Radiation therapy has been the usual initial treatment for mediastinal seminoma in the past. However, 25 to 40 percent of patients are not cured by this therapy, and other therapy is needed. Review of case reports show that vinblastine, bleomycin, and cis-platinum are effective therapy for metastatic disease, advanced disease within the chest, or mixed germ cell tumors with nonseminomatous elements. The use of tumor markers and chemotherapy must be integrated into the staging of and therapy for primary mediastinal seminoma.

OBJECTIVE: To review cases of testicular seminoma in Down's syndrome (DS) patients.
METHODS: The case of a patient diagnosed and treated at our institution is reported. The literature is reviewed for previous cases, with attention paid to age, stage, treatment and outcome.

Between 1969 and 1980, 17 patients with bulky retroperitoneal seminoma and 1 patient with bulky mediastinal seminoma were treated with radiation therapy. One patient had resection of gross retroperitoneal disease, and 1 patient had resection of gross mediastinal disease. Fifteen patients received adjuvant irradiation to the mediastinum and paraclavicular lymph nodes, and 1 patient received adjuvant chemotherapy. At last follow-up, 17 of 18 patients (94%) remain free of disease. No patient has sustained treatment-related morbidity or mortality.

An extragenital metastatic seminoma in a patient with Klinefelter's syndrome is described. The increased incidence of many types of malignancies in patients with this chromosomal abnormality and the increased susceptibility of XXY cells to transformation by oncogenic SV-40 virus are discussed. A relationship is suggested circumstantially by these observations.

Primary mediastinal seminoma is a relatively rare tumor usually located in the anterior mediastinum. We report here an extremely rare case of a 66-year-old man with primary seminoma in the middle mediastinum. A physical examination showed lymphadenopathy in the right supraclavicular area. A chest CT confirmed the presence of a tumor occupying the retrotracheal space. A histological examination demonstrated metastatic seminoma from the open biopsy of the lymph node. Presentation of one case of seminoma on cryptorchid testicle which presented with torsion of spermatic cord clinically mimicking an inguinal incarcerated hernia. Despite the infrequency of the picture, the high association rate of neoplastic disease in cryptorchid testis which become twisted and force the adoption of oncological criteria for their treatment is highlighted.

A germinal cells tumor in a male manifested as left supraclavicular mass, without other localisation, is presented. The pathologic anatomy corroborate the diagnosis of seminoma. Very few cases of a germinal cell tumor in the supraclavicular fossa, with a normal testicular exploration, have been reported.
