[Epistaxis following skull trauma].

BACKGROUND: Epistaxis is a common problem, which is usually benign in nature. In some cases, however, epistaxis is a symptom of a severe underlying condition. CASE DESCRIPTION: A 20-year-old male was treated conservatively following head injury with craniofacial and base-of-skull fractures. Recurrent episodes of epistaxis occurred some weeks after treatment. Further investigation showed an extradural dissecting aneurysm of the left internal carotid artery (ICA), projecting into the sphenoid sinus. The aneurysm was treated endovascularly by placing a coil in the aneurism and a stent in the art

[Endonasal skull base endoscopy].

INTRODUCTION AND OBJECTIVE: The endoscopic endonasal techniques used in skull base surgery have evolved greatly in recent years. Our study objective was to perform a qualitative systematic review of the likewise systematic reviews in published English language literature, to examine the evidence and conclusions reached in these studies comparing transcranial and endoscopic approaches in skull base surgery. MATERIAL AND METHODS: We searched the references on the MEDLINE and EMBASE electronic databases selecting the systematic reviews, meta-analyses and evidence based medicine reviews on skull

Skull base tumor model.

OBJECT: Resident duty-hours restrictions have now been instituted in many countries worldwide. Shortened training times and increased public scrutiny of surgical competency have led to a move away from the traditional apprenticeship model of training. The development of educational models for brain anatomy is a fascinating innovation allowing neurosurgeons to train without the need to practice on real patients and it may be a solution to achieve competency within a shortened training period. The authors describe the use of Stratathane resin ST-504 polymer (SRSP), which is inserted at different

Controversies in skull reconstruction.

In the early 1980s, it was shown that bone from the skull (membranous bone) maintained its volume to a significantly greater extent than bone from the rib and iliac crest regions (endochondral bone). However, the reason for this enhanced volume maintenance was not clarified for many years. On the basis of this enhanced volume maintenance, cranial bone became the ideal autogenous graft of choice for hard tissue repair. In the ensuing years, the current authors performed a large number of autogenous split skull cranial bone cranioplasties with significant success. However, the lure of an off-the

[Goethe's skull and skeleton].

In 1970 there was a conservation of Goethe's skeleton, housed in the Fürstengruft in Weimar. This procedure was kept secret until 1999. The restoration of the sarcophagus and the conservation of the skeleton have been documented together with many photos in the so-called "Sonderakte Mazeration Goethe" (special file mazeration Goethe). In this special file there are also photos of Goethe's skull and skeleton which will be published here for the first time and analysed under anthropological and palaeopathological aspects. A short description of the skull will be given. The dentition of Goethe at

[Imaging of cloverleaf skull].

The cloverleaf skull is a rare and due to complex craniosynostosis. This malformation can be isolated or associated with extracranial skeletal anomalies. This anomaly has a poor prognosis both in terms of neurological outcome and esthetic appearance. Our report underscores the need to obtain a global study of the skull, the brain and the cerebral vessels with 3D reconstructions and MRI prior to early surgery.

The infant Apert skull.

During early infancy, the Apert skull is literally wide open. It is characterized by a gaping midline calvarial defect that extends almost from the root of the nose through the metopic suture area, anterior fontanelle, and sagittal suture area to a widely patent posterior fontanelle. Only the coronal suture area is prematurely fused. During the first 2 to 4 years of life, bony islands that have formed in the midline enlarge and coalesce, obliterating the midline calvarial defect without any evidence of suture formation.

Skull repair in children.

Skull defects in children may be congenital or acquired after trauma, infection or tumor. For defects that do not close spontaneously a variety of repair (cranioplasty) materials are available including bone, metals and acrylic. Where possible calvarial autogenous bone is preferred, particularly in younger infants, but in older children especially with large defects other methods may be useful.

[Skull lacunae in myelomeningocele].

Lacunar skull was found in 60% of 80 patients with myelomeningocele. Most patients with lacunae were neonates. Lacunar skull is significantly correlated with hydrocephalus, stenosis of aqueduct of Sylvius, and Arnold-Chiari malformation. All infants with myelomeningocele should have roentgenograms of the skull at birth. Lacunar skull is associated with a poor mental prognosis.

Meningioangiomatosis with Skull Erosion.

Meningioangiomatosis (MA) is a rare, poorly studied brain hamartomatous lesion, the etiology of which is not fully elucidated. It typically involves the leptomeninges, extending to the underlying cortex, characterized by small vessel proliferation, perivascular cuffing, and scattered calcifications. Given its close proximity to, or direct involvement of, the cerebral cortex, MA lesions typically manifest in younger patients as recurrent episodes of refractory seizures, comprising approximately 0.6% of operated-on intractable epileptic lesions. Due to the absence of characteristic radiological

[Operative skull base approaches].

CLINICAL ISSUE: Knowledge of skull base anatomy is essential for recognizing the pathological conditions affecting the skull base structures (including cranial nerves and cerebral vasculature), for correct interpretation of their presentation on imaging studies, and finally for proper planning of surgical skull base approaches. PURPOSE OF THE ARTICLE: The main goal of the following work is to present a brief recapitulation of the regular skull base anatomy (with a focus on the bony structures) and based on this, to provide an overview of the most commonly used neurosurgical skull base approac

Anterior skull base fractures.

Anterior skull base trauma evaluation and management has historically been difficult to systematically study secondary to the relative rarity of its occurrence, associated major morbidity and high mortality, and poor radiographic techniques. It has only been recently that improved care has allowed for decreased morbidity and mortality. The advent of computed tomography imaging techniques has led to anatomic characterization and detailed study of injury patterns. This article reviews current techniques for evaluation and management of the associated sinonasal, orbital, and neurologic sequelae o

Lateral skull base malignancies.

OBJECT: Historically poor outcomes have been characteristic in patients with lateral skull base malignancies. As advances in skull base surgical techniques have been made, complete resection has increasingly been achieved. This has resulted in improved survival rates and local tumor control. METHODS: The authors performed a retrospective review of 95 patients treated for lateral skull base malignancies. The mean age of the patients was 49.4 years. There were 44 females and 51 males. The mean follow-up period was 50 months. Resection was performed in all patients, and postoperative radiotherap
