Fifteen patients with the loin pain and haematuria syndrome (LPH) were compared with 10 patients with complicated renal stone disease referred to the same tertiary centre and matched for age, sex and duration of illness. LPH patients had a history of three times more medically unexplained somatic symptoms other than loin pain (p < 0.01) and a higher proportion took analgesics regularly (p < 0.01). The onset of pain was associated with an adverse psychologically important life-event in eight of the LPH patients but in none of the controls (p < 0.02). LPH patients more frequently recalled seriou

We present a case of tabes dorsalis, which consisted of intermittent, sharp pains and diffuse neurological abnormalities, and was initially considered to be a somatoform disorder. The unusual behavioural presentations of neurosyphilis may lead to premature psychiatric diagnoses. It is thus important that psychiatric consultants be aware of the myriad manifestations of the disease.

OBJECTIVE: Two centuries of clinical observations have suggested that conversion symptoms are associated with strong emotions or situations that threaten the individual's physical or psychological integrity. This study tested the hypothesis that childhood conversion reactions reflect the motor-sensory components of two distinct emotional responses (one inhibitory, one excitatory) that develop as adaptations to recurring threats within intimate relationships.
METHOD: Emotional responses to interpersonal threats were assessed in 28 children with conversion disorders using Dynamic-Maturational-M

Consecutive new neurology inpatients and outpatients (N=198) were assessed for somatoform disorders by using the Schedules for Clinical Assessment in Neuropsychiatry. Sixty-one percent of the patients (59% of the female patients and 63% of the male patients) had at least one medically unexplained symptom, and 34.9% fulfilled the diagnostic criteria for an ICD-10 somatoform disorder (27.7% of the male patients, 41.3% of the female patients, 20.5% of the inpatients, and 43.2% of the outpatients). The prevalence figures were about the same when DSM-IV criteria for somatoform disorders were used. …

BACKGROUND: Unexplained medical symptoms are common in children. They constitute the main feature of somatoform disorders of childhood (i.e. pain disorders, conversion disorder and chronic fatigue syndrome or neurasthenia).
OBJECTIVE: To describe assessment and treatment strategies for severe somatoform disorders of childhood.
METHODS: Review of recommendations for clinical practice and clinical trials.
RESULTS: A number of specific techniques have been described. Clinical accounts indicate that many children benefit and can recover. There is some empirical evidence supporting the beneficia…

There is a major need for an empirical evaluation of classification criteria for somatoform disorders. The present study analyzes psychometric properties of the existing criteria for somatization disorder. The full sample consisted of 324 patients seeking help because of "psychosomatic problems." Data from a subsample of carefully diagnosed patients with somatization syndrome (n = 76) and a clinical comparison group (n = 32) permitted the analysis of the discriminative power of items. Twenty-one somatic symptoms adopted from DSM-IV and ICD-10 criteria did not exhibit the necessary psychometric…

Three cases of adolescents with respiratory stridor of psychiatric etiology are described. All three fulfilled DSM-III-R criteria for diagnoses of somatoform disorders and showed characteristics of repressive defense style. Such patients are likely to undergo extensive medical investigation and treatment if the psychiatric nature of their disorder is not recognized, but few data describing their psychological characteristics or treatment exist. Repressors are typically unaware of emotional arousal and do not recognize the negative affects which lead to their somatic symptoms. They therefore re…

BACKGROUND: Patients with persistent somatoform pain disorder (PSPD) are not uncommon. Still, the disease diagnosis relies primarily on structured interviews, with no objective indicators yet available to aid in the diagnosis. This has led to low diagnostic rates and overconsumption of health care resources for the disorder. Although there is a large body of research to improve the diagnosis of the condition, there are currently no objective indicators available for diagnosis.
OBJECTIVES: The aim of this study is to investigate the clinical value of infrared thermography (IRT) for diagnosing

This review describes the conceptual and clinical relations between irritable bowel syndrome (IBS), other functional, somatoform, and mental disorders, and points to appropriate future conceptualizations. IBS is considered to be a functional somatic syndrome (FSS) with a considerable symptom overlap with other FSSs like chronic fatigue syndrome or fibromyalgia syndrome. IBS patients show an increased prevalence of psychiatric symptoms and disorders, especially depression and anxiety. IBS is largely congruent with the concepts of somatoform and somatic symptom disorders. Roughly 50% of IBS pati

