastric pain after receiving subcutaneous semaglutide for weight loss. La…

Semaglutide-Associated Acute Interstitial Nephritis: A Case Report.

Glucagon-like peptide 1 receptor agonists (GLP-1RAs) are being investigated to slow the decline of kidney function in type 2 diabetics with chronic kidney disease (CKD). These agents have proven benefits on cardiac outcomes and all-cause mortality as well as in reducing the incidence of macroalbuminuria. Ours is a case of drug-associated acute interstitial nephritis requiring hemodialysis temporally related to a semaglutide dose increase. This case is unique as the index patient had no underlying CKD. Limited cases of acute kidney injury, superimposed on underlying CKD, in patients taking the …

Impact of semaglutide withdrawal on cardiometabolic profile and physiology of energy balance: Recovery effects after semaglutide termination - The REST trial study protocol.

BACKGROUND: Glucagon-like peptide-1 (GLP-1) receptor agonists such as semaglutide have been shown to induce substantial weight loss and improve cardiometabolic risk factors in patients living with obesity. However, most individuals regain weight after abrupt withdrawal of semaglutide, with reversal of its beneficial cardiometabolic effects. METHODS: We propose a randomized controlled trial to determine whether a gradual dose reduction of semaglutide prior to complete discontinuation is associated with differential changes in weight and cardiometabolic profile as compared to immediate treatmen…
