This case report describes the use of sevoflurane in a 26-year-old woman who presented to a rural critical access hospital emergency department in status asthmaticus and subsequently failed conventional therapy. Although the use of potent inhalation agents in the treatment of refractory status asthmaticus has been documented, there is little written about the use of sevoflurane in this situation. The standard anesthetic agent for electroconvulsive therapy (ECT) has been methohexital. We compared sevoflurane, a short-acting halogenated anesthetic, to methohexital for induction in ECT. Lipodystrophies are acquired or inherited disorders characterized in part by the selective loss of adipose tissue and hypertriglyceridemia. The present case report of 2 sevoflurane anesthetics given to a child with such a disorder, before and after correction of severe hypertriglyceridemia, illustrates the considerable effects of increased serum lipids and decreased fat stores on the pharmacokinetics of sevoflurane.