BACKGROUND: Clinical limitations of the criteria of somatoform disorders (SDs) have been criticized. However, little objective evidence supports this notion. We aimed to examine the prevalence of SDs in a population with medically unexplained symptoms (MUS), which was expected to have higher probabilities meriting such diagnoses, and to evaluate factors that may influence the clinical judgment of psychiatrists.
METHODS: Data of subjects with MUS (n = 101, 9.5%) as their chief consulting problems, of 1,068 consecutive ethnic Chinese adult medical inpatients referred for consultation-liaison ps…

OBJECTIVE: To highlight key research issues relevant to the diagnostic classification of somatoform disorders and assess whether DSM-V can facilitate research.
METHODS: A selective review of recent research findings in relation to the diagnostic categories of somatization disorder and hypochondriasis.
RESULTS: The DSM-IV criteria have been ignored by researchers as they were either too restrictive or too broad to study the associated features of somatization and hypochondriasis. In view of the linear relationship between number of bodily symptoms and impairment, the appropriate cutoff point …

OBJECTIVE: To evaluate the possible biological aspects of 'unexplained physical symptoms', this study examined serotonergic and noradrenergic monoamino acids in somatoform disorders and/or depression.
METHODS: Blood samples of 150 subjects from 4 groups (somatization syndrome; depression; depression and somatization; controls) were analyzed for amino acids contributing to the serotonergic and noradrenergic system and peripheral muscle energy balance (tryptophan, valine, leucine, isoleucine, phenylalanine and tyrosine).
RESULTS: Tryptophan, branched chain amino acids and other serotonergic am…

Thirty inpatients with somatoform disorders were examined with the structured clinical interview SCID for psychiatric lifetime diagnosis. In the present diagnoses, we found a concordance of 63% for somatoform and affective disorders and the lifetime comorbidity of both disorders was 87%. Additionally, patients with somatoform disorders frequently had a history of other psychiatric disorders (for example, anxiety disorders, 40%). For 73% of patients with somatoform disorders and a history of affective disorders, the onset of the somatoform disorder was prior to the onset of another psychiatric …

OBJECTIVE: To review the available epidemiological evidence on associations between somatoform disorders with anxiety and depressive disorders.
RESULTS: Clinical and population-based studies have found that the co-occurrence of some types of somatoform disorders (e.g., somatization disorder, somatic-symptom-index (SSI)4,6, and pain disorder) and anxiety and depressive disorders is common. These findings may suggest either a causal relationship between these disorders or that they share some common etiological factors. For other forms of somatoform disorders, empirical evidence about co-occurr…

AIM: The prevalence of somatization is dependent on the classification methods and measures used to define it and the context from which the collected data are gathered. The aim of this study was to compare the prevalence of somatization and Somatoform Disorders (SFD) in Primary Health Care.
METHOD: A systematic review of the studies of the prevalence of somatization and SFD in PC (Primary Care) was made, from July 1998 to June 2009. The key words of this investigation, in Pub Med, were; somatoform disorders; somatization; somatisation; medically unexplained symptoms; MUS; functional somatic …

OBJECTIVE: To examine how sociocultural factors shape the professional classification and transformation of neurasthenia (shenjing shuairuo in the Chinese language) in China.
METHODS: We review the relevant literature as well as cite our own research experience on the topic.
RESULTS: We identify three main periods of transformation, namely, the prereform period (before 1980) in which neurasthenia encompassed a wide range of anxiety and mood disorders; the reform period (1980s-1995) when the Diagnostic and Statistical Manual of Mental Disorders-III (DSM-III) and Kleinman's study in Hunan stea

INTRODUCTION: The diagnosis of Somatization Disorder (SD) requires the presence of somatic medically unexplained symptoms (MUS) which must be assessed so that organic diseases may be excluded. SOMS-2 is a self-report measure for SD that assesses medically unexplained symptoms by requiring participants to answer affirmatively and qualify any of the complaints as MUS, only if they have obtained from his doctor the opinion that the said complaint is not due to an organic disease.
PARTICIPANTS AND METHODS: According to the authors, original SOMS-2 has a good internal consistency with Cronbach's a

Migrants from Turkey face several barriers to a successful integration in Switzerland including equal access to the health care system; most important are language problems, cultural and economic barriers. According to the scientific report of the Federal Bureau of Health "Migration and Health " current forms of therapy do not fulfil the needs of this patient population. Ambulatory and in-hospital services are devoted to the acute phase of a pain disorder; they are not imbedded into the circumstances of daily living. Re-Adjusting to daily life however, is crucial for patients' attempt to retur
