Skin ulcers misdiagnosed as pyoderma gangrenosum.

BACKGROUND: Pyoderma gangrenosum is a diagnosis of exclusion, and the misdiagnosis of pyoderma gangrenosum can result in substantial complic … re. RESULTS: Ninety-five patients (49 from our institution and 46 described in the literature) had skin ulcers with a clinical resemblance to pyoderma gangrenosum. The final diagnoses were vascular occlusive or venous disease, vasculitis, cancer, primary infection, drug-induced or exogenous tissue injury, and other inflammatory disorders. Of the 95 patients …

Excision and skin grafting of leg ulcers.

BACKGROUND AND AIMS: Skin grafting is a simple and cheap method of treating leg ulcers, and if done in the early phase it can lower health care costs. This study analyses patients who underwent excision and skin-grafting of leg ulcers in the Department of Plastic Surgery during 1993-1995. MATERIAL AND METHODS: Sixty-five patients were operated on; 17 of them, however, died during follow-up and were excluded from the study. The average follow-up time was 1 year 11 months (range 11 months-2 years 11 months). RESULTS: Ninety per cent of all ulcers were cured in the course of an average hospital

[Treatment of skin ulcers of venous origin].

Ulceration of the skin is the most severe complication of chronic venous insufficiency of the lower limbs. The ulcer reflects a profound impairment of the venous anatomy and function, often the late sequel to a deep venous thrombosis. The available surgical techniques are rarely apt to repair these lesions completely. Medical treatment is palliative, based upon the strict control of superficial venous hypertension (leg drainage and compression) and the rational use of local therapies (ulcer cleanliness, debridement and dressing). The physician must obtain the co-operation of the patient and th

Non-cultivable mycobacteria in ulcers of the skin.

In biopsies of 54 patients suffering from chronic dermatological lesions (mostly ulcers of the skin) acid-fast bacilli were found in 14. In these 14 cases in 4 were lesions caused by M. tuberculosis, in 1 the lesion was caused by M. avium-intracellulare, in 1 the lesion was caused by M. fortuitum and in 2 the lesions were caused by non-cultivable mycobacteria (Feldmann-Hershfield ulcer?). In 2 cases the cultures were heavily contaminated, and the diagnosis remained uncertain. In the remaining 4 cases the mycobacteria were considered occasional isolates without clinical significance.

[Pathomorphology of experimental chronic skin ulcers].

Dynamics of morphological and some histochemical changes was shown on an experimental model of chronic ulcer of the skin (25 guinea-pigs). On the previously epilated and scaridied skin of animals consisting of 4 groups (5 animals in each group) applications of "chromin alkali" with various concentrations of bichromates of potassium and sodium were made; biopsy specimens were taken at 1, 4, 7, 10, 15, 20, 25, and 30 days. The results obtained made it possible to identify 4 stages in the development of chromic ulcer (prenecrotic, necrotic, chromic ulcer, healing), each stage being characterized

Skin surface infrared thermography in pressure ulcer outcome prognosis.

OBJECTIVE: To assess the usefulness of skin surface infrared thermography (SSIT) as a prognostic tool in the treatment of stages III and IV pressure ulcers (PU), with hydrocolloid/hydrogel dressings plus 20 exposures to low-level laser therapy (LLLT), compared with hydrocolloid dressings alone, in a group of long-term bedbound care patients. METHOD: In this comparative study, participants were randomly assigned to group I: PUs treated with specialist wound dressings and laser therapy, or to group II: PUs treated with specialist wound dressings without laser therapy. Thermal imaging sessions w

Buruli Ulcer: Review of a Neglected Skin Mycobacterial Disease.

Buruli ulcer is caused by Mycobacterium ulcerans This neglected disease occurs in scattered foci around the world, with a higher concentration of cases in West Africa. The mycobacteria produce mycolactones that cause tissue necrosis. The disease presents as a painless skin nodule that ulcerates as necrosis expands. Finding acid-fast bacilli in smears or histopathology, culturing the mycobacteria, and performing M. ulcerans PCR in presumptive cases confirm the diagnosis. Medical treatment with oral rifampin and intramuscular streptomycin or oral treatment with rifampin plus clarithromycin for 8

[Marjolin's ulcer in a 54-year-old skin graft].

Marjolin's ulcer (MU) is a malignant degeneration of a chronic wound and spinocellular carcinoma is the most common type. MU is described as being aggressive in nature and carries a high incidence of metastases. A 71-year-old man developed spinocellular carcinoma 54 years after a primary amputation and a construction of a skin graft due to a severe foot trauma. The carcinoma was excised and a forefoot amputation was performed.

Therapeutic effects of pregnancy on a chronic skin ulcer.

OBJECTIVES: Sex hormones strongly influence skin physiology and evidences suggest wound healing as well. Estrogens establish a prohealing setting, and androgens play an antagonist role. METHODS AND RESULTS: We present the case of a young woman in whom pregnancy with its associated hormonal shifts allowed the spontaneous healing of a chronic wound on the left leg with bone exposure, which had followed a car accident and had resisted medical and surgical treatments for 5 years. DISCUSSION: Systemic or topical estrogens may offer alternative options in the therapy of chronic wounds, in particul
