Sleep, slow-wave sleep, and blood pressure.

There is increasing evidence that alterations in sleep continuity due to central nervous system arousal and/or reductions in deeper stages of sleep adversely affect blood pressure and contribute to hypertension. Disturbed sleep also blunts the normal nocturnal dip in blood pressure and may lead to sustained daytime hypertension as well. Nocturnal drops in blood pressure result from increased parasympathetic and reduced sympathetic activity during sleep. Slow-wave sleep, considered to be the most "restorative," is the specific sleep state associated with the largest decline in sympathetic activ

Sleep, sleep-disordered breathing and metabolic consequences.

Sleep profoundly affects metabolic pathways. In healthy subjects, experimental sleep restriction caused insulin resistance (IR) and increased evening cortisol and sympathetic activation. Increased obesity in subjects reporting short sleep duration leads to speculation that, during recent decades, decreased sleeping time in the general population may have contributed to the increasing prevalence of obesity. Causal inference is difficult due to lack of control for confounders and inconsistent evidence of temporal sequence. In the general population, obstructive sleep apnoea (OSA) is associated w

Isolated sleep paralysis elicited by sleep interruption.

We elicited isolated sleep paralysis (ISP) from normal subjects by a nocturnal sleep interruption schedule. On four experimental nights, 16 subjects had their sleep interrupted for 60 minutes by forced awakening at the time when 40 minutes of nonrapid eye movement (NREM) sleep had elapsed from the termination of rapid eye movement (REM) sleep in the first or third sleep cycle. This schedule produced a sleep onset REM period (SOREMP) after the interruption at a high rate of 71.9%. We succeeded in eliciting six episodes of ISP in the sleep interruptions performed (9.4%). All episodes of ISP exce

Alzheimer's disease. Sleep and sleep/wake patterns.

Significant changes in sleep/wake patterns, particularly loss of SWS and increased amount and frequency of nighttime wakefulness, apparently occur even at an early stage of the AD process. These disruptions of nighttime sleep increase in magnitude with increasing severity of dementia. While the REM sleep of early stage AD patients is relatively unaffected by the disease process, later stages of AD are marked by significant losses of REM sleep and perhaps more importantly the breakdown of the sleep/wake circadian rhythm with significant amounts of sleep occurring during the day. This daytime sl

The effect of central sleep apnea on sleep architecture in children with obstructive sleep apnea.

OBJECTIVE: This study aimed to investigate how central sleep apnea (CSA) impacts sleep patterns in children with obstructive sleep apnea (OSA). METHODS: Children undergoing polysomnography (PSG) were enrolled and sorted into two groups: those with OSA alone (Group A) and those with both OSA and CSA (CAI <1 nd: children with 10 % CSA or more and less than 50 %, Group B). Statistical analysis was conducted to compare sleep structure and clinical features between Group A and Group B. RESULTS: Group B exhibited significantly higher respiratory events, apnea hypoventilation index, apnea index and

Subjective sleep quality reflects sleep continuity rather than respiratory severity or sleep stage composition.

BACKGROUND: Positive airway pressure (PAP) therapy normalizes respiratory events in obstructive sleep apnea (OSA), yet many patients report poor sleep quality despite adequate physiological control. This suggests that conventional respiratory indices may fail to capture key determinants of subjective sleep experience. We hypothesized that sleep continuity, rather than respiratory severity or sleep stage composition, is more closely associated with perceived sleep quality. METHODS: In this cross-sectional study, adults undergoing in-laboratory polysomnography (PSG), including diagnostic and PA

[Sleep is the best medicine: what internists should know about sleep and sleep medicine].

Sleep is a fundamental biological function that affects nearly all organ systems, yet it has been insufficiently documented and considered in internal medicine practice to date. The aim of this review is to present the physiological basis of sleep, clinically relevant sleep disorders, and their significance for internal medicine conditions, as well as to outline practical approaches to prevention, diagnosis, and treatment. Sleep deprivation and sleep disorders are associated with an increased risk of hypertension, type 2 diabetes mellitus, obesity, cardiovascular events, and neurodegenerative

Effects of sleep deprivation on sleep and sleep electroencephalogram in secretin-receptor knockout mice.

Recent studies has consistently demonstrated a relationship between secretin and autism-like behavior in mice. Therefore, secretin-receptor knockout (SCTR-KO) mice are used to study autism. However, with respect to humans, some studies have reported that secretin administration could improve autistic symptoms in contrast to other studies. A consistent finding revealed that several patients with autism spectrum disorders (ASD) experience comorbid sleep disorders. To examine the relationship between secretin and sleep, we recorded the core body temperature and locomotor activity of SCTR-KO (-/-)

Obstructive sleep apnoea and sleep disorders in pregnancy.

This review provides a summary for obstetricians, midwives, other health professionals and women contemplating pregnancy about the interactions between pregnancy and breathing during sleep. This review will first examine the normal physiological changes of pregnancy and their relationship to sleep-disordered breathing (SDB), and it will then summarise the current knowledge of SDB in pregnancy. Many changes in the respiratory system during pregnancy, particularly during the third trimester, can alter respiratory function during sleep, increasing the incidence and severity of SDB. These changes

Disrupted nighttime sleep and sleep instability in narcolepsy.

STUDY OBJECTIVES: This review aimed to summarize current knowledge about disrupted nighttime sleep (DNS) and sleep instability in narcolepsy, including self-reported and objective assessments, potential causes of sleep instability, health consequences and functional burden, and management. METHODS: One hundred two peer-reviewed publications from a PubMed search were included. RESULTS: DNS is a key symptom of narcolepsy but has received less attention than excessive daytime sleepiness and cataplexy. There has been a lack of clarity regarding the definition of DNS, as many sleep-related sympto

Metabolomics, sleepiness, and sleep duration in sleep apnea.

PURPOSE: Although the mechanism is unclear, daytime sleepiness, a common sequela of obstructive sleep apnea (OSA), has been found to be correlated with a adverse cardiovascular outcomes. Reviewing metabolomics mechanisms of sleep disturbances and cardiovascular disease may help to explain this correlation. METHODS: This review examines the current literature on the relationships between sleepiness, sleep duration, and metabolites in sleep apnea. RESULTS: Although there is a lack of comprehensive literature in this emerging area, existing studies point to a variety of metabolites in different

Sustained sleep fragmentation induces sleep homeostasis in mice.

STUDY OBJECTIVES: Sleep fragmentation (SF) is an integral feature of sleep apnea and other prevalent sleep disorders. Although the effect of repetitive arousals on cognitive performance is well documented, the effects of long-term SF on electroencephalography (EEG) and molecular markers of sleep homeostasis remain poorly investigated. To address this question, we developed a mouse model of chronic SF and characterized its effect on EEG spectral frequencies and the expression of genes previously linked to sleep homeostasis including clock genes, heat shock proteins, and plasticity-related genes

Obstructive sleep apnea and other sleep-related syndromes.

Obstructive sleep apnea syndrome (OSAS) is a common disorder characterized by repetitive episodes of breathing cessation due to complete or partial collapse of the upper airway therefore affecting ventilation. It is quite common, with a prevalence of about 2-4%, has a strong genetic component, and creates a proinflammatory state with elevated TNFα and other cytokines. If untreated, OSA can lead to significant neurological problems that include stroke, cognitive decline, depression, headaches, peripheral neuropathy, and nonarteritic ischemic optic neuropathy (NAION). Treatment reverses some of

Sleep and sleep-wake manipulations in bipolar depression.

In the last 30 years, it has been convincingly demonstrated that sleep in major depression is characterized by disturbances of sleep continuity, a reduction of slow wave sleep, a disinhibition of REM sleep including a shortening of REM latency (i.e. the time between sleep onset and the occurrence of the first REM period) and an increase in REM density. Furthermore, manipulations of the sleep-wake cycle like total or partial sleep deprivation or phase advance of the sleep period have been proven to be effective therapeutic strategies for patients with unipolar depression. The database concernin

Frontal lobe function, sleep loss and fragmented sleep.

Recent experimental studies involving total sleep loss, sleep reduction and clinically related sleep fragmentation report impaired performance on tasks of frontal lobe or executive function, including measures of verbal fluency, creativity and planning skills. Severity of sleep disturbance in obstructive sleep apnoea syndrome (OSAS) is correlated with level of executive impairment, with some residual impairment despite treatment (continuous positive airway pressure - CPAP). Executive impairment appears to be more closely related to hypoxaemic events rather than daytime sleepiness. Studies of e

Sleep bruxism in patients with sleep-disordered breathing.

The aim was to test the hypothesis of a direct association between sleep-disordered breathing and sleep bruxism. The frequency of masseter contraction (MC) episodes and rhythmic jaw movements (RJM) was measured in patients with mild and moderate obstructive sleep apnoea (OSA). The diagnosis of sleep bruxism was made from a combination of questionnaire, clinical observation and all-night polysomnographic recording which included masseter electromyography. A total of 21 patients (19 males/two females, mean age 40.0 years+/-9.2 SD) were randomly selected from a provisional diagnosis of snoring an
